This permit, issued on Fehruary 1-,2015 is subject to compliance with the application (where consistent
with the permit), all applicable regulations, special conditions and notes set forth below. Any violation of these terms may
be subject to fines, imprisonment or civil action; or may cause the permit to be null and void.
1) Unless specifically altered herein, this minor modification authorizes the additional fixed platform, ramp, sidewalk, and
gravel parking spaces,all as depicted on the attached workplan drawings and letter.
2) Appropriate sedimentation and erosion control devices, measures or structures shall be implemented to ensure that eroded
materials do not enter adjacent wetlands,watercourses and property(e.g.silt fence,diversion swales or berms,etc.).
3) All disturbed areas shall be properly graded and provided a ground cover sufficient to restrain erosion within thirty days of
project completion.
4) All conditions and stipulations of the active permit remain in force under this minor modification unless specifically altered
herein.
5) This minor modification shall be attached to the original of Permit No. 150-05, which was issued on 1015105, as well as all
subsequent modifications,and copies of all documents shall be readily available on site when Division personnel inspect the

The enclosed permit constitutes authorization under the Coastal Area Management Act,and where
applicable,the State Dredge and Fill Law,for you to proceed with your project proposal.The original(buff-
colored form)is retained by you and it must be available on site when the project is inspected for
compliance.Please sign both the original and the copy and return the copy to this office in the enclosed
envelope. Signing the permit and proceeding means you have waived your right of appeal described below.
If you object to the permit or any of the conditions,you may request a hearing pursuant to NCGS
113A-121.1 or 113-229.Your petition for a hearing must be filed in accordance with NCGS Chapter 150B
with the Office of Administrative Hearings, 6714 Mail Service Center,Raleigh,NC 27611-6714,(919)733-
2698 within twenty(20)days of this decision on your permit.You should also be aware that another
qualified party may submit an objection to the issuance of this permit within twenty(20)days.
The project plan is subject to those conditions appearing on the permit form. Otherwise,all work
must be carried out in accordance with your application.Modifications,time extensions, and future
maintenance requires additional approval. Please read your permit carefully prior to starting work and review
all project plans, as approved.If you are having the work done by a contractor, it would be to your benefit to
be sure that he fully understands all permit requirements.
From time to time,Department personnel will visit the project site.To facilitate this review,we
request that you complete and mail the enclosed Notice Card just prior to work initiation.However, if
questions arise concerning permit conditions, environmental safeguards,or problem areas,you may contact
Department personnel at any time for assistance. By working in accordance with the permit,you will be
helping to protect our vitally important coastal resources.

On 5 October 2005 the Town of Windsor was issued CAMA Major Permit No. 150-05 for the construction of 10
slips on two 8' x 32' docks on the Cashie River in the Town of Windsor,Bertie County.
The CAMA Major Permit was modified on 8 June 2008 to construct a stormwater diversion berm and a sand
beach for kayak/canoe launching. On 18 July 2013 the Major CAMA permit was further modified to construct a 12' x 4'
kayak/canoe launch,a 12' x 4' floating kayak/canoe platform,a step down and 12 formalized boat slips. This modification
was further modified on 23 April 2014 to reconfigure the kayak launch's orientation.
On 29 December 2014, the Washington District received a request to modify and extend the 31 December 2014
permit expiration date for Major Permit No. 150-05 to add an additional fixed 8' x 90' fishing platform, a 7' x 35'
connector ramp,a 5'x 90' concrete sidewalk, and formalize 5-single car gravel parking spaces.
This office recommends that CAMA Major Permit No. 150-05 be modified to install the 8'by 9Q001"platfors,
5' x 90,sidewalk, aVAOgravel parking spaces since it is within the permitted pier alignment, extends less
than 25% of the water body width. -is a note the $200.00 for the modification and extension has been deposited into the
Division's account.

RE: Extension and modification request for Coastal Resources Major Permit No. 150-05 issued
to the Town of Windsor on June 8, 2006.
Dear Mr. Hart,
As designated "Acting Agent" for the Town of Windsor (see attached NCCAMA Agent
Authorization Form dated December 23, 2014), 1 am providing technical assistance to the
town's personnel for implementation of planned improvements for access to the Cashie River
at the town's public boating access at the end of S. Queen Street immediately upstream of the
Roanoke/Cashie River Center, 112 Water Street, Windsor, NC. The following narrative offers
insight to the planned improvements:
PROJECT SETTING:
Site is located at the northeast end of S. Queen Street, approximately 75' north and upstream
of the Town's Public Boat Access; eastward and directly behind commercial development
(WilcoHess Gas Station). Access to S. Queen Street is from Business HWY 17/Water Street.
PROJECT PURPOSE:
The Town of Windsor desires to request an extension and modification to Coastal Resources
Major Permit No. 150-05, issued to the Town of Windsor on June 8, 2006. Previous Minor
modifications were granted on June 8, 2006 and July 18, 2013, with the latest minor
modification having an expiration date of December 31, 2014. All planned work in the previous
Minor modifications has been completed in accordance with the stated applicable regulations,
special conditions and notes.
The Town of Windsor has adopted a cooperative mission of promoting sustainable, community-
driven economic enhancement and stewardship in the region through environmental
education, water-based recreation, and nature-based and cultural tourism. The focal point for RECEIVED

such activities centers around the historical Cashie River, the only river in the nation whose
headwaters and receiving waters are located in the same county.
This project builds on such theme by proposing water-based recreation development on the
Cashie River adjacent the town's S. Queen Street Street Boating Access. Deliverables of this
project will create ADAAG compatible access to the Cashie River and offer ADAAG passive
recreation for fishing the town's spring shad run and waterfront access in general. Water-based
recreational benefits of the project, over and beyond providing direct access to the Cashie
River, include the following:
1) supplemental support of on-going eco-tourism efforts in the region being
implemented by the Town of Windsor to enrich the lives of the community and facilitate the
growth of tourism;
2) expansion of new user groups to the Cashie River with ADAAG compatible access for
individuals with diverse needs for access to a peaceful and relaxing outdoor recreational
environment;
3) support of recent efforts by the town to create and expand water-based recreational
accesses along the Cashie River;
4) collaborative partnership with the NCWRC Cashie River access improvements to be
constructed in the summer of 2015 to provide improved water-based recreational access to the
Cashie River;
5) support efforts of the town as they implement improvements to various town
properties to enhance eco-tourists user groups' outdoor lifestyles and experiences.
PROJECT DESCRIPTION:
The Town of Windsor, Bertie County, North Carolina, in collaborative partnership with the
North Carolina Wildlife Resources Commission (NCWRC), proposes to construct an ADAAG
compatible 8'W x 901 public fishing pier on the Cashie River. Additional components
supporting this passive, water-based recreational project shall be 1) 2-ADAAG concrete parking
spaces, 2) 5-single car gravel parking spaces, 3) TW x 351 boardwalk connector ramp, and 4)
5'W x 901 concrete sidewalk to provide access from parking to the ramp/pier. (see attached

The fishing pier to be constructed approximately 4.5' above the Cashie River and will result in
some localized short-term turbidity during the installation of support pilings. No new docking
slips are planned for this structure. The "shaded impact area" on the Cashie River will be
increased by+825 sq. ft. of river bottom.

ACTION:
The Town of Windsor respectively requests a Minor Modification and Extension of CAMA Major
Permit #150-05 for proposed activities. Planned work is earmarked to begin in the summer of
2015, once all necessary regulatory requirements are addressed.
APPLICABLE FEES
Please find the attached check #5368 dated December 12, 2014 from the Town of Windsor to
NCDENR for the sum of $200.00 to cover costs associated with the Renewal and Minor
Modification request.
If there is additional information required on the Town of Windsor's behalf, please advise. We
