Detailed Observations:
1. Construction operations are not being conducted today (2/16). Construction operations will resume on 2/22/2022.
2. Armor stones (Size A) were observed being delivered to the project site via flatbed tractor trailer from the
Rockingham Vulcan quarry (See Photo #3). All stones appeared to be within the size range for the Size A stone.
3. Construction signs were observed in place and fencing was observed around the 4th Street and Shallotte Blvd
staging and laydown areas.
4. No work was observed at the Shallotte Blvd staging area. Signs and barricades were observed in place.
5. Steel piling to be used for the navigational "Danger" sign at end of structure was measured to be 34 feet long. The
navigational aid signs were also inspected (See Photo #9). All dimensions and attributes of the signs were
compliant with the plans and specs.
6. Waterway survey crew was onsite to perform surveys at the end of the structure.
7. Adam Priest (CPE) and Tom Pierro (CPE) were on site to observe the structure and construction area from 9:30
am to 12:30 pm.
8. Pieces of marine mattress were observed on the beach east of the groin (See Photo #6). Onsite observers removed
some of the pieces. Adam Priest (CPE) informed Richard Mattingly (CDC) about the issue.
9. The onsite observer measured and inspected three stones to ensure they were compliant with the specs (See
Photo #5). All stones observed appeared to be compliant with the specs. The three stones that were measured
were also compliant.

Items Discussed with Contractor's Superintendent, Surveyor, Regulatory Agencies, ... etc.:
1. Adam Priest (CPE) spoke with Richard Mattingly (CDC) over the phone about pieces of marine mattress east of
the groin (See Photo #6). Richard Mattingly stated the crew would be aware of the issue going forward and will
remove pieces in the future. Adam Priest (CPE) spoke to excavator operator about the issue. The excavator
operator could be seen combing the beach for pieces of marine mattress later in the day.
Were any issues identified today which may result in a field adjustment report, contract modification,
request for interpretation, or noncompliance? If so, describe:
1. No issues were discussed today.
