Yes
1. Excavation, marine mattress installation, and armor stone (Size A) installation operations were being conducted
today on the terminal groin between Stations 8+00 and 8+20. (See Photo #2 and #3). The On -Site Observer
observed sixteen (16) marine mattresses being placed and Type A armor stones being placed on top of the
mattresses.
2. Armor stone (Size A) was observed being delivered and offloaded from trucks during the site visit (See Photo #4).
3. No work was observed at the Shallotte Blvd staging area. Signs and barricades were observed in place. All mattress
filling equipment and filling stones had been removed from the area (See Photo #5). Staging area is being used
solely for mattress storage. Reeds and other debris were built up in the back of the staging area pointing to this
area being flooded as well (See Photo #6).
4. The On -Site Observer noted that East 3rd Street and portions of Shallotte Blvd had been buried with sand due to
the storm the day before (See Photo #7, #8, #9, and #10).
5. Evidence of flooding was observed throughout the neighborhood. Trash and built-up sand was observed around
the neighborhood (See Photo #14).
6. The On -Site Observer noted that bulldozers, a backhoe, and pumps were being used by the Town on East 3rd Street
to remove sand, debris, and standing water (See Photo #7).
7. The On -Site Observer noted that significant portion of sand had been removed from the beach due to the flooding.
East of the groin, the dune had been severely diminished and the whole beach was lower in elevation (See Photo
#19). West of the groin, the On -Site Observer noted that more sandbags were visible than before the storm and
that a significant amount of sand had been lost from that area as well (See Photo #20).
8. Spare marine mattress filling stone was observed placed at the entrance to the development (See Photo #16).
9. Pieces of sandbags were seen in a few places on the structure (See Photo #13).
10. A storm impacted the area on January 3, 2022. The storm brought waves measuring up to 8.5' at a buoy located

offshore Sunset Beach (CORMP Buoy Station 41024) with sustained SSW winds of 30 mph gusting to 45 mph. The
high tide on the morning of the January V was predicted to be 6.6' MLLW at Springmaid Pier in Myrtle Beach,
however, the actual high tide was recorded 7.8' MLLW.
Items Discussed with Contractor's Superintendent, Surveyor, Regulatory Alzencies, ... etc.:
Sam Lohman (CPE) talked to Seth (CDC) about the storm that occurred 1/3/2022. Seth (CDC) stated that the waves
were pushing water all the way back to the development and that the wooden mats at the entrance of the job
site were floating. He stated the majority of the neighborhood, and the jobsite was flooded the morning of
1/3/2022 due to the storm.
2. Sam Lohman (CPE) talked to Seth (CDC) about construction operations today (1/4/2022). Seth (CDC) stated that
yesterday (1/3/2022) afternoon the crew cleaned up the jobsite, replaced any rubber mats that had been moved
during the storm, and excavated the sand at the end of the groin to meet the correct grade. This morning the
crew moved armor stones off of the rubber mats and excavted to meet the grade in preperation of placing marine
mattresses.
Were any issues identified today which may result in a field adjustment report, contract modification,
request for interpretation, or noncompliance? If so, describe:
