Was the constructed structure verified to meet the plans and specifications? Yes Measurement: Visual comparison of the UAS orthomosaic with the structure footprint. 17. Was the constructed sheet pile verified to meet the plans and specifications? N/A Measurement: N/A 18. Were any photographs taken? 19. Was the drone flown today? Detailed Observations: Yes Yes 1. Armor stone (Size B) installation operations were being conducted today on the terminal groin. No marine mattresses were installed today. CDC was observed placing stone on top of stone that was placed yesterday (12/16) between Sta. 6+75 and Sta. 7+05 (See Photo #2). 2. No marine mattress filling operations were observed today at the staging area located at the north end of Shallotte Blvd (See Photo #4). CDC stated in their daily report (12/17) that marine mattress filling operations were completed for the project earlier today. 3. Armor stones (Size B) were observed being delivered to the project site via flatbed tractor trailer from the Rockingham Vulcan quarry (See Photo #12). All stones appeared to be within the size range for the Size B stone. 4. Construction signs were observed in place and fencing was observed around the 4th Street and Shallotte Blvd staging and laydown areas. 5. Small and large fragments or chunks of armor stone were observed on top of the structure around Sta 6+75 that appeared to be less than 3.1-ton lower limit of the specification range for Size B armor stone. (See Photos #7 and #8) (See Items Discussed #1). 6. Small rocks and wood chunks were seen at the west side of the intersection of 4th street and Shallotte Blvd. (See Photo #6). 7. Markings from marine mattress operations were seen on the asphalt in the Shallotte Blvd staging area (See Photo #10). 8. Stockpiles of marine mattresses were observed in the Shallotte Blvd staging area. In the very back of the staging area a large stockpile of mattresses was observed. The stockpile consisted of approximately 31 rows with each row contain approximately 6 mattresses (See Photo #3). 9. The onsite observer inspected the intersection of Columbia Street and 4th Street. Most of the intersection was covered in dirt; the asphalt that was visible did not appear to be damaged. The onsite observer also walked down Columbia Street which was partially covered in dirt and did not appear to be damaged (See Photo #15). COASTAL PROTECTION ENGINEERING OF NORTH CAROLINA, INC. Dump trucks were observed backing up on the structure around Sta. 5+00 to dump stones near excavator (See Photo #14). Items Discussed with Contractor's Superintendent, Surveyor, Regulatory Agencies, ... etc.: 1. An email was sent by Adam Priest (CPE) to Richard Mattingly (CDC), Richard Gunn (CDC), and Jim Gunn (CDC) on 12/17 regarding the smaller stones that were placed on the structure near Sta. 6+75. To the On -site Observer these stones appeared to be smaller than 3 tons. Adam Priest (CPE) explained that small stone fragments that are broken due to placement and inadvertently fall into the structure are a part of the construction process; however, stones that do not meet the specifications should not be placed within the footprint of the structure. In response to the email, CDC stated in their daily report (12/17) that the smaller stones around 6+75 were used to ramp up the elevation for the rubber mats and that those stones would be removed during Phase Two of construction. 2. Sam Lohman (CPE) talked Seth (CDC) and excavator operator Steve (CDC) about construction operations today. Steve (CDC) stated that during excavation sand was filling in too fast to continue excavating (See Photo #11). Steve (CDC) stated that the only construction operations that would take place today would be placing armor stones on top of armor stones placed yesterday (12/16) until the correct grade was met between Sta 6+75 and Sta. 7+05. Were any issues identified today which may result in a field adjustment report, contract modification, request for interpretation, or noncompliance? If so, describe: 1. No issues were discussed today.
