Detailed Observations:
1. Marine mattress and armor stone (Size B) installation operations were being conducted today on the terminal
groin. CDC was observed placing stone on top of stone that was placed yesterday (12/14) prior to excavating and
laying eighteen (18) marine mattresses and placing more Size B armor stone on top of the placed mattresses
between Sta. 6+45 and Sta. 6+75 (See Photos #6, #7, and #8)
2. Marine mattress filling operations were being conducted today at the staging area located at the north end of
Shallotte Blvd. Jim Gunn (CDC) noted that the filling operations would be completed this week.
3. Armor stones (Size B) were observed being delivered to the project site via flatbed tractor trailer from the
Rockingham Vulcan quarry. All stones appeared to be within the size range for the Size B stone.
4. CDC provided survey check data for the cross -sections at Sta. 6+00 through Sta. 6+60 indicating that the marine
mattresses were installed to grade.
5. Construction signs were observed in place and fencing was observed around the 4th Street and Shallotte Blvd
staging and laydown areas.
6. Representatives from Vulcan Materials Company (Rock Quarry) were observed on -site today observe the flatbed
stone delivery operations and to discuss the changes that have and are being made at the quarry to increase the
efficiency of the delivery operations.
7. Representatives from the USACE Wilmington District were observed on -site to inspect and observe the terminal
groin construction operations (See Photos #3, #4, and #5).

Items Discussed with Contractor's Superintendent, Surveyor, Regulatory Agencies, ... etc.:
Mickey Sugg (USACE), Greg Currey (USACE), David Bailey (USACE), and Jordan Jessup (USACE) met on -site today
(12/15) with Adam Priest (CPE), Brad Rosov (CPE), and Jim Gunn (CDC) to discuss the progress of the project and
to observe the materials and construction operations. During the site visit the representatives from the USACE,
CDC, and CPE observed and discussed the marine mattresses, rubber blasting mats, armor stone, marine mattress
spreader bar, stone placing equipment, flatbed trucks delivering armor stone, quarry operations and observed
sand being excavated to grade and marine mattresses being installed at the current seaward end of the structure
(Sta. 6+45 to Sta. 6+75).
2. Richard Mattingly (CDC) explained to Adam Priest (CPE) that JLS Contracting (CDC Subcontractor) would likely be
completing the marine mattress filling operations this week and that the filled marine mattresses would continue
to be staged at the north end of Shallotte Blvd.
Jim Gunn (CDC) explained that they will start receiving Size A armor stone on 12/27/21 and are planning to stage
the stone along the southwest portion (adjacent to the sand bags) of the 4th Street staging area. Jim indicated
that CDC would install fencing offset from the sand bags to prevent the stone from being placed to close to the
sand bags.
4. Richard Mattingly (CDC) indicated that the CDC crew would be working through 12/22/21 but would only be
receiving deliveries of armor stone from 12/20 to 21/22 (no work would be performed on the terminal groin).
Richard indicated that the crews would return to work on the terminal groin on 12/27 and would be on -site
through 01/05/22.
Were any issues identified today which may result in a field adjustment report, contract modification,
request for interpretation, or noncompliance? If so, describe:
1. No issues were discussed today.
