Detailed Observations:
1. Marine mattress and armor stone (Size B) installation operations were being conducted today on the terminal
groin. CDC was observed laying eighteen (18) marine mattresses and placing Size B armor stone on top of the in -
place mattresses between Sta. 5+05 and Sta. 5+50 (See Photo #2)
2. Marine mattress filling operations were being conducted today at the staging area located at the north end of
Shallotte Blvd (See Photo #5 & #6).
3. Armor stones (Size B) were observed being delivered to the project site via flatbed tractor trailer from the
Rockingham Vulcan quarry (See Photo #3). All stones appeared to be within the size range for the Size B stone.
4. CDC provided survey check data for the cross -sections at Sta. 4+80 through Sta. 5+40 indicating that the marine
mattresses were installed to grade.
5. The On -site Observer observed several stone fragments lying next to the terminal groin and informed Richard
Mattingly (CDC) (See Photo #11). Richard stated that the fragments would be removed and placed up at the
stockpile area for disposal (See Photo #12).
6. The On -site Observer observed that a flap of the marine mattress was not placed properly and informed Richard
Mattingly (CDC). Richard stated that he is aware of this issue, and they will try to the best of their ability to
prevent that happening going forward (See Photo #7).
7. The On -site Observer observed armor stones being place on rubber mats atop the structure after the groin
construction had concluded for the day (See Photo #14).
Items Discussed with Contractor's Superintendent, Surveyor, Regulatory Alzencies, ... etc.:
1. Richard Mattingly (CDC) sent photos of the damaged road at the entrance to the development to Adam Priest
(CPE). Richard stated the damage was done by another contractor (RH Moore) installing a sewer in the
development with an excavator (See Photo #13).
