This permit, issued on September 29,2020 , is subject to compliance with the application (where consistent
with the permit), all applicable regulations, special conditions and notes set forth below. Any violation of these terms may
be subject to fines, imprisonment or civil action; or may cause the permit to be null and void.
1) This Renewal shall be attached to the original of Permit No. 60-05, which was transferred to the
permittee on 3/21/19, as well as all subsequent renewals, refinements and modifications, and copies
of all documents shall be readily available on site when a Division representative inspects the project
for compliance.
2) All conditions and stipulations of the active permit remain in force under this Renewal.

This permit action may be appealed by the permittee or Signed by the authority of the Secretary of DEQ and the Chair
other qualified persons within twenty (20) days of the issuing of the Coastal Resources Commission.

ADDITIONAL CONDITIONS
Maintenance Clause
3) The Division of Coastal Management shall be notified in writing at least two (2) weeks in advance of
any maintenance work authorized by this permit, and such notification shall include:
A. The number of the original permit.
B. A statement that no dimensional changes are proposed.
C. A copy of the original permit plans with cross-hatching indicating the area to be maintained, the
area to be used for spoil disposal, and the estimated amount of material to be removed. The
location, design and holding capacity of the spoil disposal site shall be approved by a
representative of the Division prior to the initiation of any maintenance dredging activities.
D. The date of map revision and the permittee's signature shown anew on the original plan.
NOTE: The N.C. Division of Water Resources has assigned the proposed project DWR Project No. 08-
071E V4.
NOTE: The U.S. Army Corp of Engineers has assigned the proposed project COE Action Id. No. SAW-
2004-00705.
NOTE: A permit renewal application processing fee of$100 was received by DCM for this project.

Essentially,for renewals,we need the following:
1.Renewal Request—The letter must contain the follow information:
a.Signed and dated letter from the applicant or agent requesting the renewal.
i. If an agent is requesting a renewal,then the appropriate agent authorization form must accompany the request.
b.Applicant's mailing address
c.Permit Number
d.Expiration date of permit[If previously renewed,use the most recent date]
i. If permit has expired as of the date the renewal request letter was received by the field office,then the renewal request may
not be approved for that reason. Please see field staff to verify.
e.Development statements from applicant—The applicant shall submit a letter stating in detail the following:
i. Description of the development that has occurred since the permit was issued;
ii. Their intent to complete the remaining work in reasonable time;
iii. There has been no change in project from previously authorized;
iv. Reasons why the project was not been completed prior to expiration.
f.No change in ownership statement
i. Provide a statement that declares the property is still under the same ownership.(If ownership has changed,then a request for
Transfer is required and the renewal request goes to the appropriate field staff for processing.)
g.Permit compliance statement
i. provide a statement that the development is in compliance with all conditions of the current permit.
h.5-year dredge maintenance excavation request
i. If the request is for a 5-year maintenance excavation renewal,then the request must contain a statement that all other work
authorized in the permit has been completed and only maintenance dredging is requested.
2.Payment
a.A renewal is$100 payable to DEQ in check or money order,or through electronic fund transfer when available-
Cal I Tara if you need anything else.
Also,on another note,I'm still missing information on the Yusko major.You may want to sift back through your email messages(from me)and look for this one.
