NCDOT is requestingforthe issuance of a General Permit .1900 for a temporarycoffer dam dewatering structure
that will be constructed to install a pipe liner in the pipes on NC 94, just south of Kilkenny Road, in Tyrrell
County. The structures wilt be in place for no longer than 4-6 days while the pipe liners are being installed by
Nu Pipe. I have been contacted by our team and have been told that NuPipe will be mobilizing to a nearby area
starting tomorrow and would like to expedite this as quickly as possible. Attached is the Permit Packet containing
the location map, permit drawings, MP-1 Form, and the signed Adjacent Riparian Owner Form waiving any
objections to the proposed activity. Also, if you look at the tax maps for Tyrrell County, one of the pieces of
property belongs to a Mr. Dodge. I was told today that Mr. Dodge is deceased and the property was underway of
being sold and transitioning into Mr. Jeff and Mrs. Edna Gibbs' ownership (the other riparian owners). This
transaction should be finalized in the latter part of 2024. As always, NCDOTwill use our best management
practices for dewatering and construction of the pipe liner. If you have any questions or concerns please feel free

Subject: Request for a CAMA General Permit .1900 for a Temporary Structure on NC 94
in Tyrrell County. WBS # 1B.201511
Dear Mr. Daisey,
The North Carolina Department of Transportation is requesting the issuance of a GP
.1900 permit for the installation of a temporary dewatering structure for a proposed pipelining
activity. This culvert is located on NC 94 in Tyrrell County. The culverts are located
approximately 0.04 miles southwest of Kilkenny Loop Road on an unnamed canal to the
Alligator River. The existing (2) 72"x53' CMP will be dewatered using NCDOT's best
management practices and will then be repaired and lined with an environmentally friendly
concrete liner from the contractor, NuPipe. This temporary coffer dam structure should not be
installed longer than 4-5 days.
NCDOT is also coordinating with the USACOE and NCDWR to acquire a nationwide
permit for the culvert replacement. Included with this letter are the location map, permit
drawings, MP -I form, and adjacent landowner notification forms. If you have any questions,

This repair project would allow the continuance of safe transportation along NC 94 by repairing the existing damaged
culverts with a pipe liner from NuPipe. Once the culverts are repaired, the roadway facility would continue to provide
connectivity and safe access for the traveling public and residents within the area of NC 94.
c. Describe the proposed construction methodology, types of construction equipment to be used during construction, the number of each type
of equipment and where it Is to be stored.
The culverts would be repaired using NCDOT best management practices and construction techniques. The culverts would
be repaired "in the dry" during dewatering of the site. The culverts will be repaired using a concrete pipe liner, which would
be installed by NuPipe.
d. List all development activities you propose.
NCDOT plans on installing a temporary coffer dam on both the inlet and outlet ends of the pipe to dewater the two existing
(73"45W) CMAPs. NuPipe will then repair the pipes as needed and will then line the pipes with an environmental friendly
concrete. After the concrete drys, the temporary cofferdams will be removed to continue water connectivity.

ADJACENT RIPARIAN LANDOWNER STATEMENT
(Tyrrell County: Pipe Linning Activity on NC 94, South of Kilkenny Loop Road)
General Statute 113-229 requires that riparian landowners with property adjoining a
proposed development in an Area of Environmental Concern (AEC) be given ten (10)
days in which to comment on the proposed development. This form allows the adjacent
riparian landowner to express either: (1) that he objects to the project; or (2) that he does
not object and desires to waive his/her right to the 10-day period so that the processing of
the application can progress more rapidly. Of course, the adjacent riparian landowner
need not sign this form at all if he/she so chooses. No response within 10 days
following receipt of this letter and form will be construed to mean the adjacent
riparian landowner has no objections.
