in an area of environment concern pursuant to Section 113A-118 of the
General Statutes, "Coastal Area Management"
Issued to Michael Verreault , authorizing development in the Estuarine Shoreline - ORW (AEC) at Emerald D in Emerald
Isle, as requested in the permittee's application, dated December 10, 2008. This permit, issued on December 22 2008,
is subject to compliance with the application and site drawing (where consistent with the permit), all applicable
regulations and special conditions and notes set forth below. Any violation of these terms may subject permittee to a
fine, imprisonment or civil action, or may cause the permit to be null and void.
This permit authorizes: Replacement of underground concrete wall and paving of driveway.
(1) All proposed development and associated construction must be done in accordance with the permitted work plat
drawings(s) dated received on December 10, 2008.
(2) All construction must conform to the N.C. Building Code requirements and all other local, State and Federal regulations,
applicable local ordinances and FEMA Flood Regulations.
(3) Any change or changes in the plans for development, construction, or land use activities will require a re-evaluation and
modification of this permit.
(4) A copy of this permit shall be posted or available on site. Contact this office at 252-354-3338 for a final inspection at
completion of work.

This permit action may be appealed by the permittee or other qualified persons
within twenty (20) days of the issuing date. From the date of an appeal, any
work conducted under this permit must cease until the appeal is resolved. This
permit must be on the project site and accessible to the permit officer when the
project is inspected for compliance. Any maintenance work or project
modification not covered under this permit, require further written permit
approval. All work must cease when this permit expires on:
DECEMBER 31, 2011
In issuing this permit it is agreed that this project is consistent with the local Land
Use Plan and all applicable ordinances. This permit may not be transferred to
another party without the written approval of the Division of Coastal
Management.

(5) The amount of impervious surface shall not exceed 25% of the lot area within 575 feet of Normal High Water Line
(Estuarine Shoreline - ORW Area of Environmental Concern), in this case, 1065.75 is authorized.
(6) Unless specifically allowed in 15A NCAC O7H. 0209(d)(10), and shown on the permitted plan drawing, all
development/construction shall be located a distance of 30 feet landward of Normal High Water Line. No portion of
the roof overhang shall encroach into the 30 ft. buffer.
(7) All unconsolidated material resulting from associated grading and landscaping shall be retained on site by effective
sedimentation and erosion control measures. Prior to any land -disturbing activities, a barrier line of filter cloth must
be installed between the land disturbing activity and the adjacent marsh or water areas, until such time as the area
has been properly stabilized with a vegetative cover.

CAMA PERMIT NOTICE
Pursuant to NCGS 113A-119(b), Emerald Isle, a locality authorized to issue
permits in Areas of Environmental Concern, through the Coastal Area
Management Agency (CAMA), hereby gives notice that on December 10, 2008
Michael Verreault applied for a Minor CAMA Permit to replace a underground
concrete wall and pave driveway in the Area of Environmental Concern, at 2006
Emerald Dr in Emerald Isle.
The application may be inspected at the address below. Public comments received
by December 22, 2008 will be considered. Later comments will be accepted and
considered up to the time of permit decision. Project modifications may occur
based on further review and comments. Notice of the permit decision in this
matter will be issued upon written request. -

Maintenance and cleaning records should be kept by the system owner and should be available for review by the Carteret County Health Department
during inspections of the system.
*This Operation Permit shall be valid as long as the sanitary sewage is in compliance with Article Ii of G.S. Chapter 130A Laws and Rules
for Sewage Treatment and Disposal Systems 915 NCAC 18A.1900) of all conditions of this permit, including the items specified below.
1. The sewage collection, treatment, and disposal system shall be operated and maintained at all times to prevent public health
hazard, to prevent seepage or discharge of sewage or effluent to the surface of -the ground or surface waters and to prevent
the direct discharge of effluent to the ground water.
2. If a grease trap is installed, it shall be inspected a minimum of 2 to 4 weeks and pumped as needed, with records of maintenance
and cleaning kept by the owner. The grease trap will be required to be pumped and cleaned more frequently during
peak season or periods of high use.
3. This permit does not constitute a warranty and does not negate or supersede any zoning restriction or restricted covenants in
the chain of title. It is the responsibility of the permittee to determine whether or not such restrictions apply.
4. This permit is transferable and is valid only with respect to the facilities described herein and the specified design flow.
Prior to any expansions or revisions, a revised permit shall be obtained from the Carteret County Health Department.
5. Septic tank sludge accumulation shall be evaluated every three (3) years and pumped when sludge level is found to be
12" or greater in depth.
6. No construction of buildings, parking, paving or driveways shall be allowed over system or repair area

10. All ditches, swales, subsurface drainage, and outlets pertinent to the proper functioning of the septic system shall be -"
maintained as required as long as .the system is in use.
11. Garbage grinders are not recommended when a home is served by sub -surface sewage disposal. If a garbage grinder is used then
it is recommended that the septic tank be pumped out annually to prevent the excessive accumulation of sludge.
12. Discharge from water softeners shall not be allowed into the system.
13. All other requirements of Section .1900 NCAC.
The Carteret County Health Department reserves the right to make inspections of these sewage collection, treatment, and disposal facilities as necessary
to assure compliance with the provisions of this -Operations Permit and the North Carolina Laws and Rules for Sanitary Sewage Collection, Treatment
and Disposal. - -

As requested, please find below an update to our previous letter of Dec 3:
• Area of non -pervious areas including garage and all cement slabs = 2426 square feet
(NOT 2860 as stated in the previous letter). Two areas (new concrete driveway slab and
patio) were already included in calculations included on diagram of proposed construction
included with original application.
