This permit, issued on January 182024 1 is subject to compliance with the application (where consistent
with the permit), all applicable regulations, special conditions and notes set forth below. Any violation of these terms may
be subject to fines, imprisonment or civil action, or may cause the permit to be null and void.

1) UN ess specifically altered herein, this Minor Modification authorizes the additi on ofthe lower
platform with swim ladder, all as expressly and specifically set forth in the attached letter and
workplandrawings. Any addfional development activities may require a modification of this permit.

3) In order to protect water quality, runoff from construction shall not visibly increase the amount of

4) Appropriate sedimentation and erosion control devices, measures or structures shall be implemented to
ensure that eroded materials do not enter adjacent wetlands, watercourses or properties. [07H

5) The permittee and/or their contractor shall meet with a representative of the Division prior to project

6) All conditions and stipulations of the active permit remain in force unless specifically altered herein.

NOTE: This Minor Modification shall be attached to the original Permit No. 85-21, which was issued to
the permittee on 7/9/21, as well as all subsequent modifications, refinements, and renewals, and
copies of all documents must be readily available on site when Division personnel inspect the
project for compliance.
NOTE: This permit does not eliminate the need to obtain any additional state, federal or local permits,
approvals or authorizations that may be required.
NOTE: The U.S. Army Corps of Engineers assigned the project Action ID SAW-2021-00513.
NOTE: The Division of Water Resources assigned the proposed project DWR Project No. 2021-1871.
NOTE: A minor modification application processing fee of $100 was received by DCM for this project.
