I am submitting the general permit forms and agent authorization for 128 Gemstone Dr, Sneads
Ferry. The owner is seeking a permit to have a dock and boat lift added to the property.
I have attached copies of :
- Agent authorization form - signed
- Closing disclosure showing the sale is final &Tricia Mearkley is the owner
- Green card and USPS tracking showing proof of delivery to both adjacent neighbors
-A copy of the proposed project drawing

We are working on the ARPO waiver forms today and the docs needed for permit submission. I
was hoping you could let me know if the drawing attached is sufficient or if it needs to be

Hello Heather,
One more question. This property is under contract and set to close on August 3rd.
For the purpose of the agent authorization form & adjacent property owners forms...Do we
need to use the current owners name, she is the one selling and moving or is it ok to use the
purchases name?The purchaser is the one wanting to add a dock after she purchases. If I
use the current owners name is there a way to "transfer" the paperwork or permit into the
new owners name after August 3rd?They are both willing to do whatever is needed, best &
makes most sense to get the paperwork & application started.

You're welcome.The permit is valid for 120 days.With that said,the signed riparian signatures are
valid for 1 year from the date they are signed. If the permit expires and nothing has changed from
the original issuance,they can ask to re-issue the permit using a copy of the original permit and
signatures and would just have to pay the permit fee again.

need to be signed by the owners if they are going to authorize you to obtain the permit.You will
also need the adjacent neighbor on the right and left of the property sign the attached riparian
statements. You would also need to provide them with a drawing (with dimensions) of what is
proposed. Once you have all of this,you can email everything to me. If I determine that what you
are proposing meets the general permit criteria, I will draft the permit and contact you for
payment (check or money order only).Once we receive payment the permit can be emailed for
signature.

this permit has described to me, as shown on the attached drawing, the development they are proposing.
A description or drawing, with dimensions, must be provided with this letter.

Management(DCM) in writing within 10 days of receipt of this notice. Correspondence should be

contacted at(252) 515-5400. No response is considered the same as no objection if you have been
notified by Certified Mail.
WAIVER SECTION
I understand that any proposed pier, dock, mooring pilings, boat ramp, breakwater, boathouse, lift, or
groin must be set back a minimum distance of 15'from my area of riparian access unless waived by me
(this does not apply to bulkheads or riprap revetments). (If you wish to waive the setback, you must sign
