Matthews & Nadia Matthews 1416 Harbour Point Drive West Palm Beach, FL 33410-3419 Telephone: (703) 888-9742 Email: usafnaul983@yahoo.com Attachment to CAMA ARPO Notification -Waiver Form Job Site: 126 North Water Street Point Harbor, NC 27964 JOB DESCRIPTION OF HEAVY-DUTY VINYL BULKHEAD AND BOAT RAMP REPAIRS - Construct two (2) 50 linear foot heavy duty vinyl bulkheads on either side of the proposed boat ramp that will be 15' X 50' and have a 15' bulkhead at the base of the ramp. - Concrete will be 3,500 psi and an 8" thick concrete slab will be formed. - Install 8" x 16' 2.5 marine grade salt treated pilings set on 5'4" centers. - Install 6" x 6" 2.5 marine grade salt treated boards to be installed as upper and lower wales. - All piling to wale points of contact to be bolted with 5/8" galvanized timber bolts. - Install 16' Shoreguard 425 Series vinyl bulkhead sheeting. - Vinyl sheeting to be secured to wales with 3/8" x 3" galvanized lag bolts with two washers. - Bulkhead to be secured to land using 3/4" x 12' galvanized tieback rods set at each piling and will go to a 6" x 6" x 6' deadman. - Each end of tieback rods to have a galvanized OG dock washers installed. - 2 True cut 2.5 marine grade 2" x 8" back band to be installed. - Waterside face of piling countersunk to recess OG washer and nut. - 2" x 6" ground contact salt treated to be attached to land side of vinyl sheeting with 3/8" galvanized carriage bolt installed every 2'. - Top of bulkhead to be capped with 2" x 6" x 18" ground contact salt treated boards and secured with 3-1//4" stainless steel screws. - Install (24) 12 cubic yards of backfill, grade and rake. - A final clean-up of all areas affected by this project will be performed. � .
