Subject: Request for CAMA General Permits for the replacement of Bridge 910 on
SR 1311 Boones Bridge Road in Hertford County. WBS # BP1.R003.1
Dear Mr. Daisey,
The North Carolina Department of Transportation requests the issuance of
CAMA General Permit .2300 for the replacement of Bridge #10 in Hertford County. The
bridge is located on SR 1311 Boones Bridge Road on the Northampton County line and
approximately 1.2 mi. from SR 1351. The bridge crosses the Meherrin River, classified
by NCDWR as C;NSW waters. The existing structure is an 8-span bridge measuring 365
ft. long X 28 ft. wide. This structure would be removed using NCDOT's guidelines for
bridge removal. The new bridge structure would be a 7-span precast concrete cored -slab
bridge; 375 ft. long X 30 ft. wide. An off -site detour would be utilized during
construction. To completely remove the large concrete footings of 3 existing interior
bents, steel sheeting would be used to construct temporary cofferdams for dewatering
during demolition.
The proposed bridge would be placed on the existing alignment using top -down
construction methodology. To ensure the existing bridge can safely accept the weight of
construction equipment during top down construction, 8 temporary crutch bents would be
used for the existing bridge removal. Only two crutch bents would be in place within
jurisdictional waters at any given time to reduce navigational impacts in the river.
In addition, NCDOT requests the issuance of a CAMA General Permit .1600 for
utility relocations. The telecommunications line and overhead power line conflict with
the project, and both would be relocated aerially as shown on the drawings. The
elevation of the proposed relocated lines would be equal to or greater than the existing
line elevation.
Jurisdictional wetland impacts have been avoided and jurisdictional stream
impacts have been minimized as much as practicable, resulting in approx. 40 linear feet
of temp. stream impacts and approx. 1,307 sq. ft. of temp. surface water impacts. This
water body has been designated as an anadromous fish spawning. area by NC WRC and
NCDMF, and an in -water work moratorium would be observed from February 15 — June
30.

Deck drains would not be located over open waters. Storm water would be routed
to deck drains over upland areas and to outlets located on all 4 quadrants of the bridge.
The outlets have been placed at feasible locations that would allow for vegetated buffer
conveyance prior to entering surface waters.

The purpose of this project is to replace an aging bridge structure that has received deficency ratings, qualifying it for
replacement. NCDOT wants to maintain SR 1311 as a safe and reliable travel route for the public.
c. Describe the proposed construction methodology, types of construction equipment to be used during construction, the number of each type
of equipment and where it is to be stored.
The existing bridge will be removed following NCDOT guidelines and BMP's for bridge demolition. The new bridge will be
constructed using a top down methodology, utilizing an off -site detour. The project will be built using cranes, excavators,
backhoes, dump trucks and small equipment associated with bridge construction. To ensure the existing bridge can safely
accept the weight of construction equipment during top down construction, 8 temporary crutch bents would be used for the
existing bridge removal. Only two crutch bents would be in place within jurisdictional waters at any given time to reduce
navigational impacts in the river.To completely remove the large concrete footings of 3 existing interior bents, steel sheeting
would be used to construct temporary cofferdams for dewatering during demolition.

Replace an existing 8-span 365 ft. long X 28 ft. wide bridge with a new 7-span bridge structure measuring 375 ft. X 30 ft. Upon
completion of the structure, NCDOT will have to rebuild the roadway approaches on either end of the bridge. This will involve
some minor widening of the roadway bridge approaches and the roadway shoulders. This will be placed, as shown on the
attached permit drawings. Project will also require the relocation of the power line and telecommunications line, also shown
on the attached permit drawings.

The NC Department of Transportation is proposing to replace Bridge # 10 over the
Meherrin River on SR 1311 & SR 1355 (Boone's Bridge Road) in Northampton and
Hertford Counties. Bridge #10 is currently an 8-span bridge measuring 365 ft. long X 28
ft. wide. The proposed replacement bridge is a single span 375 ft. long X 30 ft. wide
structure. The new structure will be a precast concrete cored slab bridge. This bridge
will be replaced along its existing alignment, as shown in the provided permit drawings.
The overhead power line and telecommunications line conflict with the project, and both
will be relocated as shown on the drawings. An offsite detour will be utilized to maintain
traffic within the project region.
The Division of Coastal Management (DCM) considers the waters at this location to be
"Public Trust Waters" under their regulations. Therefore, it is provided protection under
state law. For this reason, the NCDOT will apply for a permit from DCM to perform the
described work. Part of the permit process is notifying the adjacent landowners of the
proposed work. A map depicting the project area and permit drawings are included with

TIPS FOR ADJACENT RIPARIAN OWNER NOTIFICATION & WAIVER FORMS
CAMA Regulations require notice of proposed development to the Adjacent Riparian Property Owners (15A
NCAC 7J.0204(b)(5)). Proof of actual notice (a sign -off by the owner on this form) or certified mail return
receipts (showing delivery of notice) are needed.
The purpose of this notice is to make adjacent riparian property owners aware of the proposed development
so that they have an opportunity to provide comments (or potentially object to) the proposed development,
and to give DCM an opportunity to consider these comments/objections before a permit decision is made.
"Permission" of adiacent property owners is not necessarily required for DCM to issue a permit; however,
permission must be obtained for certain types of proposed developments if they fall within a 15' setback area
along the riparian access area of the adjacent property, as determined solely by DCM. It is in the applicant's
best interest to provide comprehensive and accurate notice so that any concerns or objections can be
resolved early in the permit review process. In addition, if the adjacent riparian property owner appeals the
permit decision, insufficient notice could be a basis for granting such an appeal.
Who is an Adjacent Riparian Property Owner?
What is Adjacent? (Note: DCM reads this broadly to ensure comment by potentially impacted neighbors)
• A property that shares a boundary line with the site of proposed development; AND
• A property that fronts a natural or manmade waterbody that is connected to coastal waters and
can support some form of navigation, even a kayak or canoe, including a common canal system
or a manmade basin.
• Easement holders? Yes, if the easement could be impacted by the proposed development.
• Streets/Roads? Only if the streettroad could be impacted by the proposed development. This
might include street -ends which might be used for parking and beach access.
• Holders of recognized submerged lands claims/shellfish franchises.
What is Riparian?
Do the boundaries of the adjacent property legally intersect with the water at mean high tide? If there is a
question about whether an adjacent property is considered "riparian," please reach out to DCM Staff -
especially on the oceanfront beach where there may be undeveloped parcels on the beach or in the water.
Who/What is a Property Owner?
For private individuals (or families), send notice to the address listed on the tax card. If the property is owned
by an Inc. or LLC, please send notice to the person listed as the registered agent on the NC Secretary of
State's Corporations Look Up site: https://www.sosnc.gov/search/index/corp. For Condominiums or
neighborhoods with an owners' association (HOA/POA), send notice to the association (which is usually a
corporation, for which you send notice to its registered agent).
What is Notice, and how do I ensure it is received?
• You can meet with your adjacent property owner, provide a description or drawing of the
proposed development, and ask them to sign this form and return it to DCM; OR
• You can hand -deliver this form and a description or drawing, and ask your neighbor to return it to
DCM (consider providing them with a stamped envelope); OR
• You can mail this form by USPS Certified Mail, return receipt requested (the Green Card). If you
choose this option, you must provide either the signed & returned green card OR track the
certified mail package number on USPS.GOV's online tracking system, and provide the tracking
print-out as proof of delivery to DCM

Subject: Request for CAMA GP Modification — Cofferdam Extensions
Bridge #10 on SR 1311 Boones Bridge Road in Hertford County.
WBS # BP1.R003.1
Dear Mr. Daisey,
The North Carolina Department of Transportation requests the modification of
CAMA General Permit .2300 for the replacement of Bridge #10 in Hertford County. The
contractor proposed to change the configuration of the cofferdams utilized for the
removal of the interior bents in the Meherrin River. The contractor stated the limited
accessibility of the previously authorized impervious dikes around each individual bent
would limit demolition of the bents. To provide enough clearance to completely remove
the interior bents, the cofferdams would need to be enlarged. The contractor would use a
"horseshoe" configuration from the top of bank on each side of the river. The horseshoe
cofferdams would be in place sequentially to minimize the impact to the hydraulic flow
of the river. The cofferdams would not be in place at the same time and would be
completely removed when no longer needed.
The revised cofferdam configuration would increase the temp. surface water
impacts to approx. 4,481 sq. ft.(0.100 acre). NCDOT would continue to observe the in -
water work moratorium from February 15 — June 30. This modification would not result
in any other jurisdictional impacts.
Included with this letter are a revised permit drawing and updated impacts table.
