ERIC BOYETTE SECRETARY Subject: Request for CAMA General Permits for the replacement of Bridge 910 on SR 1311 Boones Bridge Road in Hertford County. WBS # BP1.R003.1 Dear Mr. Daisey, The North Carolina Department of Transportation requests the issuance of CAMA General Permit .2300 for the replacement of Bridge #10 in Hertford County. The bridge is located on SR 1311 Boones Bridge Road on the Northampton County line and approximately 1.2 mi. from SR 1351. The bridge crosses the Meherrin River, classified by NCDWR as C;NSW waters. The existing structure is an 8-span bridge measuring 365 ft. long X 28 ft. wide. This structure would be removed using NCDOT's guidelines for bridge removal. The new bridge structure would be a 7-span precast concrete cored -slab bridge; 375 ft. long X 30 ft. wide. An off -site detour would be utilized during construction. To completely remove the large concrete footings of 3 existing interior bents, steel sheeting would be used to construct temporary cofferdams for dewatering during demolition. The proposed bridge would be placed on the existing alignment using top -down construction methodology. To ensure the existing bridge can safely accept the weight of construction equipment during top down construction, 8 temporary crutch bents would be used for the existing bridge removal. Only two crutch bents would be in place within jurisdictional waters at any given time to reduce navigational impacts in the river. In addition, NCDOT requests the issuance of a CAMA General Permit .1600 for utility relocations. The telecommunications line and overhead power line conflict with the project, and both would be relocated aerially as shown on the drawings. The elevation of the proposed relocated lines would be equal to or greater than the existing line elevation. Jurisdictional wetland impacts have been avoided and jurisdictional stream impacts have been minimized as much as practicable, resulting in approx. 40 linear feet of temp. stream impacts and approx. 1,307 sq. ft. of temp. surface water impacts. This water body has been designated as an anadromous fish spawning. area by NC WRC and NCDMF, and an in -water work moratorium would be observed from February 15 — June 30. The purpose of this project is to replace an aging bridge structure that has received deficency ratings, qualifying it for replacement. NCDOT wants to maintain SR 1311 as a safe and reliable travel route for the public. c. Describe the proposed construction methodology, types of construction equipment to be used during construction, the number of each type of equipment and where it is to be stored. The existing bridge will be removed following NCDOT guidelines and BMP's for bridge demolition. The new bridge will be constructed using a top down methodology, utilizing an off -site detour. The project will be built using cranes, excavators, backhoes, dump trucks and small equipment associated with bridge construction. To ensure the existing bridge can safely accept the weight of construction equipment during top down construction, 8 temporary crutch bents would be used for the existing bridge removal. Upon completion of the structure, NCDOT will have to rebuild the roadway approaches on either end of the bridge. This will involve some minor widening of the roadway bridge approaches and the roadway shoulders. This will be placed, as shown on the attached permit drawings. Project will also require the relocation of the power line and telecommunications line, also shown on the attached permit drawings. e. The overhead power line and telecommunications line conflict with the project, and both will be relocated as shown on the drawings. An offsite detour will be utilized to maintain traffic within the project region. ERIC BOYETTE SECRETARY Subject: Request for CAMA GP Modification — Cofferdam Extensions Bridge #10 on SR 1311 Boones Bridge Road in Hertford County. WBS # BP1.R003.1 Dear Mr. Daisey, The North Carolina Department of Transportation requests the modification of CAMA General Permit .2300 for the replacement of Bridge #10 in Hertford County. The contractor proposed to change the configuration of the cofferdams utilized for the removal of the interior bents in the Meherrin River. The contractor stated the limited accessibility of the previously authorized impervious dikes around each individual bent would limit demolition of the bents. To provide enough clearance to completely remove the interior bents, the cofferdams would need to be enlarged. The contractor would use a "horseshoe" configuration from the top of bank on each side of the river. The horseshoe cofferdams would be in place sequentially to minimize the impact to the hydraulic flow of the river. The cofferdams would not be in place at the same time and would be completely removed when no longer needed. The revised cofferdam configuration would increase the temp. surface water impacts to approx. 4,481 sq. ft.(0.100 acre). NCDOT would continue to observe the in - water work moratorium from February 15 — June 30. This modification would not result in any other jurisdictional impacts.
