request dated 11/8/91 .
/• This permit,issued on January 13, 1992 /
is subject to compliance with the application(where consistent
with the permit),all applicable regulations,special conditions and notes set forth below. Any violation of these terms may be
subject to a fine,imprisonment or civil action; or may cause the permit to be null and void.

1) NC DOT shall develop a SAV mitigation plan within one year of the
project's completion if it is determined by the National Marine Fisheries /
Service and the U. S. Fish and Wildlife Service that SAVs have not
/ sufficiently re-established within the project area. .
/
/
2) All dredging for the construction of a work channel is hereby deleted. /
All reference to the excavation of a work channel in the project's plats /
and application is no longer applicable. ,
/
/
3) Upon selection of a method of construction by the D.O.T. , detailed work /
plans of such method will be submitted to the N. C. Division of Coastal /
Management for review and, if appropriate, modification of this permit to /
/ authorize the method of construction. /

' This permit action may be appealed by the permittee or Signed by the authority of the Secretary of DEHNR and the '
' other qualified persons within twenty (20) days of the Chairman of the Coastal Resources Commission. /

ADDITIONAL CONDITIONS
i
4) The riprap material to be used for sedimentation and erosion control will
be clean and free of any pollutants, except in trace quantities.
•
5) The activity will be conducted in such a manner as to prevent a
•
significant increase in turbidity outside the area of construction or
construction-related discharge. Increases such that the turbidity in the
waterbody is 50 NTUs or less are not considered significant.
6) Appropriate erosion and sedimentation control devices, measures, or
structures shall be used to prevent sediment from entering the adjacent
wetlands/watercourses/property (e.g. , silt fence, diversion swales/berms,
sand fence, etc. )
7) A ground cover sufficient to restrain erosion will be provided within 30
working days of completion of any phase of grading on exposed slopes.
8) In the event that significant archaeological resources are present within
the boundaries of the authorized project, the permittee shall develop a
cultural resources management plan in consultation with the Division of
Archives and History for the effective long-term management of such
resources.
NOTE The Division of Environmental Management has authorized this project
under General Water Quality Certification No. 1734.
