• This permit,issued on May 29, 1992 , is subject to compliance with the application(where consistent /
with the permit),all applicable regulations,special conditions and notes set forth below. Any violation of these terms may be
subject to a fine,imprisonment or civil action; or may cause the permit to be null and void.
1 ) This permit amendment authorizes the construction method presented in
the T.A. Loving letter and attachments dated 4/27/92.

2) Attach: this amendment to the permit issued on 1/13/92.
3) All conditions and stipulations of the permit amended on 1/13/92 apply '
/ to the project under this amendment. '

4) In addition to the requirements of condition No. 3, above, there may
/
be no excavation or filling of wetland or waters of the Pasquotank River
, without a modification of this permit. '

' This permit action may be appealed by the permittee or Signed by the authority of the Secretary of DEHNR and the '
' other qualified persons within twenty (20) days of the Chairman of the Coastal Resources Commission. '

RE: CAMA Permit No. 112-91
Dear Mr. O'Quinn:
This letter refines CAMA Major Permit No. 112-91, Perquimans
River Bridge Project, to authorize the use of 27" steel beams
supported by timber crane mats placed in wetlands, to bridge from
high ground on the west bank of the Perquimans River, to the work
barge, as depicted in the June 15, 1992 plat submitted by your
contractor, T.A. Loving Company.
