Due to the isolated nature of the site, a field investigation was not accomplished until March 17, 1989. Based on my observations at the time of this visit, the application was certified complete on that date. The projected deadline for making a decision is May 31, 1989. An additional 75-day review period may be necessary and if so, you will be advised of the extension and the new deadline. A processing fee of $100.00 is required for this project. Please forward a check for that amount to Mr. John Parker, DCM. NCGS 113A-119(b) requires that Notice of an application be posted at the location of the proposed development. Enclosed you will find a "Notice of Permit Filing" postcard which must be posted at the site of your proposed project. You should post this card at a conspicuous point along your property where it can be observed from a public road. (D) Total Area Disturbed: Approx. 2550 s.f. 8. Project Summary The applicants are proposing to replace an existing timber bridge with a prestressed cored slab structure. N.C. DOT-DOH Page two 9. PROJECT DESCRIPTION The proposed project is located on the county line between Duplin and Pender Counties. The existing creek at the time of the visit had a moderately swift flow and appeared to be navigable year round. The width of the creek at the location of the existing and proposed structure is approximately 45' . There is an existing barrow pool in the northeast quadrant of the crossing (inside Duplin County) . This site is well inland and no significant coastal resources are present. The existing structure rests on a series of timber pilings. The travel way shows signs of deterioration. The applicant proposes to replace the existing structure with approximately 208 total feet of prestressed cored slab approximately 30' wide. An area within Duplin County approximately 120' x 87' will be filled with approximately 520 cubic yards of material. This is within a "404" wetland area and a categorical exclusion letter has been issued for this action. 10. ANTICIPATED IMPACTS Provided that the existing navigation within the creek is not blocked, no adverse impacts will result from the proposed activity. Submitted by: C.
