The enclosed pennit constitutes authorization under the Coastal Area Management Act,and where applicable,the State
Dredge and Fill Law,for you to proceed with your project proposal.The original(buff-colored form)is retained by you and it
must be available on site when the project is inspected for compliance.Please sign both the original and the copy and return the -
copy to this office in the enclosed envelope.Signing the permit and proceeding means you have waived your right of appeal
described below.
If you object to the permit or any of the conditions,you may request a hearing pursuant to NCGS 113A-121.1 or 113-229.
Your petition fora hearing must be filed in accordance with NCGS Chapter 150B with the Office ofAdministrative Hearings,P.O.
Drawer 11666,Raleigh,NC 27611,(919)733-2698 within twenty(20)days ofthis decision on yourpemut You should also be aware
that if another qualified party submits a valid objection to the issuance of this permit within twenty(20)days,the matter must
be resolved prior to work initiation.The Coastal Resources Commission makes the fmal decision on any appeal.
The project plan is subject to those conditions appearing on the permit form.Otherwise,all work must be carried out
in accordance with your application.Modifications,time extensions,and future maintenance require additional approval.Please
read your permit carefully prior to starting work and review all project plans,as approved.If you are having the work done by
a contractor, it would be to your benefit to be sure that he fully understands all permit requirements.
From time to time,Department personnel will visit the project site.To facilitate this review,we request that you
complete and mail the enclosed Notice Card just prior to work initiation.However,if questions arise concerning permit
conditions,environmental safeguards,or problem areas,you may contact Department personnel at any time for assistance.By
working in accordance with the permit,you will be helping to protect our vitally important coastal resources.

CAMA Major Permit No. 45-94 was issued to the N. C. DOT for the replacementtof
of
bridge No. 88 on SR 1122 over Bee Tree Canal in Tyrrell County, North Carolina. I support
renewal of the permit.
The $50 fee to cover the cost of the'renewal pas been deducted from the outstanding
balance that DOT has with us. This leaves the iia°laife at $50. If you have any questions, please
give me a call.

The existing CAMA major permit, issued on March 25, 1994 and refined on
July 21, 1997, will expire on December 31, 1997. Since construction is
scheduled to begin in 1998 on this project please issue a renewal of the
permit. If there are no objections, please deduct the $50.00 renewal
application fee from the Department's outstanding credit balance.

Dear Mr. Turner:
This letter is in reference to your 14 July 1997 correspondence requesting a change in CAMA
Major Permit No. 45-94. CAMA Major Permit No. 45-94 was issued to the Department of Transportation
on 25 March 1994 for the replacement of Bridge No. 88 on SR 1122 over Bee Tree Canal in Tyrrell
County, North Carolina.
The NC DOT is requesting that the permit be modified to change from the permitted bridge to the
design depicted in the attached plats dated 7 July 1997. The modified plan shows that the bridge proposed
in the plat dated 7 July 1997 is identical to the one originally permitted except that the new design
maintains the existing vertical clearance at ±5.1'. The original bridge that was permitted by CAMA Major
permit No. 45-94 had a proposed vertical clearance of ±4.7'.
This letters serves as a refinement to CAMA Major Permit No. 45-94 reflecting the aforementioned
change. Your modified plats dated 7 July 1997 will become a permanent part of the file and construction
should be carried out accordingly. All previous permit conditions including the 31 December 1997
expiration date still apply and remain unchanged. If you are unable to complete construction by the 31
December 1997 date you should apply to renew the permit for a two year time frame and include a check
for $50 to cover the cost of renewal with the request.
