Bonner Bridge over Oregon Inlet: (a) the construction of one additional temporary work trestle of approximately 11,400 square feet east of the new bridge and west of the existing bridge, (b) construction of four breasting lines associated 1 with the additional temporary work trestle(lengths of 150 linear feet, 150 linear feet, 190 linear feet, and 240 linear feet), and(c) construction of two breasting lines associated with staging and crane storage(lengths of 400 linear feet each), all as depicted on the attached workplan drawings. NOTE: The specific development being permitted does not preclude the remainder of the B-2500 i project being built in the Pamlico Sound provided that future development will be constructed in a way that avoids and minimizes impacts to AECs. Department of Transportation Permit No. 106-12 ¢ Page 2 of 4 ADDITIONAL CONDITIONS I2) The authorized activities shall not result in any additional permanent or temporary porary impacts to vegetated iwetlands or areas with subaquatic vegetation,without permit modification. 1 NOTE: The proposed project will increase temporary impacts to surface waters by approximately 0.01 acres. 3) Nothing in this permit authorizes any activity that has not received approval from the National Park Service(NPS)and the U.S. Fish and Wildlife Service(USFWS)for work within the Cape Hatteras National Seashore and Pea Island National Wildlife Refuge. The proposed work shall not commence until the permittee has been issued Special Use Permits from the NPS and the USFWS, if required, and a 1 copy of the Special Use Permits are received by DCM. k 4) The installation and removal of the piles for the new temporary work trestles and temporary breasting lines shall be accomplished by pile driving and/or the use of a vibratory hammer, as specified in the $ modification request Should the q permittee and/or its contractor propose to utilize another type of pile installation, such as jetting,additional authorization from DCM shall be required. t 5) No dredging or excavation shall occur within waters of the State, either by conventional methods or by i prop wash"kicking"with boat propellors,without permit modification. t 1 6) No sewage, whether treated or untreated, shall be discharged at any time from any vessels using the { authorized facilities. Any sewage discharge at the authorized facilities shall be considered a violation of this permit for which the permittee is responsible. Department of Transportation Permit No. 106-12 Page 3 of 4 1 ADDITIONAL CONDITIONS 12) The permittee and/or its contractor shall provide for proper r storage and handlingof all oils,chemicals, P � etc.,necessary to carry out the project. • 13) The temporary placement and double handling of any excavated or fill material within waters or vegetated wetlands is not authorized. This condition also applies to the materials stored on work platforms and removal of the existing bridge, roadway asphalt,and associated materials. • t14) Any waste materials or debris associated with construction,demolition,or other activities shall be . disposed of at an approved upland site or shall be recycled in an environmentally appropriate manner •• provided appropriate authorizations from any relevant state,federal,or local authorities are obtained. k k 15) The permittee shall exercise all available precautions in the day-to-day operation of the facility to prevent waste from entering the adjacent waters and wetlands. 1 16) If it is determined that additional permanent and/or temporary impacts are necessary that are not shown j on the attached workplan drawings or described in the authorized permit application, a permit t modification and/or additional authorization from DCM may be required. In addition,any changes in the approved plan may also require a permit modification and/or additional authorization from DCM. The permittee shall contact a representative of DCM prior to commencement of any such activity for this determination and any permit modification. 17) The permittee and/or his contractor shall contact the DCM Transportation Project Field Representative ; in Elizabeth City at(252) 264-3901 to discuss the requirements of this Minor Modification prior to project initiation. • 18) This permit authorizes only the structures that are expressly and specifically set forth in the permit . application or depicted on the attached workplan drawings. No other structures,whether floating or • stationary, may become a part of the facilities authorized by this Minor Modification without additional authorization from DCM. No non-water dependent uses of structures may be conducted on, in, or over : public trust waters without permit modification. 19) During bridge construction,the permittee shall make every attempt to not impede navigation in the project vicinity. • i 20) No attempt shall be made by the permittee to prevent the full and free use by the public of all navigable waters at or adjacent to the authorized work following completion of construction and demolition 1 activities. I 21) The permittee shall install and maintain, at his expense,any signal lights, signals, signage, or notifications prescribed by the U.S. Coast Guard,through regulations or otherwise,on the authorized structures. At a minimum,permanent reflectors shall be attached to the structures authorized by this Minor Modification in order to make them more visible during hours of darkness or inclement weather. • 22) The N.C. Division of Water Resources(DWR)authorized the proposed project on 11/21/18 (DWR Project No. 20120629 v.6) under General Water Quality Certification No. 3939. This modification is required due to development of a detailed demolition plan for the existing bridge,and the need to ensure the safety of the demolition personnel while also protecting the new bridge infrastructure.The Design-Build Team is continuing to look for ways to minimize impacts through their means and methods of demolition of the existing bridge. Bridge Demolition Temporary Trestle and Associated Breasting Lines The Design-Build Team has determined a need for an additional temporary bridge demolition trestle and associated breasting lines. The additional bridge demolition temporary trestle will be located east of the new bridge and west of the existing bridge. This trestle would serve as a transfer platform for the existing bridge superstructure,substructure and pile pieces before they are transferred to barges during the bridge demolition process. The trestle dimensions represent an—11,400 square foot area and will be 10 feet above the water surface,supported by an estimated 28 temporary hollow steel trestle piles(36"inch diameter piles)totaling approximately 1,008 square feet. The additional bridge demolition temporary trestle will be built from barge- mounted cranes thereby minimizing temporary surface water disturbance. Table 1 states the location of the bridge demolition temporary trestle corner piles. Below are your questions and responses from the DB Team are in blue. What is the pile installation methodology for the trestle and breasting line pilings? The piles for the trestle will be installed by the same method that has been used for the installation of all of the existing trestle currently on the job.A crane will be used along with a vibratory hammer to vibrate each of the piles to the correct elevation. A GPS unit along with a template to ensure location and spacing of the piles is correct. The trestle piles will be "proofed"by striking the pile with a diesel hammer to confirm capacities for equipment use. Breasting piles will be installed in the same manner using the vibratory hammer, however, the use of the diesel hammer is not required. Please provide a typical profile and/or cross-section view for the trestle and breasting line pilings. See the attached three drawings for the trestle and each breasting line location in the file entitled, "Breasting Line Trestle Elevation—Permit". What is the proximity of breasting line S2 to the terminal groin and navigation channel? Please provide a vicinity map that includes the proposed work with the surrounding environmental features(human and natural), including the shoreline,terminal groin, and navigational channel. A plan view showing the location of the south end breasting lines (S-1,S-2)relative to the channel, bridge, and shoreline are shown in the enclosed map entitled, "Bonner Bridge South End Breasting Lines". If the foundation has a pile cap then the cap would be in the water- 1.0'based on the mean high water elevation.so the cap dimensions were used instead of the individual pile sizes(giving us a larger footprint). If the foundation has 54"circular piles with no pile cap then the pile surface area was used to calculate the impact, plus a small area immediately around the pile. **The impacts for the removal of the existing bridge were calculated using the pile window(the extent that the piles project into the water)and not individual pile sizes. The demolition/pile removal in wetlands was considered "Temporary Fill"because"Temporary Excavation"is not a category. NC DEPARTMENT OF TRANSPORTATION *** Impacts for the temporary work trestle,work bridge, and work platform were calculated as pile impacts. The piles being proposed are 36"circular piles The work trestle, platforms,and bridge all require two piles per span and spans will be spaced every 40 feet apart. The boardwalk impacts are calculated as the entire footprint of the 10'wide boardwalk. Impacts for the trestle and breasting lines installed to remove the existing bridge were calculated as 36"pipe piles with the number of piles needed called out on the plan sheet. The impact has been added to the total shown under the existing bridge demo category. DIVISION OF HIGHWAYS **** The underground utlility relocation going to be installed by bore and jack and the temporary weland impacts are for the bore pit needed to install the utility lines' and the footprint of the temporary riser poles. The permanent fill in wetland hatching represents the permanent riser pole structure footprint. If the foundation has 54"circular piles with no pile cap then the pile surface area was used to calculate the impact,plus a small area immediately around the pile. **The impacts for the removal of the existing bridge were calculated using the pile window(the extent that the piles project into the water)and not individual pile sizes. The demolition/pile removal in wetlands was considered 'Temporary Fill"because"Temporary Excavation"is not a category.
