Subject: Hydraulic Line connection failure on crane pile driving operation- Bonner Bridge
Agencies,
I am notifying you all in accordance with the permit requirements of an incident that occurred yesterday afternoon on
the Bonner Bridge work trestle. During the pile driving operation, a threaded hydraulic line connection failed and
approximately 15 gallons of biodegradable hydraulic fluid was lost into the water.The connection was between the
vibro hammer and the power pack. PCL's incident report documents a leak noticed around the threaded connection, at
which point the operation was stopped and the connection was tightened to stop the leak.The operation resumed and
during the next pile driving operation the connection completely separated under pressure.The foreman immediately
went to the power pack and shut the motor down.The on-site personnel immediately deployed a spill kit boom and
pads to absorb the fluid that discharged to the water surface.The contractor has inspected the threaded fittings on the
power pack and replaced any that had a manufactured thread defect,which was identified as the failure.The boom and
pads were cleaned up and disposed of properly. If you have any questions about the report or need any more

They were working on the north side pile installation around Bent#74.This is a section over the marsh area.
