I. PROJECT SCOPE/SUMMARY OF WORK A. Project Scope Project scope for New Hanover County Bridge No. 29 Over Smith Creek Demolition includes: Demolish and remove Bridge No. 29. B. HAZARD ANALYSIS IDENTIFICATION An Activity Hazard Analysis will be done for each phase of work on New Hanover County Bridge No. 29 Over Smith Creek Demolition, including demolition. Activity Hazard Analysis is a tool used to review work methods and procedures and identify any associated hazards. These hazards may be known prior to the start of work, or may be manifested during the course of construction operations. Once the hazard has been identified, the best means of control will be developed and implemented. In the event that there are hazardous abatement/demolition activities, 4 Seasons Demolition, Inc. will require that subcontractors involved with hazardous abatement/demolition activities submit a separate Hazard Analysis. C&D materials with lead paint and/or mold will be demolished with the building under wet conditions and transported to a schedule D landfill. On each shift there will be a trained competent person overseeing demolition operations. IV. UTILITIES/OUTAGES All outages required will be requested in writing prior to the outage, as required. For New Hanover County Bridge No. 29 Over Smith Creek Demolition, coordination with the proper authorities, and other required entities and all required Lock Out/Tag Out Procedures are required to be followed. V. DEMOLITION ACTIVITIES Engineering Survey Prior to the commencement of demolition activities, the Competent Person will determine the condition of all items scheduled for demolition, to prevent injury or damage to surrounding facilities. Demolition shall be performed in accordance with the contract documents. Utilities Existing site utilities that are indicated on the contract documents will be protected from damage by whatever means deemed necessary. Utility services such as electrical service, telephone service,water, gas, steam lines, and sewer services affected by demolition will be cut and/or turned off in appropriate locations by others. Mechanical systems or services below grade to be removed shall be exposed by proper, safe methods to protect surrounding utilities. Disconnection of utility services will be performed in accordance with the approved schedule. Safety All workers on the site will have and use any safety devices necessary to prevent their injury (i.e.: safety glasses, hard hats, gloves, vests, etc.). All workers will be trained in the use of tools and equipment which they are required to use. Any opening which could create a hazard will be demarcated. Traffic control devices will be installed if and where appropriate and flag persons will be used, when necessary. All holes, pits, trenches, and similar openings that may occur as a result of demolition will be back filled in eliminate any hazard. Safety of pedestrians or other personnel in the near vicinity of the demolition activities shall be assured by the use of barricades and other restrictive devices, as deemed necessary. A flag person/guard will be posted in any area where it may be necessary to monitor and warn pedestrian traffic of any potential hazards. Workers and pedestrians who may be in the path of debris or harm, will be removed from those areas. Advanced notice will be given to the General Contractor prior to the beginning of any such work. Adequate protection by barricades and/or protective coverings will be provided for all electrical services, mechanical services, and utilities at the project site. Demolition Demo Upper Bridge Structure: Utilize cable cranes on land from each side of the bridge to dismantle the bridge structure above the deck. Workers will access the structure using a boom lift. Using an oxyacetylene torch, the structure will be cut into manageable size pieces. Workers will access the structure using a boom lift. The crane will place the sections of the bridge in the designated processing and loading area on land. After the sections are processed the material will be transported to the proper recycling facility. Demo Bridge Deck: Method 1 Utilize mini excavators with hydraulic hammers to break the concrete free from the supporting metal structure below. Once the concrete is free it will be removed using a mini excavator with bucket and thumb attachment. Smaller pieces will be removed by hand labor. The concrete will be loaded into dump trucks and transported to the proper recycling facility. After all of the concrete is removed, the steel deck support structure will be cut into manageable size pieces using an oxyacetylene torch. Method 2 Utilize a rope saw to cut the deck (steel and concrete) into manageable size pieces. Access Workers will access the structure utilizing several methods: from the ground and/or barge, using a boom lift, and using a man basket attached to the crane. Removal After each section is cut free, the crane will place the sections of the bridge in the designated processing and loading area on land. The fender system and wood piles will be removed using an excavator with bucket and thumb attachment. The debris will be loaded onto the barge and later transferred to the designated processing and loading area on land. After the debris is processed it will be transported to the proper recycling facility. Demo Concrete Foundations and Piles: Workers will access the concrete structures utilizing several methods: from the ground, using a boom lift, using a man basket attached to the crane and from the barge. The concrete over land will be removed using excavators with hydraulic hammer attachments. The concrete over water will be cut into manageable size pieces using a cable rope saw. The crane will place the sections of concrete in the designated processing and loading area on land. The concrete piles will be removed using a VIBRO head attached to the crane. The crane will place the piles in the designated processing and loading area on land. After the concrete is processed the material will be transported to the proper recycling facility. Pollution Control Dust and noise resulting from the demolition operations will be controlled in accordance with the environmental regulations so that the minimum inconvenience to the Owner's operations is caused. All Demolition will be performed under wet conditions. The slurry from concrete cutting and smaller pieces of concrete and debris from demolition will be contained utilizing tarps, netting systems, fabric, and positioning the barge under the work area when possible. All Federal, State, and local regulations will be complied with regarding demolition, hauling, and disposal. All other requirements of the specifications will be strictly enforced and adhered to, as they related to environmental protection, protection of existing work, and disposal of material and existing utilities. The scope of work includes: A. Contractor's Staff Mr. Chuck Ohnmacht, On-site Environmental Manager, will be responsible for waste prevention and management. They shall enforce competent waste management procedures when sizing, cutting and installing products and materials. They will use all practical resources to divert construction and demolition waste from landfills and incinerators and to promote their recycling or reuse. Their responsibilities also include the management of construction and demolition waste for all subcontractors, vendors, and suppliers to participate in the effort. B. Education 4 Seasons Demolition, Inc. shall assign the project superintendent or representative to oversee and enforce proper solid waste management procedures and practices. As education is key to the success of this plan, the following Best Management Practices (BMPs)will be implemented: • Instruct employees and subcontractors on identification of solid waste and hazardous waste. • Educate employees and subcontractors on solid waste storage and disposal procedures. • Hold regular meetings to discuss and reinforce disposal procedures (incorporate into regular safety meetings). • Require that employees and subcontractors follow solid waste handling and storage procedures. • Prohibit littering by employees, subcontractors, and visitors. • Minimize production of solid waste materials wherever possible. Actions Taken to Manage Solid Waste Generation In order to reduce the amount of solid waste generated on the New Hanover County Bridge No. 29 Over Smith Creek Demolition project 4 Seasons Demolition, Inc. personnel and subcontractors will implement the following Best Management Practices (BMP)for Solid Waste Management: • Select designated waste collection areas onsite. • Utilize dump trucks with secure doors and tarp systems to cover the load. • Remove site trash daily, especially during rainy and windy conditions. • Remove solid waste promptly since erosion and sediment control devices tend to collect litter. • Make sure that construction waste is removed and disposed of only at authorized disposal areas. • Hold regular meetings to discuss and reinforce disposal procedures (incorporate into regular safety meetings). • Require that employees and subcontractors follow solid waste handling and storage procedures. • Prohibit littering by employees, subcontractors, and visitors • Minimize production of solid waste materials wherever possible. D. Recycling/Reuse Approaches The Contractor will endeavor to recycle materials when practical. Demolished components during construction will be analyzed and recyclable items will be sorted for transportation to the appropriate recycling facility. E. Cost Estimate The estimated cost of disposal is approximately$7,000.00. This figure is based on calculations of this project's scope of work and from previous projects with similar conditions. This figure will be partially offset by receivables obtained from the efforts of this project's recycling program. The percentage for waste on this site is 7%. F. Disposal Creasoted wood will be disposed of a the Sampson County Disposal, Inc.. Records Records shall be maintained to document: • The quantity of waste generated. • The quantity of waste diverted through recycling. • The quantity of waste disposed by landfill or incineration. The records will be available to the Contracting Officer during construction. Upon completion of the project, a copy of the records will be submitted (unless otherwise required) to the General Contractor as part of the closeout documentation. L. Material Collection and Disposal Dump trucks with secure door and tarp systems will be utilized for transportation for all debris. The following BMPs should be followed: • Littering on the project site should be prohibited. • Collected litter and debris should not be placed in or next to drain inlets, stormwater drainage systems, or watercourses. • Construction debris and waste should be removed from the site as soon as possible. • Construction material visible to the public should be stored or stacked in an orderly manner. The DCM is in agreement with the subject proposal, therefore, please consider this email as the DCM's official authorization of this request. Due to the water line being part of the public water supply system, NCDOT is encouraged to contact Heidi Cox with the NCDEQ-Public Water Supply Section in the Wilmington Regional Office to ensure there are no additional precautions needed when working adjacent to the water line. Please see the attached mark-up of Permit Drawing Sheet 2 of 10. As we discussed during an online Teams meeting last week, our surveyor was able to show the stationing and off-sets which create an area around the existing water line on the east side of the bridge. This area is shown on the attached drawing and shaded in red. Therefore, the department would like to request to be able to cut off(at 1-foot below the bed of the water body) any piles which are within this area. This will include any piles of the existing north and south fender system which are within this area. By cutting off the piles instead of attempting to pull them, we can avoid conflict with, and possible 2 disturbance to,the existing 16"water line which will remain at its current location under the channel. Also attached is the plan view and profile view of the existing water line from the UC plan sheets for your reference. Please let me know if you need any additional information.
