WAIVER SECTION
I understand that any proposed pier, dock, mooring pilings, boat ramp, breakwater, boathouse, lift, or
groin must be set back a minimum distance of 15' from my area of riparian access unless waived by me
(this does not apply to bulkheads or dprep revetments). (If you wish to waive the setback, you must sign
the appropriate blank below.)

DESCRIPTION
We propose to install a new concrete pier consisting of a 5' wide x 220' long
walkway connecting to an 18' wide x 20' long platform.
We propose to install a new 12.5' wide x 16.5' long gazebo roof. This will be a hip
roof having a pitch of no greater than 6/12.
We propose to install a new boat lift with four (4) new 10" x 30' long pilings.
We propose to install two (2) new 10" x 25' long outer mooring pilings.
We propose to construct and install a set of timber steps along the side of the
walkway. This will be constructed using marine grade 4" x 10" timbers and held in
place with driven 8" pilings.
We propose to construct and install a sailboat storage rack. This will consist of 4"
x 6" timbers bolted underneath the walkway and will have two (2) 4" x 6" wooden
bunks.
We propose to install an oyster bag shoreline revetment to be 100 linear feet long
and approximately 3' wide x 20" tall +/-.
