1) This Renewal shall be attached to Permit No. 18-15, which was transferred to the permittee on
12/12/19, as well as all subsequent modifications, refinements, and renewals, and copies of all
documents shall be readily available on site when a Division representative inspects the project for
compliance.
2) All conditions and stipulations of the active permit remain in force under this renewal.
NOTE: The N.C. Division of Water Resources has assigned the proposed project DWR Project No.
14-1242.
NOTE: The U.S. Army Corps of Engineers has assigned the proposed project SAW Action Id. No.
2014-00887.
NOTE: A permit renewal application processing fee of $100 was received by DCM for this project.

This permit must be accessible on -site to Department
personnel when the project is inspected for compliance.
Any maintenance work or project modification not covered
hereunder requires further Division approval.

Dear Mr. Howell:
We would like to ask for a renewal to our Permit Number 18-15. We have been delayed on
construction since the COVID-19 pandemic. Due to increase in materials and the
scarceness of labor, we have had to postpone work on our project for the past two years.
There are no changes to the permit or project. Please renew our permit again, as we are
hopeful to have the project completed before the next expiration date. We apologize for the
delay and inconvenience. Thank you for your time and assistance with our project.
