Any violation of these terms may oe suoJect to tines, unpnsonment or civn action; or may cause me perimi to oe nun ana voia. 1) This minor modification authorizes the additional 140 parking spaces, as depicted on the approved, attached workplan drawings. 2) The Division of Water Quality approval of this project under stormwater management rules of the Environmental Management Commission is covered by way of modification to Stormwater Permit No. SW8951012, which was issued on 6/15/04. Any violation of the permit approved by the DWQ will be considered a violation of this CAMA permit. 3) The Land Quality Section provided approval for this project under the Erosion and Sedimentation Control Plan which was issued on 6/10/04. Any violation of the permit approved by the DWQ will be considered a violation of this CAMA permit. (See attached sheet for Additional Conditions) This permit action may be appealed by the permittee or other qualified persons within twenty (20) days of the issuing date. An appeal requires resolution prior to work initiation or continuance as the case may be. This permit must be accessible on -site to Department personnel when the project is inspected for compliance. Any maintenance work or project modification not covered hereunder requires further Division approval. Permit # 91-96 Page 2 of 2 ADDITIONAL CONDITIONS 4) In order to protect water quality, runoff from construction must not visibly increase the amount of suspended sediments in adjacent waters. 5) Appropriate sedimentation and erosion control devices, measures or structures must be implemented to ensure that eroded materials do not enter adjacent wetlands, watercourses and property (e.g. silt fence; diversion swales or berms, sand fence, etc.). 6) This minor modification does not authorize any new land disturbing activities within 30-feet of the normal high water level. 7) This minor modification must be attached to the original of Permit No. 91-96 which was issued on 6/3/96, as well as all subsequent amendments, modifications, renewals and letters of refinement, and all documents must be readily available on site when Division personnel inspect the project for compliance. 8) All conditions and stipulations of the active permit remain in force under this minor modification unless . modified herein.
