ADDITIONAL INFORMATION In addition to. the completed application form, the following items must be submitted: • A copy of the deed (with state application only) or other instrument under which the applicant claims title to the affected properties. If the applicant is not claiming to be the owner of said property, then forward a copy of the deed or other instrument under which the owner claims title, plus written permission from the owner to carry out the project. • An accurate, dated work plat (including plan view and cross -sectional drawings) drawn to scale in black ink on an 8 1/2" by I1" white paper. (Refer to Coastal Resources Commission Rule 7J.0203 for a detailed description.) Please note that original drawings are preferred an4 only high quality copies will be accepted. Blue -line prints or other larger plats are acceptable only if an adequate number of quality copies are provided by applicant. (Contact the U.S. Army Corps of Engineers regarding that agency's use of larger drawings.) A site or location map is a part of plat requirements and it must be sufficiently detailed to guide agency personnel unfamiliar with the area to the Revised 03195 Form DCM-MP-1 site. Include highway or secondary road (SR) numbers, landmarks, and the like. • A Stormwater Certification, if one is necessary. • A list of the names and complete addresses of the adjacent waterfront (riparian)- landowners and signed return receipts as proof that such owners have received a copy of the application and plats by certified mail. Such landowners must be advised that they have 30 days in which to submit comments on the proposed project to the Division of Coastal Management. Upon signing this form, the applicant further certifies that such notice has been provided. Water is drawn from wells west of New Bern and pumped through transmission lines across the Trent River to the James City community and beyond. Proposed system improvements, which include approximately 62,000 linear feet of 12-inch water main, are designed to remove "bottlenecks" in transmission lines, eliminate low pressure problems in some areas and increase fire flows. Approximately 8,600 linear feet of 12-inch water main is to be installed along the eastern shoulder of SR 1004, parallel to an existing 8-inch line located along the western shoulder. The new main will run from the SR 1167 (Keso Road) intersection across Scotts Creek, attached to the existing bridge, to a point just north of SR 1143. The new line will be constructed using SDR-21 PVC, except for the bridge section which will be Class 50 restrained joint ductile iron pipe. As part of this project, the existing 8-inch main will be abandoned in place for later use as a force main transporting reclaimed water to a proposed land application site near Reedy Branch. The 8-inch line will be disconnected and plugged at SR 1167 and at SR 1143. All services, distribution laterals, and fire hydrants will be disconnected and reconnected to the new 12-inch pipe line. The existing pipe attached to the Scotts Creek Bridge was recently visually inspected. No evidence of damage or leakage was found. The proposed amendment for a land application system near Reedy Branch has been determined to be satisfactory and is hereby approved. The Department of Administration has also determined that the review process for the amendment was adequate, and the requirements of the State Environmental Policy Act have been satisfied. The North Carolina Coastal Federation and concerned citizens of the Deer Run subdivision submitted the enclosed comments to the State Clearinghouse during the review process. No additional information was presented which would alter the selected alternative as advertised. However, it is recommended that Craven County review the comments and work with the citizens to address the local concerns. It is further recommended that Craven County continue to develop and implement appropriate policies and regulatory measures (storm water management, zoning ordinance, failed septic system connections, etc.) to further protect groundwater and surface waters in the area. This contract covers the following: Installation of approximately 13.550 L.F. of 8" PVC reclaimed water force main and conversion of approximately 7,080 L.F. of abandoned 8" water line to reclaimed water force main along Marion Drive, SR 1004, and SR 1144; also appurtenances as shown on the attached plans. APPROVED SUBJECT TO: (1) Notify the District Engineers Office in New Bern (919) 514-4716 prior to beginning and upon completion of work. (2) Workmanship and materials to conform to D.O.T. standards and specifications. Use of Class 200 minimum strength PVC pipe required. (3) Contact the appropriate Utility Company(ies) involved and make satisfactory arrangements to adjust the utilities in conflict with the proposed work prior to construction. (4) Any pavement or drainage structure disturbed or damaged shall be restored to its original condition as directed by the District Engineer. Proper traffic control devices, signs, etc, shall be installed to insure public safety. (6) Trenching, bore pits, and/or other excavations shall not be left open or - unsafe overnight. (7) Fill areas/backfill material to be thoroughly compacted to 95% original density. All trenched material shall be compacted in maximum 6" layers by mechanical tamp or other previously approved method of compaction. Any material removed from the trench/pifthat is contaminated or unsuitable for backfill shall be disposed of off DOT right of way. (8) Dress, seed and mulch all disturbed areas in accordance with Attachment #3. This operation to be performed on a weekly basis. Under no circumstances shall rye grass be allowed (abruzzi or otherwise) for the seeding of areas within the right of way. (9) Encroaching party shall provide D.O.T. with appropriate verification (seed tags, invoices, etc.) of applied seed mixtures. (10) Proper temporary and permanent measures shall be used to control erosion and sedimentation in accordance with all local, State and Federal regulations. 01) When surface areas in excess of one acre will be disturbed, the Encroaching Party shall submit to the District Engineer a Sediment and Erosion Control Plan which has been approved by the appropriate regulatory agency or authority prior to beginning any work on the Right of Way. Failure to provide this information shall be grounds for suspension of operations. (13) All roadway signs are to be replaced before the end of the same work day. (14) Reclaimed water force main under all concrete and asphalt driveways, and State maintained roads within the State right of way crossed by this construction shall be installed per attachments 91 and #2 entitled Special Provisions - Untrenched Construction. Any open cut in contradiction to the above will be allowed only with written permission from the property owner. (15) No open cut of pavement on any state maintained road will be allowed. (16) A construction project to upgrade Marion Drive to acceptable maintenance condition for addition to the state system is scheduled for Spring 1997. A 60-foot right of way has been acquired along the road as staked by NC DOT. Installation shall be a minimum of 13 feet from the proposed centerline of the road, or a maximum of 17 feet from the DOT right of way stakes to avoid possible conflict with this project. (17) A construction project to widen SR 1004 for left turn lanes into SR 1188 and the entrance to Hunters Ridge Subdivision is scheduled in the near future. Installation shall be a minimum of 10 feet from the existing edge of pavement for a distance of 500 feet each side of SR 1188 or the entrance to Hunters Ridge Subdivision to avoid conflict with this project and possible future relocation. (18) Remain 3 foot minimum or 1 foot distance per 1 foot depth from edge of pavement whichever is greater for shoulder installations. (20) Where pipelines are installed in the right of way and are not of ferrous material, a locating tape shall be installed with the pipeline. (21) - In cases where less than minimum bury is necessary or where bores are greater than 6", the pipe shall be protected with a casing. (22) Valves and other appurtenances shall be located at the right of way or to not Interfere with maintenance activities. All appurtenances to be located flush with existing ground elevation. (23) All manholes for air release valves and sewer line shall be of a preapproved design. If any proposed structure is not of a design preapproved by North Carolina Department of Transportation, the second party shall submit details and design calculations signed and sealed by a professional engineer for approval prior to construction. (24) Due to the nature of the proposed work, the Division Engineer may assign an inspector to make periodic inspections as the work progresses, the cost of which shall be borne by the encroaching party. (25) The Department of Transportation does not guarantee the right of way on these roads, nor will it be responsible for any claim for damages brought by any property owner by reason of the installation. (26) No portion of the right of way shall be used for parking equipment during construction. (27) A $15,000 Performance Bond shall be submitted to the District Office prior to beginning of work. Don Evans Mr. Doug Linderman, Ward & Smith Mrs. Linda Teegarden CIU DMU SRF NORTH CAROLINA ENVIRONMENTAL MANAGEMENT COMMISSION DEPARTMENT OF ENVIRONMENT, HEALTH AND NATURAL RESOURCES RALEIGH SPRAY IRRIGATION SYSTEM PERMIT In accordance with the provisions of Article 21 of Chapter 143, General Statutes of North Carolina as amended, and other applicable Laws, Rules, and Regulations PERMISSION IS HEREBY GRANTED TO Neuse River Water & Sewer District Craven County FOR THE continued operation of a wastewater collection system consisting of the routine maintenance of the septic tank effluent pump (STEP) units which are connected to the system along with the associated tanks, pumps, high water alarms, connections, valves, cleanouts, and appurtenances, booster lift stations Nos. 1, 2, 3, 4, and 5 with dual 650 GPM, 310 GPM, 400 GPM, 373 GPM, and 110 GPM submersible pumps, respectively, with approximately 5128 linear feet of 4-inch force main, approximately 3994 linear feet of 6-inch force main, approximately 8778 linear feet of 8-inch force main, approximately 11,900 linear feet of 10-inch force main, and approximately 824 linear feet of 12-inch force main, nineteen (19) cleanouts, twenty-one (21) gate valves and boxes, and the construction and operation of modifications and additions to the existing 0.350 MGD lagoon and spray irrigation facility to provide for a 0.515 MGD reuse spray irrigation facility to include a 1.6 MG aerated equalization basin with dual 900 SCFM, 15 hp blowers; an equalization pump station with dual 300 GPM pumps; dual 0.26 MG aeration basins each with a 25 Hp mechanical aerator; an aeration basin pump station with two 400 GPM pumps and one 800 GPM pump; dual 31 ft diameter clarifiers; a 198 ftZ traveling bridge filter with standby pumps, motors and blowers; (dual 25 ft2 `FUZZY' filters with dual 400 GPM feed pumps and air scour blowers are to be bid as an alternate); a dual channel chlorine contact tankwith dual calcium hypochlorite mixing and feed facilities; continuous turbidity monitoring and recording; autodialer alarm system; ultrasonic effluent flow measuring; a 0.86 MG aerobic digester with dual 25 Hp mechanical aerators; a 5-day side stream storage pond; approximately 15,325 linear feet of 8-inch effluent force main; a 0.260 MG steel reuse water storage tank; and two fixed nozzle spray irrigation fields with a total wetted area of 109 acres consisting of a pump station with dual 1900 GPM pumps and a 20 GPM pressure maintenance pump, approximately 1100 linearfeet of 12-inch manifold, approximately 3600 linear feet of 10-inch manifold, approximately 4500 linear feet of 8-inch manifold, approximately 7600 linear feet of 4-inch irrigation pipe, approximately 3800 linear feet of 3-inch irrigation pipe, approximately 28,000 linear feet of 2-inch irrigation pipe, approximately 2500 linear feet of 1-inch riser pipe, 425 - 12 GPM sprinkler heads, 14 - 8 GPM sprinkler heads, 14 - 13.5 GPM sprinkler heads, planting 6 acres of buffer and 109 acres of spray fields, and 9 monitoring wells; a 125 kW standby generator with automatic transfer switch; and associated appurtenances, site and electrical work and the continued operation of the existing 45 day storage lagoon and effluent chlorination facilities, dual 1000 GPM vertical turbine irrigation pumps with approximately 11,925 linear feet of force main, two (2) fixed nozzle spray irrigation fields with 1,091,178 square feet and 564,538 square feet, respectively, and a linear move spray irrigation system with a wetted area of 2,921,250 square feet, approximately 1170 linear feet of 2-inch water line and all associated piping, valves and appurtenances to serve the Neuse River Water & Sewer District with no discharge of wastes to the surface waters, pursuant to the application received December 2, 1996, and in conformity with the project plan, specifications and other supporting data subsequently filed and approved by the Department of Environment, Health and Natural Resources and considered a part of this permit. This permit shall be effective from the date of issuance until March 31, 2002, shall void Permit No. WQ0003765 issued September 8, 1995, and shall be subject to the following specified conditions and limitations: I. PERFORMANCE STANDARD Upon completion of construction and prior to operation of this permitted facility, a certification must be received from a professional engineer certifying that the permitted facility has been installed in accordance with this permit, the approved plans and specifications, and other supporting materials. Mail the Certification to the Water Quality Permits and Engineering Unit, P.O. Box 29535, Raleigh, NC 27626-0535. 2. The Washington Regional Office, telephone number (919) 946-6481 shall be notified at least forty-eight (48) hours in advance of operation of the installed facilities so that an in -place inspection can be made. Such notification to the regional supervisor shall be made during the normal office hours from 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding State Holidays. 3. The spray irrigation facilities shall be effectively maintained and operated at all times so that there is no discharge to the surface waters, nor any contamination of ground waters which will render them unsatisfactory for normal use. In the event that the facilities fail to perform satisfactorily, including the creation of nuisance conditions or failure of the irrigation area to adequately assimilate the wastewater, the Permittee shall take immediate corrective actions including those actions that may be required by the Division of Water Quality, such as the construction of additional or replacement wastewater treatment and disposal facilities. 4. The issuance of this permit shall not relieve the Permittee of the responsibility for damages to surface or groundwaters resulting from the operation of this facility. 5. The residuals generated from these treatment facilities must be disposed in accordance with General Statute 143-215.1 and in a manner approved by the North Carolina Division of Water Quality. 6. Diversion or bypassing of the untreated wastewater from the treatment facilities is prohibited. 7. The following buffers shall be maintained: a) 400 feet between wetted area and any residence or places of public assembly under separate ownership, b) 150 feet between wetted area and property lines, c 100 feet between wetted area and wells, d) 100 feet between wetted area and drainageways or surface water bodies, e) 50 feet between wetted area and public right of ways, f) 100 feet between wastewater treatment units and wells, and g) 50 feet between wastewater treatment units and property lines. Some of the buffers specified above may not have been included in previous permits for this waste treatment and disposal system. These buffers are not intended to prohibit or prevent modifications, which are required by the Division, to improve performance of the existing treatment facility. These buffers do, however, apply to modifications of the treatment and disposal facilities which are for the purpose of increasing the flow that is tributary to the facility. These buffers do apply to any expansion or modification of the spray irrigation areas and apply in instances in which the sale of property would cause any of the buffers now complied with, for the treatment and disposal facilities, to be violated. The applicant is advised that any modifications to the existing facilities will require a permit modification.. 8. The owner of all houses, buildings or properties used for human occupancy, employment, recreation, or other purposes, situated within the Township and abutting on -any street, alley, or right-of-way in which there is now located or may in the future be located a public sanitary sewer of the Neuse River Water and Sewer District, is hereby required (at the owner(s) expense) to connect such facilities directly with the proper public sewer should the existing nitrification field fail. Application to the NRWSD shall be made within thirty (30) days and connection to the public sewer shall be completed within ninety (90) days after the date of official notification by the Craven County Health Department to do so, provided that said public sewer is within reasonable access of the property line. 9. The influent splitter box weir elevations shall be set so that no more than 1.5 times the average daily flow is sent to the aeration basins. 10. The existing influent booster pump station #1 must be upgraded to provide for a pump capacity of 900 GPM with one pump out of service when the average daily flow for any one month reaches 0.374 MGD. 11. The existing abandoned 8-inch water line may be used as a portion of the reuse water effluent line subject to the following conditions: a. The line shall be tested to insure zero (0) leakage. b. The Neuse River Water and Sewer District shall provide written assurances that no sewer taps shall be made to this line. 12. As required by the Land Quality Section, a minimum of one soils compaction test shall be made for each 5000 cubic feet of compacted lift within the lagoon Z. embankments. II. OPERATION AND MAINTENANCE REQUIREMENTS The facilities shall be properly maintained and operated at all times. 2. Upon classification of the facility by the Certification Commission, the Permittee shall employ a certified wastewater treatment plant operator to be in responsible charge (ORC) of the wastewater treatment facilities. The operator must hold a certificate of the type and grade at least equivalent to or greater than the classification assigned to the wastewater treatment facilities by the Certification Commission. The Permittee must also employ a certified back-up operator of the appropriate type and grade to comply with the conditions of Title 15A, Chapter 8A, .0202. The ORC of the facility must visit each Class I facility at least weekly and each Class II, III, and IV facility at least daily, excluding weekends and holidays, and must properly manage and document daily operation and maintenance of the facility and must comply with all other conditions of Title 15A, Chapter 8A, .0202. Once the facility is classified, the Permittee must submit a letter to the Certification Commission which designates the operator in responsible charge within thirty days after the wastewater treatment facilities are 50% complete. 11 3. A suitable vegetative cover shall be maintained. 4. Irrigation shall not be performed during inclement weather or when the ground is in a condition that will cause runoff. 5. Adequate measures shalt be taken to prevent wastewater runoff from the spray field. 6. The facilities shall be effectively maintained and operated as a non -discharge system to prevent the discharge of any wastewater resulting from the operation of this facility. The application rates on the spray irrigation fields shall be limited as follows: Arthur Farm Site Field No. 1 25.05 Acres - The Northern most fixed nozzles beginning at hydrant No. 3 and extending north to include a total of 40 fixed nozzles shall be limited to 62.4 inches over any twelve (12) month period. Field No. 1 A 16.47 Acres - The linear move system spray irrigation field from the Northern most point to the point halfway between hydrants No. 2 and No. 3 shall be limited to 62.4 inches over any twelve (12) month period. Field No. 2 12.96 Acres - The existing fixed nozzles in old Field No. 2 to include a total of 22 fixed nozzles shall be limited to 39.0 inches over any twelve (12) month period. Field No. 2A 15.29 Acres - The linear move system spray irrigation field beginning at hydrant No. 5 and extending to the Southern most point of the expanded linear move system spray irrigation field shall be limited to 39.0 inches over any twelve month period. Field No. 3 3 5.3 0 Acres - The linear move system spray irrigation field beginning halfway between hydrants No. 2 and No. 3 then extending South to hydrant No. 5 shall be limited to 31.2 inches over any twelve (12) month period. Reedy Branch Site North Field Zone 1 14.4 Acres - The fixed nozzles at the extreme North end of the area North of the subdividing tributary of Reedy Branch consisting of Norfolk & Candor soils to include a total of 52 fixed nozzles shall be limited to 52.1 inches over any twelve (12) month period. Zones 2-4 39.8 Acres - The fixed nozzles in the balance of the area North of the subdividing tributary of Reedy Branch consisting of Candor soils to include a totaf'of 154 fixed nozzles shall be limited to 65.2 inches over any twelve (12) month period South Field Area A 4.0 Acres - The fixed nozzles in the portion of Zones 6 & 7 containing Goldsboro soils along the West boundary to include a total of 14 fixed nozzles shall be limited to 31.7 inches over any twelve (12) month period. Area B 4.0 Acres - The fixed nozzles in the portion of Zones 7, 8, & 9 containing Candor soils along the Southeast boundary of the area South of the subdividing tributary of Reedy Branch to include a total of 14 nozzles shall be limited to 65.2 inches over any twelve (12) month period. Area C 49.3 Acres - The fixed nozzles in the balance of the area South of the subdividing tributary of Reedy Branch excluding the Goldsboro and Candor soils to include a total of 188 fixed nozzles shall be limited to 52.1 inches over any twelve (12) month period. 8. No type of wastewater other than that from the Neuse River Water & Sewer District shall be sprayed onto the irrigation area. 9. No traffic or equipment shall be allowed on the disposal area except while installation occurs or while normal maintenance is being performed. 10. Public access to the land application sites shall be controlled during active site use. Such controls may include the posting of signs showing the activities being conducted at each site. 11. The permittee shall take appropriate actions necessary to reduce the impacts of sodium in the wastewater or in the soil to levels which are conductive to crop growth and maintenance of good soil structure. 12. An agronomic plan shall be developed which maximizes the assimilative capacity of the nutrients in the wastewater through crops grown, for crop rotation, etc. 13. Freeboard in the wastewater lagoon shall not be less than two feet at any time. 14. All wastewater shall be routed to the 5-day holding pond should the limit for fecal coliform (daily maximum concentration of 25 per 100 nil) or turbidity (instantaneous maximum of 10 NTU) be exceeded, until such time that the problems a1.ssociated with the treatment capability of the wastewater treatment plant have been corrected. The wastewater in the 5-day holding pond shall be pumped back to the treatment plant for retreatment or treated in the 5-day pond prior to discharge to the storage pond. III. MONITORING AND REPORTING REQUIREMENTS 1. Any monitoring (including groundwater surface water soil or plant tissue analyses) deemed necessary by the Division of Water Quality to insure surface and ground water protection will be established and an acceptable sampling reporting schedule shall be followed. 2. Monthly averages for all but faceal coliform shall be the arithmetic mean of all samples collected during the reporting period. b. Monthly averages for fecal coliform shall be the geometric mean of all samples collected during the reporting period. C. Daily maximum shall be the maximum value of all samples collected during the reporting period. During the remainder of the year, these parameters shall be monitored monthly. **Triannually sampling shall be conducted during April, August, and December. If Groundwater sampling indicates or predicts problems with the compliance with Groundwater Standards, this permit will be modified to include additional and/or more restrictive limitations. 3. Adequate records shall be maintained by the Permittee tracking the amount of wastewater disposed. These records shall include, but are not necessarily limited to, the following information: a. date of irrigation, b. volume of wastewater irrigated, C. field irrigated, d. length of time field is irrigated, e. continuous weekly, monthly, and year-tn-date hydraulic (inches/acre) loadings for each field, £ weather conditions, and g. maintenance of cover crops. 8 4. A representative soils analysis (Standard Soil Fertility Analysis) shall be conducted in January and July on the spray irrigation fields. These analises shall be representative of the entire irrigation area. The Standard Soil Fertility Anayisis shall include, but is not necessarily limited to, the following parameters: Acidity Manganese Potassium Calcium Percent Humic Matter Sodium Copper pH Zinc Mangesium Phosphorus Base Saturation (by calculation) Cation Exchange Capacity Sodium Adsorption Ratio 5. Three (3) copies of all operation and disposal records (as specified in condition III 3) on Form NDAR-1 shall be submitted on or before the last day of the following month. Three (3) copies of all effluent monitoring data (as specified in condition III 2) on Form NDAR-1 and all soil analyses (as specified in condition III 4) shall be submitted on or before the last day of the following month. All information shall be submitted to the following address: NC Division of Water Quality Facility Assessment Unit PO Box 29535 Raleigh, NC 27626-0535 6. A record shall be maintained of all residuals removed from this facility. This record shall include the name of the hauler, permit authorizing the disposal or a letter from a municipality agreeing to accept the residuals, date the residuals were hauled, and volume of residuals removed. 7. A maintenance log shall be maintained at this facility includung but not limited to the following items: a. Daily sampling results including residual chlorine, settleable matter, and dissolved oxygen in the aeration basin and at the clarifier weir. b. Visual observations of the plant and the plant site. C. Record of preventative maintenance (changing of filters, adjusting belt tensions, alarm testing, etc.). d. Date of calibration of flow measurement device. e. Date and results of power interruption testing on alternate power supply. 8. Noncompliance Notification: The Permittee shall report by telephone to the Washington Regional Office, telephone number (919) 946-6481, as soon as possible, but in no case more than 24 hours or on the next working day following the occurrence or first knowledge of the occurrence of any of the following: f a. Any occurrence at the wastewater treatment facility which results in the treatment of significant amounts of wastes which are abnormal in quantity or characteristic, such as the dumping of the contents of a sludge digester, the known passage of a slug of hazardous substance through the facility, or any other unusual circumstances; b. Any process unit failure, due to known or unknown reasons, that renders the facility incapable of adequate wastewater treatment, such as mechanical or electrical failures of pumps, aerators, compressors, etc.; C. Any failure of a pumping station, sewer line, or treatment facility resulting in a by-pass directly to receiving waters without treatment of all or any portion of the influent to such station or facility; or d. Any time that self -monitoring information indicates that the facility has gone out of compliance with its permit limitations. Persons reporting such occurrences by telephone shall also file a written report in letter form within five (5) days following first knowledge of the occurrence. This report must outline the actions taken or proposed to be taken to ensure that the problem does not recur. IV. GROUNDWATER REQUIREMENTS A. Arthur Farm Site 1. Arthur Farm spray field monitoring wells MW-4, MW-5, MW-6, Lagoon monitoring wells MW-7, MW-8, and MW-9, shall be sampled every April, August, and December for the following parameters: Water Level Nitrate (NO3-N) Chloride Iron Total Dissolved Solids (TDS) Total Organic Carbon (TOC) pH Total Ammonia Sodium Sulfate(SO4) Fecal Coliforms Volatile Organic Compounds (VOC) - In December only using Method 6230D (Capillary Column), "Standard Methods for the Examination of Water and Wastewater", 17th ed., 1989 10 The measurement of water levels must be made prior to sampling for the remaining parameters. The depth to water in each well shall be measured from the surveyed point on the top of the casing. The measuring points (top of well casing) of all monitoring wells shall be surveyed to provide the relative elevation of the measuring point for each monitoring well. If TOC concentrations greater than 10 mg/1 are detected in any downgradient monitoring well, additional sampling and analysis must be conducted to identify the individual constituents comprising this TOC concentration. If the TOC concentration as measured in the background monitor well exceeds 10 mg/I, this concentration will be taken to represent the naturally occurring TOC concentration. Any exceedances of this naturally occurring TOC concentration in the downgradient wells shall be subject to the additional sampling and analysis as described above. The results of the sampling and analysis shall be sent to the Groundwater Section, Permits and Compliance Unit, P.O. Box 29578, Raleigh, N.C. 27626-0578 on Form GW-59 [Compliance Monitoring Report Form] within 30days of sample collection. 2. Any additional groundwater quality monitoring, as deemed necessary by the Division, shall be provided. 3. The COMPLIANCE BOUNDARY for the disposal system is specified by regulations in 15A NCAC 2L, Groundwater Classifications and Standards. The Compliance Boundary for the disposal system constructed prior to December 31, 1983 is established at either ( 1) 250 feet from the waste disposal area, or (2) 50 feet within the property boundry, whichever is, closest to the waste disposal area. An exceedance of Groundwater Quality Standards at or beyond the Compliance Boundary is subject to immediate remediation action in addition to the penalty provisions applicable under General Statute 143-21.5.6A(a)( I ). In accordance withl5A NCAC 2L, a REVIEW BOUNDARY is established around the disposal systems midway between the Compliance Boundary and the perimeter of the waste disposal area. Any exceedance of standards at the Review Boundary shall require remediation action on the part of the B. Reedy Branch Site 1. Within 60 days of permit reissuance, all site characterization observations wells (installed with a drill rig) on the Reedy Branch site shall be abandoned according to 2C.0113 (Abandonment of Wells) in 15A NCAC 2C.0100 Well Construction Standards Rules. 1. 11 2. Prior to beginning waste disposal operations at the Reedy Branch site, nine (9) monitor wells (MW-10 through MW-18) shall be installed to monitor groundwater quality. The wells shall be constructed such that the water level in the well is never above or below the screened (open) portion of the well at any time during the year. The general location and name for each well is marked on Attachment A. Each monitoring well shall be located at the review boundary, _ constructed in accordance with this permit, and approved by the Washington Regional Office. 3. All wells that are constructed for purposes of groundwater monitoring shall be constructed in accordance with 15A NCAC 2C .0108 (Standards of Construction for Wells Other than Water Supply) and any other state and local laws and regulations pertaining to well construction. 4. The Washington Regional Office, telephone number (919) 946-6481 shall be notified at least forty-eight (48) hours prior to the construction of any monitoring well so that an inspection can be made of the'monitoring well location. Such notification to the regional groundwater supervisor shall be made during the normal office hours from 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding state holidays. The Owner shall stake out the Review and Compliance Boundaries prior to making the above notification. Within sixty (60) days of completion of all monitoring wells, the permittee shall submit two original copies of a scaled topographic map (scale no greater than 111:300') signed and sealed by a professional engineer or a state licensed land surveyor that indicates all of the following information: a. the location and identity of each monitoring well, b. the location of all components of the waste disposal system, C. the location of all property boundaries, d. the latitude and longitude of the established horizontal control monument, e. the relative elevation of the top of the well casing (which shall be known as the "measuring point"), and f. the depth of water below the measuring point at the time the measuring point is established. This survey shall be conducted using approved practices outlined in North Carolina General Statutes Chapter 89C and the North Carolina Administrative Code Title 21, Chapter 56. The surveyor shall establish a horizontal control monument on the property of the waste disposal system and determine the latitude and longitude of this horizontal control monument to a horizontal positional accuracy of +/-10 feet. All other features listed in a. through e. above shall be surveyed relative to this horizontal control monument. The positional accuracy of features listed' in a. through e. above shall have a ratio of precision not to exceed as error of closure of 1 foot per 10,000 feet of perimeter of the survey. 12 Any features located by the radial method will be located from a minimum of two points. Horizontal control monuments shall be installed in such a manner and made of such materials that the monument will not be destroyed due to activities that may take place on the property. The map shall also be surveyed using the North American Datum of 1983 coordinate system and shall indicate the datum on the map. All bearings or azimuths shall be based on either the true or NAD 83 grid meridian. If a Global Positioning System (GPS) is used to determine the latitude and longitude of the horizontal control monument, a GPS receiver that has the capability to perform differential GPS shall be used and all data collected by the GPS receiver will be differentially corrected. The maps and any supporting documentation shall be sent to the Groundwater Section, NC Division of Environmental Management, P.O. Box 29578, Raleigh, NC, 27626-0578. 6. Within thirty (30) days of completion of all well construction activities, a certification must be received from a professional engineer certifying that the monitoring wells are located and constructed in accordance with the Well Construction Standards (15A NCAC 2C) and this permit. This certification should be submitted with copies of the Well Completion Form (GW-1) for each well. Mail this certification and the associated GW-1 forms to the Permits and Compliance Unit, Groundwater Section, P.O. Box 29578, Raleigh, NC, 27626- 0578. 7. For the initial sampling of the well as specified elsewhere in the permit, the permittee shall submit a copy of the GW-1 Form (Well Completion Form) with the Compliance Monitoring Form (G W-59) for that well. Compliance Monitoring Forms that do not include copies of the GW-1 form will be returned to the permittee without being processed. Failure to submit these forms as required by Us permit may result in the initiation of enforcement activities pursuant to NC General Statutes 143-215.6. 8. Monitoring wells`MW-10 through MW-18 shall be, sampled initially after construction (and prior to waste disposal operations at the Reedy Branch site) for the following parameters: Water Level Nitrate (NO3-N) Chloride Iron Total Dissolved Solids (TDS) Total Organic Carbon (TOC) pH Total Ammonia Sodium Sulfate(SO4) Fecal Coliforms Volatile Organic Compounds (VOC) - using Method 6230D (Capillary Column), "Standard Methods for the Examination of Water and Wastewater", 17th ed., 1989 13 The measurement of water levels must be made prior to sampling for the remaining parameters. The depth to water in each well shall be measured from the surveyed point on the top of the casing. The measuring points (top of well casing) of all monitoring wells shall be surveyed to provide the relative elevation of the measuring point for each monitoring well. The results of the sampling and analysis, in addition to the GW-1 Well Construction records, shall be sent to the Groundwater Section, Permits and Compliance Unit, P.O. The Reedy Branch site spray field monitoring wells MW-10 through MW-18 shall be sampled every April, August, and December for the following parameters: Water Level pH Nitrate (NO3-N) Total Ammonia Chloride Sodium Iron Sulfate(SO4) Total Dissolved Solids (TDS) Fecal Coliforms Total Organic Carbon (TOC) Volatile Organic Compounds (VOC) - In December only using Method 6230D (Capillary Column), "Standard Methods for the Examination of Water and Wastewater", 17th ed., 1989 The measurement of water levels must be made prior to sampling for the remaining parameters. The depth to water in each well shall be measured from the surveyed point on the top of the casing. The measuring points (top of well casing) of all monitoring wells shall be surveyed to provide the relative elevation of the measuring point for each monitoring well. If TOC concentrations greater than 10 mg/I are detected in any downgradient monitoring well, additional sampling and analysis must be conducted to identify the individual constituents comprising this TOC concentration. If the TOC concentration as measured in the background monitor well exceeds 10 mg/l, this concentration will be taken to represent the naturally occurring TOC concentration. Any exceedances of this naturally occurring TOC concentration in the downgradient wells shall be subject to the additional sampling and analysis as described above. 14 The results of the sampling and analysis shall be sent to the Groundwater Section, Permits and Compliance Unit, P.O. Box 29578, Raleigh, N.C. 27626-0578 on Form GW-59 [Compliance Monitoring Report Form] within 30days of sample collection. 10. Any additional groundwater quality monitoring, as deemed necessary by the Division, shall be provided. 11. The COMPLIANCE BOUNDARY for the disposal system is specified by regulations in 15A NCAC 2L, Groundwater Classifications and Standards. The Compliance Boundary is for the disposal system constructed after December 31, 1983 is established at either (1) 250 feet from the waste disposal area, or (2) 50 feet within the property boundary, whichever is closest to the waste disposal area. An exceedance of Groundwater Quality Standards at or beyond the Compliance Boundary is subject to immediate remediation action in addition to the penalty provisions applicable under General Statute 143-215.6A(a)(1). In accordance with 15A NCAC 2L, a REVIEW BOUNDARY is established around the disposal systems midway between the Compliance Boundary and the perimeter of the waste disposal area. Any exceedance of standards at the Review Boundary shall require remediation action on the part of the perniittee. 12. The 5-day storage pond, the equalization lagoon, and the aerobic digester shall have a liner of natural material at least one foot in thickness, compacted to 95% proctor standard dry density and installed and compacted in layers no thicker than six inches, with a hydraulic conductivity of no greater than 1 x 10-' cm/sec. Following installation and inspection of the lagoon liner, and prior to waste disposal operations, verification of the liner's compliance with hydraulic conductivity and thickness specifications must be provided to the Division of Water Quality, Groundwater Section, by the project engineer. 13. The existing well at Station 51+00 of the effluent force main shall be abandoned in accordance with 15A NCAC 2C.0113 as it is within 25 feet of the effluent force main. V. INSPECTIONS 1 Adequate inspection, maintenance, and cleaning shall be provided by the Permittee to insure proper operation of the subject facilities. 2. The Permittee or his designee shall inspect the wastewater treatment and disposal facilities to prevent malfunctions and deterioration, operator errors and discharges which may cause or lead to the release of wastes to the environment, a threat to human health, or a nuisance. The Permittee shall keep an inspection log or summary including at least the date and time of inspection, observations made, 15 and any maintenance, repairs, or corrective actions taken by the Permittee. This log of inspections shall be maintained by the Permittee for a period of three years from the date of the inspection and shall be made available upon request to the Division of Water Quality or other permitting authority. 3. Any duly authorized officer, employee, or representative of the Division of Water Quality may, upon presentation of credentials, enter and inspect any property, premises or place on or related to the disposal site or facility at any reasonable time for the purpose of determining compliance with this permit, may inspect or copy any records that must be maintained under the terms and conditions of this permit, and may obtain samples of groundwater, surface water, or leachate. VI. GENERAL CONDITIONS This permit shall become voidable unless the facilities are constructed in accordance with the conditions of this permit, the approved plans and specifications, and other supporting data. This permit shall become voidable unless the interlocal agreement for emergency interconection between the City of New Bern and the County of Craven is Z maintained in full force and effect during the period of construction. 3. This permit is effective only with respect to the nature and volume of wastes described in the application and other supporting data. 4. This permit is not transferable. In the event there is a desire for the facilities to change ownership, or there is a name change of the Permittee, a formal permit request must be submitted to the Division of Water Quality accompanied by an application fee documentation from the parties involved, and other supporting materials as may be appropriate. The approval of this request will be considered on its merits and may or may not be approved. 5. Failure to abide by the conditions and limitations contained in this permit may' subject the Permittee to an enforcement action by the Division of Water Quality in accordance with North Carolina General Statute 143-215.6(a) to 143-215.6(c). 6. The issuance of this permit does not preclude the Permittee from complying with any and all statutes, rules, regulations, or ordinances which may be imposed by other government agencies (local, state, and federal) which have jurisdiction. 7. A set of approved plans and specifications for the subject project must be retained by the Permittee for the life of the project. 8. The annual administering and compliance fee must be paid by the Permittee within thirty.(30) days after being billed by the Division. Failure to pay the fee z accordingly may cause the Division to initiate action to revoke this permit as specified by 15A NCAC 2H .0205 (c)(4). 9. The Permittee, at least six (6) months prior to the expiration of this permit, shall request its extension. Upon receipt of the request, the Commission will review the adequacy of the facilities described therein, and if warranted, will extend the permit for -such period of time and under such conditions and limitations as it may deem appropriate.
