1) This permit must be attached to the original of Permit #111-96, which was issued on 7/29/96,
and all documents must be readily available on site when Division personnel inspect the project
for compliance.
2) All conditions and stipulations of the active permit remain in force trader this renewal.
This permit_action may be appealed by the permittee or
other qualified persons within twenty (20) days of the
issuing date. An appeal requires resolution prior to work
initiation or continuance, as the case may be.
This permit must be accessible on -site to Department
personnel when the project is inspected for compliance.
Any maintenance work or project modification not
covered hereunder requires further Departmental approval..
All work must cease when the�ermit expires on

original and the copy and return the copy to this office in the enclosed envelope. Signing the permit and proceeding
means you have waived your right of appeal described below.

Administrative Hearings, P.O. Drawer 11666, Raleigh, NC 27611, (919) 733-2698 within twenty (20) days ofthis
decision on your permit. You should also be aware that if another qualified party submits a valid objection to the
issuance of this permit within twenty (20) days, the matter must be resolved prior to work initiation. The Coastal

Resources Commission makes the final decision on any appeal.
The project plan is subject to those conditions appearing on the permit form. Otherwise, all work must be
carried out in accordance with your application. Modifications, time extensions, and future maintenance require
additional approval. Please read your permit carefully prior to starting work and review all project plans, as

understands all permit requirements.
From time to time, Department personnel will visit the project site. To facilitate this review, we request
l that you complete and mail the enclosed Notice Card just prior to work initiation. However, if questions arise
concerning permit conditions, environmental safeguards, orproblem areas, you may contact Department personnel at
any time for assistance. By working in accordance with the permit, you will be helping to protect our vitally
important coastal resources.

SUBJECT: Renewal of Permit No. 111-96 issued to Mr. Sammie Williams for a
bulkhead and dock, Hyde County
CAMA Major Permit No. 111-96 was issued to Mr. Sammie Williams for the
construction of a bulkhead and dock onsite his property off SR 1101 adjacent to Far
Creek in Englehard, Hyde County, North Carolina. The staff of the DCM WaRO has been
onsite Mr. Williams' property on several occasions during recent years and concur that
a portion of the work has been completed as reported by Mr Williams. Therefore, I do not
object to the renewal of this permit.
The $50 fee to cover the cost of the renewal has been deposited in the
Department's account. If you have any questions, please give me a call.

Dear Mr. Moore,
I would like to request an extension to my existing CAMA Major Permit No. 111-96. Due to time, weather, and
supply constraints, much of the needed work has not been completed My property has been enhanced greatly
thanks to the permitted projects your agency has helped me with in the past: I would appreciate any cooperation
you could extend me now.
The project site is located at the former Cargill grain property of the south bank of Far Creek, Engelhard. CAMA
Major Permit No.111-96 allowed the replacement of an existing metal bulkhead(which has outlived its usefulness)
with a treated wood bulkhead, the stalbilization of the shoreline where there is no bulkhead with a new one, and
the fill of a shallow existing boat basin. This work will allow large fishing boats to moor at the facility.
Thus far, approximately half of Jarvis Ditch has been buldheaded; soldier pihmds and tie-up pilands have been put
down on about fifty -percent of the Far Creek side. None of the proposed dock has been built. The main reason the
work has not been completed is because 1 could not get more pilands(My supplier ran "dry" until recently.). The
weather has also contributed by supplying many rainy days and hurricanes during the last year and a half. I have
enclosed a plat with the completed work marked on it.
The permitted work that has not been completed is highlighted on the enclosed plat Approximately half of the
Jarvis Ditch area remains to be bulkheaded; just over half of the bulkhead work remains to be done on the Far
Creek line; all of the dock has to be built In other words, there is a LOT of work left to do.
Please allow me an extension of permit no. 111-96 so that I may complete the much needed work If you have any
