Hey Jamie,
I have attached the agent form as well as the riparian forms you will need the neighbors on either side
of the HOA property sign. The agent form must be signed by the HOA authorizing you to obtain the
CAMA permit for your slip. I will also need a drawing showing what is existing and what you are
proposing with dimensions. As far as filling out the riparian forms the top is the HOA information as well
as your information and slip number in the agent section. The rest of the form is signed by the adjacent
neighbors.
