Primarily, item V A dealing with intended use must be completed because the modification involves an excavation of some 1400 feet long. Also, item IV C dealing with disposal area needs more attention. Finally, you should indicate as requested on X C, that Mr. Jarvis was notified. After a thorough evaluation of the work involved, it was determined that with minor exception the work had been carried out in accordance with the permit. I have requested that the U. S.
