claims title to the affected property. OR if applicant is not claiming to be the owner of said property,
then forward a copy of the deed or other instrument under which the owner claims title plus written
permission from the owner to carry out the project on his land. (Note: Federal law requires the
applicant to be the owner or his designated agent).
B. Attach an accurate work plat drawn to scale on 8%x" X I I" white paper (see instruction booklet for
details). Note: Original drawings preferred - only high quality copies accepted.
C. A copy of the application and plat must be served upon adjacent riparian landowners by registered or
.c+j certified mail or by publication (G. S. 113-229 (g) (9) and (10)). Enter date served
Such owners have 30 days in which to submit comments to agencies listed below.

Dear Mr. Cooke:
In accordance with the writtey-tdquest of 5 April 78 and the ensuing
administrative record, the a is inclosed a permit authorizing you to
construct and backfill a 141khead, place riprap mie:tirial waterward of
the bulkhead, coustruct a bolt_ramp,_ and rebu#id a pier on the Pamlico
River at Pamlico Beach, Beaufort County, :forth Carolina.
If any change in the authorized work is required because of unforeseen
or altered conditions or for any other reason, plans revised to show the
change must be sent promptly to this office. Such action is necessary
as revised plans must be reviewed and the permit modified.
Please carefully read your permit. The general and special conditions
are important. Your failure to comply with these conditions could
result in a violation of Federal law. Certain significant general
conditions require that:
1. You must begin your work on or before 31 July 1979 and complete
construction before 31 December 1981.
2. You must notify this office in advance as to when you intend to
commence and complete work.
3. You must allow representatives from this office to make periodic
visits to your worksite as deemed necessary to assure compliance with
permit plans and conditions.

MAINTENANCE DREDGING: (1) That when the work authorized herein includes periodic maintenance dredging, it may he performed
under this permit for_years from the date of issuance of this permit (ten years unless otherwise indicated); and (2) That the
pwmittee will advise the District Engineer in writing at least two weeks before he intends to undertake any maintenance dredging.

landward of the permitted bulkhead.
This permit shall become effective on the date of the District Engineer's signature.
