This permit, issued on November 22, 2022 , is subject to compliance with the application (where
consistent with the permit), all applicable regulations, special conditions and notes set forth below. Any violation
if these terms may be subject to tines, imprisonment or civil action; or may cause the permit to be nuts ana voia.
Docking Facility Expansion
1) This permit authorizes only the finger pier and two boat lifts and uses located in or over the water
that are expressly and specifically set forth in the permit application. No other structure, whether
floating or stationary, shall become a permanent part of this docking facility without permit
modification. No non -water dependent uses of structures shall be conducted on, in or over Public
Trust waters without permit modification.
2) No sewage, whether treated or untreated, shall be discharged at any time from any boats using the
docking facility. Any sewage discharged at the docking facility shall be considered a violation of this
permit for which the permittee is responsible. This prohibition shall be applied and enforced
throughout the entire existence of the permitted structure.

This permit action may be appealed by the permittee or other
qualified persons within twenty (20) days of the issuing date.
This permit must be accessible on -site to Department
personnel when the project is inspected for compliance.
Any maintenance work or project modification not covered
hereunder requires further Division approval.
All work must cease when the permit expires on
November 22, 2027
In issuing this permit, the State of North Carolina agrees that
your project is consistent with the North Carolina Coastal
Management Program.

3) The permittee shall install and maintain at his expense any signal lights or signals prescribed by the U.S.
Coast Guard, through regulation or otherwise, on the authorized facilities. At minimum, permanent
reflectors shall be attached to the structure in order to make it more visible during hours of darkness or
inclement weather.
4) This permit authorizes 2 additional formalized boat slips for a maximum of 4 formalized boat slips at
this docking facility.
5) No attempt shall be made by the permittee to prevent the full and free use by the public of all navigable
waters at or adjacent to the authorized work.
6) The authorized structure and associated activity shall not cause an unacceptable interference with
navigation and shall not exceed the dimensions shown on the attached permit drawings.
Sedimentation and Erosion Control
7) In order to protect water quality, runoff from construction shall not visibly increase the amount of
suspended sediments in adjacent waters.
8) Appropriate sedimentation and erosion control devices, measures or structures shall be implemented to
ensure that eroded materials do not enter adjacent wetlands, watercourses or properties.
General
9) This permit shall not be assigned, transferred, sold, or otherwise disposed of to a third party without the
written approval of the Division of Coastal Management.
10) All construction debris associated with the removal or construction of the permitted development shall
be contained within the authorized project area and disposed of in an approved upland location.
11) The permittee and/or his or her contractor shall meet with a representative of the Division prior to
project initiation.
NOTE: This permit does not eliminate the need to obtain any additional state, federal or local permits,
approvals or authorizations that may be required.
NOTE: The U.S. Army Corps of Engineers authorized the project by way of Programmatic General
Permit 198000291 (Action ID SAW-2022-02083).
NOTE: The N.C. Division of Water Resources authorized the proposed project by way of General Water
Quality Certification 4501 and assigned the project DWR Project No. 2022-1312.

NOTE: Future development of the permittee's property may require a modification of this permit.
Contact a representative of the Division at (252) 515-5400 prior to the commencement of any
such activity for this determination. The permittee is further advised that many land disturbing
activities are not authorized within 50 feet of the normal water level unless specifically exempted
by Environmental Management Commission's (EMC) Neuse River buffer regulations in
place at the time of such development.
NOTE: An application processing fee of $250 was received by DCM for this project.

package to be complete. Items (a) - (f) are always applicable to any major development application. Please consult the application
instruction booklet on how to properly prepare the required items below.

NARRATIVE DESCRIPTION OF PROPOSED IMPROVEMENTS
The proposed waterfront improvement on the Clubfoot Creek property at 785 and 787 Beasley
Lane will add two boat lifts. The property has two single-family dwellings on it. There are
currently two boat lifts with a finger pier attached to a concrete walkway with a concrete
platform at the end.
The proposed improvements would allow each family residing on the property (785 and 787) to
keep more than one boat on a lift at a time. Although this is submitted as a Major Development
Permit Application, the final result will be similar to what would be allowed without a Major
Permit if the two residences were not on the same property. The residences have separate
postal addresses, but Craven County will not allow the property to be subdivided.

The following is my assessment and consequent recommendations of the above referenced proposal: This
proposal would impact 696 square feet of Public Trust Area. The proposal is not adjacent to a Federal
Channel. These waters of Clubfoot Creek are closed to shellfish harvest. This waterbody is not a Primary
Nursery Area. There is no submerged aquatic vegetation present at this location. Assuming that other
State and Federal agencies concur, I would recommend authorizing this proposal contingent upon the
following conditions:
1. This permit authorizes a maximum of four boat slips. At no time shall more than four
vessels be moored or docked at the facility. The restriction of the number of vessels shall
apply to all marine vessel types, including but not limited to motorboats, sailing vessels
and personal watercraft.
2. This permit authorizes only the docks, piers, and other structures and uses located in
or over the water that are expressly and specifically set forth in the permit application.
No other structure, whether floating or stationary, may become a permanent part of
this docking facility without permit modification. No non -water dependent uses of
structures may be conducted on, in or over public trust waters without permit
modification.
3. The docking facility and associated structures shall have a minimum setback distance of
15 feet between any parts of the structure and the adjacent property owner's riparian access
corridor, which is determined by drawing a line parallel to the channel, then drawing a line
perpendicular to the channel line that intersects with the shore at the point where the upland
property line meets the water's edge.

Consistency Determination: The request is consistent with/not in conflict with the Craven County
CAMA Core Land Use Plan certified by the CRC on October 30, 2009.
Overview: The proposal is within Clubfoot Creek adjacent to 785 Beasley Lane near Harlowe, Craven
County. This property is currently developed with a residence, shoreline stabilization, and a 2-slip docking
facility. The elevation of the high ground on the property is approximately 11' above the normal water
level. Vegetation on the property mainly consists of lawn grasses, ornamental shrubs, and fruit trees.
There is approximately 225' of shoreline on this 3.5-acre tract. Water depths in the project area range
from -4.0' to -5.5' nwl. The subaqueous substrate is firm without shell. There is not a Federal Channel in
the nearby vicinity of the project area. This waterbody is approximately 1,100' across at this location.
proposal is located within the Neuse River basin.
The project is in the EW & PTA AEC. The water at the project site in Clubfoot Creek is classified as as SA;
HOW, NSW. The site in not a PNA (Primary Nursery Area) and is closed to the taking of shellfish. However,
the estuarine waters are classified as a Secondary Nursery Area. There is not a cultural resource in the
project area. Submerged aquatic vegetation is not present within the project area.
Proposed Development: The applicant is proposing to expand the existing docking facility by constructing
a 5' by 24' finger pier on the north side of the existing docking facility, near the waterward terminus. This
proposed finger pier would support two proposed boatlifts on either side. Each boatlift would be 12' by
10'.
Anticipated impacts: The proposed finger pier and two boat slips would usurp an additional 696 square
feet of Public Trust waters. The proposed finger pier would shade 120 square feet of open water.

Basis for Determination:
The project site is located in Craven County and is subject to Craven County CAMA Core Land Use Plan,
The Future Land Use Map (FLUM) designates the property as "Residential" defined as "...all moderate to
high density housing developments throughout the county" (pg. 180).
The following LUP policies may be applicable to this request:
Policies- Conservation:
P.144: "Residential, commercial, and industrial development which meets 15A NCAC 7H use standards
will be allowed in estuarine shoreline, estuarine water, and public trust areas. In all other areas,
development will be allowed that is consistent with applicable local, state, and federal regulations'.
P.46 Craven County supports the construction of docks and piers if they are in compliance with applicable
policies of this plan and state/federal regulations.
1.29 Craven County will rely on the NC Department of Environment and Natural Resources, Division of
Coastal Management to regulate development through the CAMA permitting process. Also, it will rely on
the regulations of other state and federal agencies with regulatory authority, as well as existing local

A North Carolina Division of Marine Fisheries (DMF) Fisheries Resource Specialist has
reviewed the permit application for proposed actions that may impact fish and/or fish habitats.
The proposed work is located at 785 Beasley Lane, Havelock, within Craven County. The
applicant is proposing to expand the existing docking facility that is located in Clubfoot Creek.
The waters of this site are classified as SA; HQW, NSW, and are designated as Secondary
Nursery Area (SNA).
The proposed expansion will involve multiple additions. The applicant will add a 5' by 24'
finger pier on the northern side of the existing dock, adjacent to the existing finger pier. Two
additional 12' by 10' boat lifts will be added to the proposed finger pier, replicating the design of
the existing structure.
As noted above, the waters at this location are designated as SNA. Areas that have been
designated as SNA exhibit the habitat requirements needed for secondary development of many
species. Many of the species that utilize these areas for development are commercially,
recreationally, and/or ecologically important species or their prey. As such, protection of these
areas is critical to ensure that these species have the habitats necessary to thrive. Therefore, DMF
would recommend that any permit issued include a 1 April through 30 September moratorium
for all in -water work.

Reference the application of Daniel S. Grubb for a Department of the Army permit to
expand an existing docking facility and constrict new finger piers and boat lifts, on property
located at 785 Beasley Lane, adjacent to Clubfoot Creek, near Harlowe, in Craven County, North
Carolina.
The Federal agencies have completed review of the proposal as presented by the
application and your field investigation report.
We recommend that the following conditions be included in the State authorization:
1. All work authorized by this permit must be performed in strict compliance with the
attached plans, which are a part of this permit. Any modification to these plans must be approved
by the US Army Corps of Engineers (USACE) prior to implementation.
2. The permittee understands and agrees that, if future operations by the United States
require the removal, relocation, or other alteration, of the structure or work herein authorized, or
if, in the opinion of the Secretary of the Army or his authorized representative, said structure or
work shall cause unreasonable obstruction to the free navigation of the navigable waters, the
permittee will be required, upon due notice from the U.S. Army Corps of Engineers, to remove,
relocate, or alter the structural work or obstructions caused thereby, without expense to the
United States. No claim shall be made against the United States on account of any such removal,
relocation, or alteration. The permittee shall notify NOAA/NATIONAL OCEAN SERVICE
Chief Source Data Unit N CS261, 1315 E West HWY- RM 7316, Silver Spring, MD 20910-
3282 at least two weeks prior to beginning work and upon completion of work.
3. Except as authorized by this permit or any USACE approved modification to this

permit, no excavation, fill or mechanized land -clearing activities shall take place at any time in
the construction or maintenance of this project, within waters or wetlands. This permit does not
authorize temporary placement or double handling of excavated or fill material within waters or
wetlands outside the permitted area. This prohibition applies to all borrow and fill activities
connected with this project.
4. The authorized structure and associated activity must not interfere with the public's right
to free navigation on all navigable waters of the United States. No attempt will be made by the
permittee to prevent the full and free use by the public of all navigable waters at or adjacent to
the authorized work for reason other than safety.
5. The permittee must install and maintain, at his expense, any signal lights and signals
prescribed by the U.S. Coast Guard, through regulations or otherwise, on authorized facilities.
For further information, the permittee should contact the U.S. Coast Guard Marine Safety Office
at (910) 772-2191.
6. The permittee shall require its contractors and/or agents to comply with the terms and
conditions of this permit in the construction and maintenance of this project, and shall provide
each of its contractors and/or agents associated with the construction or maintenance of this
project with a copy of this permit. A copy of this permit, including all conditions, shall be
available at the project site during construction and maintenance of this project.
7. Violations of these conditions or violations of Section 404 of the Clean Water Act or
Section 10 of the Rivers and Harbors Act must be reported in writing to the Wilmington District
U.S. Army Corps of Engineers within 24 hours of the permittee's discovery of the violation.

Narrative Description:
The proposal is within Clubfoot Creek adjacent to 785 Beasley Lane near Harlowe, Craven County.
This property is currently developed with a residence, shoreline stabilization, and a 2-slip docking
facility. This proposal is for private use. The adjacent properties are similarly developed. The closest
adjacent docking facility is approximately 150' to the north.
The elevation of the high ground on the property is approximately 11' above the normal water level.
Vegetation on the'property mainly consists of lawn grasses, ornamental shrubs, and fruit trees. There
is approximately 225' of shoreline on this 3.5-acre tract.
Water depths in the project area range from --4.0' to —5.5' nwl. The subaqueous substrate is firm
without shell. There is not a Federal Channel in the nearby vicinity of the project area. There is not a
cultural resource in the project area. This waterbody is approximately 1,100' across at this location.
These estuarine waters are classified as a Secondary Nursery Area. Submerged aquatic vegetation is
not present within the project area. This waterbody is closed to shellfish harvest and is classified as
SA; HQW, NSW. This proposal is located within the Neuse River basin.
Proposed Development:
The applicants are proposing to construct a finger pier and two additional boat lifts at the existing
docking facility in order to support a total of four boat slips within Clubfoot Creek. This proposal is
adjacent to 785 Beasley Lane near Havelock, Craven County. This proposed 4-slip docking facility
would be a shared dock for private use.
The applicant is proposing to construct a 5' by 24' finger pier on the north side of the existing
docking facility, near the waterward terminus. This proposed finger pier would support two proposed
boatlifts on either side. Each boatlift would be 12' by 10'. The water depth within the proposed boat
slip areas range from -4.0' to -5.5' nwl.
Anticipated Impacts:
This proposed 4-slip docking facility would be a shared dock for private family use. This docking
facility extends approximately 140' waterward. Clubfoot Creek is approximately 1,100' across at this
location. The proposed finger pier and two boat slips would usurp an additional 696 square feet of
Public Trust waters. The proposed finger pier would shade 120 square feet of open water.
Traditional marine construction methodology would be utilized during the construction of this
project. Turbidity impacts are not anticipated during the construction activities within Clubfoot
Creek. This waterbody is not classified as a Primary Nursery Area. There is no SAV present within
the project area. There are no oyster beds in the nearby vicinity of the project area. This waterbody is
closed to shellfish harvest. There is not a Federal Channel in the nearby vicinity of the project area,
and the traditional thru-navigation route is in the center of Clubfoot Creek at this location.

NARRATIVE DESCRIPTION OF PROPOSED IMPROVEMENTS
The proposed waterfront improvement on the Clubfoot Creek property at 785 and 787 Beasley
Lane will add two boat lifts. The property has two single-family dwellings on it. There are
currently two boat lifts with a finger pier attached to a concrete walkway with a concrete
platform at the end.
The proposed improvements would allow each family residing on the property (785 and 787) to
keep more than one boat on a lift at a time. Although this is submitted as a Major Development
Permit Application, the final result will be similar to what would be allowed without a Major
Permit If the two residences were not on the same property. The residences have separate
postal addresses, but Craven County will not allow the property to be subdivided.

and appears to be adequate for processing at this time. The projected deadline for making a
decision is December 2, 2022. An additional 75-day review period is provided by law when
such time is necessary to complete the review. If you have not been notified of a final action
by the initial deadline stated above, you should consider the review period extended. Under
those circumstances, this letter will serve as your notice of an extended review. However,
an additional letter will be provided on or about the 75th day.
If this agency does not render a permit decision within 70 days from September 16, 2022, you
may request a meeting with the Director of the Division of Coastal Management and permit staff
to discuss the status of your project. Such a meeting will be held within five working days from
the receipt of your written request and shall include the property owner, developer, and project
designer/consultant.
NCGS 113A-119(b) requires that Notice of an application be posted at the location of the
proposed development. Enclosed you will find a "Notice of Permit Filing" postcard which
must be posted at the property of your proposed development. You should post this
notice at a conspicuous point along your property where it can be observed from a public
road. Some examples would be: Nailing the notice card to a telephone pole or tree along
the road right-of-way fronting your property, or at a point along the road right-of-way
where a private road would lead one into your property. Failure to post this notice could
result in an incomplete application.

TIPS FOR ADJACENT RIPARIAN OWNER NOTIFICATION & WAIVER FORMS
CAMA Regulations require notice of proposed development to the Adjacent Riparian Property Owners (15A
NCAC 7J.0204(b)(5)). Proof of actual notice (a sign -off by the owner on this form) or certified mail return
receipts (showing delivery of notice) are needed.
The purpose of this notice is to make adjacent riparian property owners aware of the proposed development
so that they have an opportunity to provide comr.ients (or potentially object to) the proposed development,
and to give DCM an opportunity to consider these comments/objections before a permit decision is made.
"Permission" of adiacent property owners is not necessarily required for DCM to issue a permit; however,
permission must be obtained for certain types of proposed developments if they fall within a 15' setback area
along the riparian access area of the adjacent property, as determined solely by DCM. It is in the applicant's
best interest to provide comprehensive and accurate notice so that any concerns or objections can be
resolved early in the permit review process. In addition, if the adjacent riparian property owner appeals the
permit decision, insufficient notice could be a basis for granting such an appeal.
Who is an Adjacent Riparian Property Owner?
What is Adjacent? (Note: DCM reads this broadly to ensure comment by potentially impacted neighbors)
• A property that shares a boundary line with the site of proposed development; AND
• A property that fronts a natural or manmade waterbody that is connected to coastal waters and
can support some form of navigation, even a kayak or canoe, including a common canal system
or a manmade basin.
• Easement holders? Yes, if the easement could be impacted by the proposed development.
• Streets/Roads? Only if the streettroad could be impacted by the proposed development. This
might include street -ends which might be used for parking and beach access.
• Holders of recognized submerged lands claims/shellfish franchises.
What is Riparian?
Do the boundaries of the adjacent property legally intersect with the water at mean high tide? If there is a
question about whether an adjacent property is considered "riparian," please reach out to DCM Staff -
especially on the oceanfront beach where there may be undeveloped parcels on the beach or in the water.
Who/What is a Property Owner?
For private individuals (or families), send notice to the address listed on the tax card. If the property is owned
by an Inc. or LLC, please send notice to the person listed as the registered agent on the NC Secretary of
State's Corporations Look Up site: https://wwvv_sosnc_gw/search/index,coip. For Condominiums or
neighborhoods with an owners' association (HOA/POA), send notice to the association (which is usually a
corporation, for which you send notice to its registered agent).
What is Notice, and how do I ensure it is received?
• You can meet with your adjacent property owner, provide a description or drawing of the
proposed development, and ask them to sign this form and return it to DCM; OR
• You can hand -deliver this form and a description or drawing, and ask your neighbor to return it to
DCM (consider providing them with a stamped envelope); OR
• You can mail this form by USPS Certified Mail, return receipt requested (the Green Card). If you
choose this option, you must provide either the signed & returned green card OR track the
certified mail package number on USPS.GOV's online tracking system, and provide the tracking
print-out as proof of delivery to DCM

TIPS FOR ADJACENT RIPARIAN OWNER NOTIFICATION & WAIVER FORMS
CAMA Regulations require notice of proposed development to the Adjacent Riparian Property Owners (15A
NCAC 7J.0204(b)(5)). Proof of actual notice (a sign -off by the owner on this form) or certified mail return
receipts (showing delivery of notice) are needed.
The purpose of this notice is to make adjacent riparian property owners aware of the proposed development
so that they have an opportunity to provide comr ,,_Ents (or potentially object to) the proposed development,
and to give DCM an opportunity to consider these connnents/objections before a permit decision is made.
"Permission" of adiacent property owners is not necessarily required for DCM to issue a permit; however,
permission must be obtained for certain types of proposed developments if they fall within a 15' setback area
along the riparian access area of the adjacent property, as determined solely by DCM. It is in the applicant's
best interest to provide comprehensive and accurate notice so that any concerns or objections can be
resolved early in the permit review process. In addition, if the adjacent riparian property owner appeals the
permit decision, insufficient notice could be a basis for granting such an appeal.
Who is an Adjacent Riparian Property Owner?
What is Adjacent? (Note: DCM reads this broadly to ensure comment by potentially impacted neighbors)
• A property that shares a boundary line with the site of proposed development; AND
• A property that fronts a natural or manmade waterbody that is connected to coastal waters and
can support some form of navigation, even a kayak or canoe, including a common canal system
or a manmade basin.
• Easement holders? Yes, if the easement could be impacted by the proposed development.
• Streets/Roads? Only if the street/road could be impacted by the proposed development. This
might include street -ends which might be used for parking and beach access.
Holders of recognized submerged lands claims/shellfish franchises.
What is Riparian?
Do the boundaries of the adjacent property legally intersect with the water at mean high tide? If there is a
question about whether an adjacent property is considered "riparian," please reach out to DCM Staff -
especially on the oceanfront beach where there may be undeveloped parcels on the beach or in the water.
Who/What is a Property Owner?
For private individuals (or families), send notice to the address listed on the tax card. If the property is owned
by an Inc. or LLC, please send notice to the person listed as the registered agent on the NC Secretary of
State's Corporations Look Up site: https://www.sosnc.gov/search/index/corn. For Condominiums or
neighborhoods with an owners' association (HOA/POA), send notice to the association (which is usually a
corporation, for which you send notice to its registered agent).
What is Notice, and how do I ensure it is received?
• You can meet with your adjacent property owner, provide a description or drawing of the
proposed development, and ask them to sign this form and return it to DCM; OR
• You can hand -deliver this form and a description or drawing, and ask your neighbor to return it to
DCM (consider providing them with a stamped envelope), OR
• You can mail this form by USPS Certified Mail, return receipt requested (the Green Card). If you
choose this option, you must provide either the signed & returned green card OR track the
certified mail package number on USPS.GOV's online tracking system, and provide the tracking
print-out as proof of delivery to DCM

NARRATIVE DESCRIPTION OF PROPOSED IMPROVEMENTS
The proposed waterfront improvement on the Clubfoot Creek property at 785 and 787 Beasley
Lane will add two boat lifts and a ramp where a boat can be launched from a trailer. The
property has two single-family dwellings on it. There are currently two boat lifts with a finger
pier attached to a concrete walkway with a concrete platform at the end. The closest public
ramp is Cahoogue Creek Landing, which is approximately 12 miles away.
The proposed improvements would allow each family residing on the property (785 and 787) to
keep more than one boat on a lift at a time. Although this is submitted as a Major Development
Permit Application, the final result will be similar to what would be allowed without a Major
Permit if the two residences were not on the same property. The residences have separate
postal addresses, but Craven County will not allow the property to be subdivided.

TIPS FOR ADJACENT RIPARIAN OWNER NOTIFICATION & WAIVER FORMS
CAMA Regulations require notice of proposed development to the Adjacent Riparian Property Owners (15A
NCAC 7J.0204(b)(5)). Proof of actual notice (a sign -off by the owner on this form) or certified mail return
receipts (showing delivery of notice) are needed.
The purpose of this notice is to make adjacent riparian property owners aware of the proposed development
so that they have an opportunity to provide comments (or potentially object to) the proposed development,
and to give DCM an opportunity to consider these comments/objections before a permit decision is made.
"Permission" of adiacent oroperty owners is not necessarily required for DCM to issue a permit; however,
permission must be obtained for certain types of proposed developments if they fall within a 15' setback area
along the riparian access area of the adjacent property, as determined solely by DCM. It is in the applicant's
best interest to provide comprehensive and accurate notice so that any concerns or objections can be
resolved early in the permit review process. In addition, if the adjacent riparian property owner appeals the
permit decision, insufficient notice could be a basis for granting such an appeal.
Who is an Adjacent Riparian Property Owner?
What is Adjacent? (Note DCM reads this broadly to ensure comment by potentially impacted neighbors)
• A property that shares a boundary line with the site of proposed development; AND
• A property that fronts a natural or manmade waterbody that is connected to coastal waters and
can support some form of navigation, even a kayak or canoe, including a common canal system
or a manmade basin.
• Easement holders? Yes, if the easement could be impacted by the proposed development.
• Streets/Roads? Only if the streettroad could be impacted by the proposed development. This
might include street -ends which might be used for parking and beach access.
• Holders of recognized submerged lands claims/shellfish franchises.
What is Riparian?
Do the boundaries of the adjacent property legally intersect with the water at mean high tide? If there is a
question about whether an adjacent property is considered "riparian," please reach out to DCM Staff -
especially on the oceanfront beach where there may be undeveloped parcels on the beach or in the water.
Who/What is a Property Owner?
For private individuals (or families), send notice to the address listed on the tax card. If the property is owned
by an Inc. or LLC, please send notice to the person listed as the registered agent on the NC Secretary of
State's Corporations Look Up site: https://www,sosnc.gov/search/index/corp. For Condominiums or
neighborhoods with an owners' association (HOA/POA), send notice to the association (which is usually a
corporation, for which you send notice to its registered agent).
What is Notice, and how do 1 ensure it is received?
• You can meet with your adjacent property owner, provide a description or drawing of the
proposed development, and ask them to sign this form and return it to DCM; OR
• You can hand -deliver this form and a description or drawing, and ask your neighbor to return it to
DCM (consider providing them with a stamped envelope); OR
• You can mail this form by USPS Certified Mail, return receipt requested (the Green Card). If you
choose this option, you must provide either the signed & returned green card OR track the
certified mail package number on USPS.GOV's online tracking system, and provide the tracking
print-out as proof of delivery to DCM
