This permit, issued on July 30, 1993 is subject to compliance with the application (where consistent
with the permit), all applicable regulations, special conditions and notes set forth below. Any violation of these terms may be
subject to a fine, imprisonment or civil action; or may cause the permit to be null and void.
General
1) Prior to any work at Site Nos. 1 through 19, a pre -construction meeting between the contractor
and a representative of the Division of Coastal Management must be held at the site.
Excavation and Filling
2) The area of waters and wetlands that is disturbed must be limited to the minimum necessary to
construct the utility line.
3) All excavated materials from Site Nos. 1, 2, and 3 will be removed and placed on an approved
highground site.
(See attached sheet for additional conditions)
This permit action may be appealed by the permittee or
other qualified persons within twenty (20) days of the
issuing date. An appeal requires resolution prior to work
initiation or continuance, as the case may be.
This permit must be accessible on -site to Department
personnel when the project is inspected for compliance.
Any maintenance work or project modification not
covered hereunder requires further Departmental approval.
All work must cease when the permit expires on
December 31, 1996
In issuing this permit, the State of North Carolina agrees
that your project is consistent with the North Carolina
Coastal Management Program.

ADDITIONAL CONDITIONS
4) All wetlands excavated at Site No. 2 will be returned to pre-existing contours, not to exceed the
elevation of adjacent non -disturbed wetlands.
5) The temporary placement of excavated or fill material in waters or wetlands will be for the
absolute minimum period of time necessary to accomplish the work.
6) In wetlands, the top 6" to 12" of the trench should generally be backfilled with topsoil from the
trench. r
7) The trenches and Site Nos. 1, 2, and 3 will be a minimum width necessary to place the pipe.
NOTE: This project is authorized under General Water Quality Certification #2664 issued by the
Division of Environmental Management on 4/21/92.
Sedimentation and Erosion Control
8). All excavated materials at Site Nos. 4 through 19 will be placed on highground and stabilized
so as not to enter any adjacent surface waters or wetland vegetation.
9) Any exposed slopes and streambanks must be stabilized immediately upon completion of the
utility line.
10) Stabilization is required immediately upon completion of each individual crossing.
11) Excess material must be removed to upland areas immediately upon completion of construction.
NOTE: An Erosion Control Plan was approved by the Division of Land Resources on 4/27/93.
Riuran and Utility Line Placement
12) No riprap will be placed on any wetland area at Site No. 2.
13) All riprap at Site Nos. 1 and 2 will be retained at or above the normal water line
14) Measures shall be taken to prevent live or fresh concrete from doming into contact with waters
of the State until the concrete has hardened.
NOTE: Prior to any pipe placement, the applicant must obtain all -necessary easements on both
State and private properties.

ADDITIONAL CONDITIONS
NOTE: Work within highway rights -of -way must be coordinated through the District Office of
the Division of Highways in Washington.
NOTE: Plans and specifications for the water system must be proposed by an engineer licensed
to practice in North Carolina and submitted to the Plan Review Branch of the Division
of Environmental Health prior to construction.

Environmental Setting
The project is located within sections of Long Acre Township in Beaufort County.
The specific sites are as follows:
Site No. 1
Upper Goose Creek, SR 1334 - At this location is a natural creek which is
a tributary to the Pamlico River. At the site is a ±53' long NC DOT
bridge. The creek has a width of ±23' and a depth of ±4'. Upland
vegetation extends to the normal water line of the creek. Very little if
any wetland vegetation is present.
Site No. 2
Broad Creek, NC 32 - At this location is a natural creek which is a
tributary to the Pamlico River. At the site is a ±170.8' long NC DOT
bridge. The creek has a width of ±105' and a depth of ±10.5' at normal
water. An area of wetlands (±40' in width) is located adjacent to the
creek at the northern end of the bridge. The area is vegetated with
pickerelweed. No coastal wetlands (marsh) is present. The southern
shoreline of the creek is vegetated with various upland vines.
Site No. 3
Broad Creek, SR 1326 - At this location is a channelized portion of the
creek that is described as Site No. 2. Site No. 3 is upstream of Site No.
2. At this site is a 137.4' long NC Dot bridge. The channelized creek
has a width of ±50' and a depth of 4.5' at normal water. No wetlands are
located at the site. Highground vegetated with various upland vines is
adjacent to both sides of the creek.

SR 1319, Entrance to Summer Haven Subdivision - This site is located
within a residential area adjacent to a manmade canal which connects to
the Pamlico River. The area is vegetated with upland grasses.

Private road within Summer Haven Subdivision - This site is located within
a residential area adjacent to a manmade canal which connects to the
Pamlico River. The area is vegetated with upland grasses.

Private road (Ridgewood Lane, Bay Street, Harbour Drive) within Ridgewood
Subdivision - This site is located within a residential area adjacent to
a boatharbor which connects to the Pamlico River. The area is vegetated
with upland grasses.
Site No. 7
SR 1320, Entrance to Shady Banks Subdivision - This site leads to a
residential area adjacent to a manmade canal which connects to the Pamlico
River. The area is vegetated with upland grasses.

SR 1323, Within Sunnyside Subdivision near Swanpoint - This site is
located within a residential area adjacent to a manmade canal which
connects to the Pamlico River. The project area is vegetated within
upland grasses, however, adjacent wetland vegetation is present.
Site No. 9
Dirt lane off of SR 1323 within Sunnyside Subdivision - This site is
located within a residential area adjacent to a manmade canal which
connects to the Pamlico River. The area is vegetated with upland grasses.
Site No. 10
Private road off of SR 1351 at Swanpoint - This site is located within a
residential area at the east end of a manmade canal which connects to the
Pamlico River. The area is vegetated with upland grasses.
Site No. 11
Private road off of SR 1351 at Swanpoint - This site is a continuation of
Site No. 10. The project area again within a residential subdivision
between a manmade canal which connects to the Pamlico River and the river
itself. The area is vegetated with upland grasses.
Site No. 12
Private road off of SR 1351 at Swanpoint (Duckview Drive) - This site is
located within a residential area adjacent to a manmade canal which
connects to the Pamlico River. The area is vegetated with upland grasses.

Private road off of SR 1351 at Swanpoint (Farmville Avenue) - This site is
located within a residential area at the west end of a manmade canal which
connects to the Pamlico River. The area is vegetated with upland grasses.
Site No. 14
Private road off of SR 1351 at Swanpoint - This site is located within a
residential area on the west side of a manmade canal which connects to the
Pamlico River. The area is vegetated with upland grasses.
Site No. 15
River Walk Drive at end of SR 1351 - This site is on the opposite side of
the canal that was discussed as Site No. 14. This project area leads to
a residential subdivision. The area is vegetated with upland grasses.
Site No. 16
At end of SR 1351 - This site is at the north end of the manmade canal
that was discussed at Sites No. 14 and No. 15. Again, this project area
leads to a residential subdivision. The area is vegetated with upland
grasses.
Site No. 17
Man-O-War Drive off of SR 1324 near Broad Creek - The project area is
between a manmade canal which connects to Pamlico River and the Pamlico
River itself. The project site is a residential area which is vegetated
with upland grasses.
Site No. 16
Man-O-War Drive adjacent to SR 1324 near Broad Creek - This site is a
private road which leads to a residential area. The site is at the west
end of a manmade canal which connects to the Pamlico River. The area is
vegetated with upland grasses.
Site No. 19
SR 1324 at entrance to Man-O-War Drive - This site is located on the north
side of a manmade canal which connects to the Pamlico River. The area is
vegetated with upland grasses.

Project Proposal
The applicant is proposing the following at each site:
Site No. 1
Cross upper Goose Creek with a 6" ductile iron pipe. The crossing will
have a width of ±23' and the pipe will be placed 4' below the existing
creek bottom. A ±10' wide trench will be excavated with a track excavator
and all spoils will be placed on highground. Riprap will be placed on
each side of the creek where the pipe enters the water.
Site No. 2
Cross Broad Creek with a 10" ductile iron pipe. The crossing will have a
width of ±105' of water area and ±40' of wetlands. The pipe will be
placed ±4' below the creek bottom. A ±10' to ±15' wide trench will be
excavated with a track excavator and all spoils will be placed on
highground. Riprap will be placed on each side of the creek where the
pipe enters the wetlands and the water.
Site No. 3
Cross the channelized portion of Broad Creek with a 4" ductile iron pipe.
The crossing will have a width of ±50' and the pipe will be placed 4'
below the creek bottom. A 10' to 15' wide trench will be excavated with
a track excavator, and all spoils are to be placed on highground. Riprap
will be placed on each side of the creek where the pipe enters the water.
Site No. 4
Estuarine Shoreline Site - A 6" PVC pipe will be placed through 160' of
estuarine shoreline. A 116" wide trench will be excavated within the
existing road shoulder to a depth of ±4'. Excavated materials will be
side cast on highground then redeposited within the trench once the pipe
is in place.
Site No. 5
Estuarine Shoreline Site - A 4" PVC pipe will be placed through 110' of
estuarine shoreline. A 1'6" wide trench will be excavated within the
existing road shoulder to a depth of ±41. Excavated materials will be
side cast on highground then redeposited within the trench once the pipe
is in place.

Estuarine Shoreline Site - A 2" PVC pipe will be placed through 900' of
estuarine shoreline. A 116" wide trench will be excavated within the
existing road shoulder to a depth of ±4'. Excavated materials will be
side cast on highground then redeposited within the trench once the pipe
is in place.
Site No. 7
Estuarine Shoreline Site - A 6" PVC pipe will be placed through 190' of
estuarine shoreline. A 116" wide trench will be excavated within the
existing road shoulder to a depth of ±4'. Excavated materials will be
side cast on highground then redeposited within the trench once the pipe
is in place.
Site No. 8
Estuarine Shoreline Site - A 4" PVC pipe will be placed through 160' of
estuarine shoreline. A 116" wide trench will be excavated within the
existing road shoulder to a depth of ±4'. Excavated materials will be
side cast on highground then redeposited within the trench once the pipe
is in place.
Site No. 9
Estuarine Shoreline Site - A 2" PVC pipe will be placed through 30' of
estuarine shoreline. A 116" wide trench will be excavated to a depth of
±41. Excavated material will be side cast on highground then redeposited
within the trench once the pipe is in place.

Estuarine Shoreline Site - A 2" PVC pipe will be placed through 160' of
estuarine shoreline. A 116" wide trench will be excavated within the
existing road shoulder to a depth of ±4'. Excavated materials will be
side cast on highground then redeposited within the trench once the pipe
is in place.
Site No. 11
Estuarine Shoreline Site - A 2" PVC pipe will be placed through 580' of
estuarine shoreline. A 116" wide trench will be excavated within the
existing road shoulder to a depth of ±41. Excavated materials will be

Estuarine Shoreline Site - A 2" PVC pipe will be placed through 60' of
estuarine shoreline. A 1'6" wide trench will be excavated to a depth of
±41. Excavated materials will be side cast on highground then redeposited
within the trench once the pipe is in place.
Site No. 13
Estuarine Shoreline Site - A 3" PVC pipe will be placed through 180' of
estuarine shoreline. A 116" wide trench will be excavated within the
existing road shoulder to a depth of ±41. Excavated materials will be
side cast on highground then redeposited within the trench once the pipe
is in place.
Site No. 14
Estuarine Shoreline Site - A 2" PVC pipe will be placed through 480' of
estuarine shoreline. A 1'6" trench will be excavated to a depth of ±4'.

within the trench once the pipe is in place.

Environmental Impacts
If the project is conducted as proposed, the following environmental impacts will
occur:
Site No. 1
±230 sq. ft. of natural creek bottom will be excavated. The excavated
trench will be allowed to backfill naturally. No wetland vegetation will
be excavated or filled.
Site No. 2
±1,260 sq. ft. of natural creek bottom will be excavated. The excavated
trench will be allowed to backfill naturally. ±480 sq. ft. of wetland
vegetation will be excavated.
Site No. 3
±600 sq. ft. of channelized creek bottom will be excavated. The excavated
trench will be allowed to backfill naturally. No wetland vegetation will
be excavated or filled.

CONDITIONS
1. All excavated materials from Sites No. 1, No. 2, and No. 3 will be removed
and placed on an approved highground site.
2. All wetlands excavated at Site No. 2 will be returned to pre-existing
contours and that of adjacent non -disturbed wetlands.
3. The trenches and Sites No. 1, No. 2, and No. 3 will be a minimum width
necessary to place the pipe.
4. No riprap will be placed atop wetlands at Site No. 2.
5. All riprap at Sites No. 1 and No. 2 will be retained at or above the
normal water line.
6. All excavated materials at Sites No. 4 through No. 19 will be placed on
highground and stabilized so as not to enter any adjacent surface waters
or wetland vegetation.
7. Prior to any pipe placement, the applicant must obtain all necessary
easements on both State and private properties.
8. Prior to any work at Sites No. 1 through No. 19, a pre -construction
meeting between the contractor and a representative of the Division of
Coastal Management must be held at the site.
