This permit, issued on 651-/V-000-3 is subject to compliance with the application and site drawing (where consistent
with the permit), all applicable regulations and special conditions and notes set forth below. Any violation of these terms may subject

THIS PERMIT IS TO CONSTRUCT A SINGLE FAMILY RESIDENCE,
LANDSCAPE, INSTALL SEPTIC SYSTEM AND CONCRETE DRIVEWAY!
1 All proposed development and associated construction must be done in accordance
with the approved plan.
2 Any change or changes in the plans for development, construction, or land use
activities will require a re-evaluation and modification of this permit.
3 No impermeable surfaces shall be allowed over any functional part of the septic tank
system. Do not drive over, park, pave, or build any structure over the area for the
septic tank system and the repair area.
4 All portion of the retention ditches shall be located a minimum of 25 feet from all
portions of the septic system. All portions of the water line shall be located at least
10 feet from all portions of the septic system. All pilings shall be located a minimum
of 5 feet from all portions of the septic system. The pool shall be located a minimum
of 15 feet from all portions of the septic system.
5 All disturbed areas shall be vegetatively planted and mulched within 30 days of
construction completion.
6 Pursuant to 15 NCAC, Subchapter 7J. 0406(b), this permit may not be assigned,
transferred or otherwise disposed of to a third party.
7. Proposed project shall meet all of the requirements of chapter 158. and 157.
located in Holden Beach Code of Ordinances.
This permit action may be appealed by the permittee or other
qualified persons within twenty (20) days of the issuing date.
From the date of an appeal, any work conducted under this per-
mit m..st cease until the appeal is resolved.
This permit must be on the project site and accessible to the
permit officer when the project is inspected for compliance.
Any maintenance work or project modifications not covered
under this permit requires further written permit approval.
All work must cease when this permit expires on December

8. The proposed project shall meet all of the State, Local, and Federal regulations.
9. All construction shall meet the requirements of the North Carolina State
Building Code.
10. All development shall comply with the Local Damage prevention ordinance as
required by the National Flood Insurance Program.
11. The structure must be set back 60 feet from the first line of stable natural vegetation,
as determined by the Local Permit Officer.
12. All pilings or structures must be elevated so that the bottom of the lowest supporting
horizontal member (excluding pilings or columns) is located lower than the FEMA base
flood level, current FEMA Zone X Base Flood.
13. The approved ground level has been approved to be elevation of 12 above sea level.
14. The innovative stormwater system authorized with this permit must be maintained
for the life of the structure addressed-
15. The structure must be located entirely off of the frontal dune.
16. This permit may be renewed for on(1) year if requested before the expiration date.
This permit action may be appealed by the permittee or other
qualified persons within twenty (20) days of the issuing date.

17. Any structure authorized by this permit shall be relocated or dismantled when it
becomes imminently threatened by changes in shoreline configuration. The structure(s)
tshall be relocated or dismantled within two (2) years of the time when it becomes
imminently threateded, and in any case, upon its collapse or subsidence. However, it
natural shoreline recovery or beach re -nourishment takes place within tow (2) years of
the time the structure becomes imminently threateened, so that the structure is no longer
imminently threatened, then it need not be relocated or dismantled at that time. This
conditions shall not affect the permit holder's right to seek authorization of temporary
protective measures allowed under CRC Rules.
16. The proposed project meets the allowances given by the Division of Water Quality
for the non -driveway footprint of 3250 and driveway footprint of 1890, total maximum
footprint 5140.
This permit action may be appealed by the permittee or other
qualified persons within twenty (20) days of the issuing date.

AEC HAZARD NOTICE

This notice is intended to make you, the applicant, aware
of the special risks and conditions associated with
development in this area, which is subject to natural
hazards such as storms, erosion and currents. The rules of
the Coastal Resources Commission require that you
receive an AEC Hazard Notice and acknowledge that
notice in writing before a permit foz development can be
issued.
The Commission's rules onbuildingstandards,oceanfront
setbacks and dune alteration are designed to minimize, but
not eliminate, property loss from hazards. By granting
permits, the Coastal Resources Commission does not
guarantee the safety of the development and assumes no
liability for future damage to the development.
The best available information, as accepted by the Coastal
Resources Commission, indicates that the annual ocean
erosion rate for the area where your property is located is
feet per year.
The rate was established by careful analysis of aerial
photographs of the coastline taken over the past 50 years.
Studies also indicate that the shoreline could move as
much as -330 feet landward in a major storm.
The flood waters in a major storm are predicted Lobe about

Preferred oceanfront protection measures are beach
nourishment and relocation of threatened structures.
Hard erosion control structures such as bulkheads,
seawalls, revetments, groins, jetties and breakwaters are
prohibited. Temporary devices, including sand bags, may
be allowed under certain conditions.
This structure shall be relocated or dismantled within two
years of becoming imminently threatened.
The applicant must acknowledge this information and
requirements by signing this notice in the below space.
Without the proper si application will not be
complete.
Inlet Hazard Area
SPECIAL NOTE: This hazard notice is required for
development in areas subject to sudden and massive
storms and erosion. Permits issued for developmen tin this
area expire on December 31 of the third year following the
year in which the permit was issued. Shortly before work
begins on the project site, the Local Permit Officer will
determine the vegetation line and setback distance at your
site. If thepropertyhas seen little change and theproposed
development can still meet the setback requirement, the
LPO will inform you that you may begin work_ It is impor-
tant that you check with the LPO before thepermit expires
for official approval to continue the work after the permit
has expired. Generally, if foundation pilings have been
placed and substantial progress is continuing, permit
renewal may not be necessary. If substantial progress has
not been made, the permit must be renewed and a new
setback line established. It is unlawful to continue work
after permit expiration without this approval.

BE TAKEN AS A WARRANTY THAT SEWAGE TREATMENT AND DISPOSAL SYSTEMS APPROVED AND PERMITTED WILL FUNCTION IN
A SATISFACTORY MANNER FOR ANY GIVEN PERIOD OF TEIE PERMIT IS SUBJECT TO REVOCATION IF THE SITE PLAN OR PLAT
WHICHEVER IS APPLICABLE, OR THE INTENDED USE CHANGES.
The issuance of this permit does not preclude the Pernittee from complying with any and all statutes, rules, regulations,
or ordinances which may be imposed by other government agencies (local, state, and federal) which have jurisdiction.
