Dear Mr. Lane:
The North Carolina Department of Transportation is applying for a CAMA General Permit 07H.1600 for
utility relocations associated with the U-5914 project. The proposed relocations are along SR 1554 Old
River Rd. and include trenchless installation of 569' of 4” underground power transmission line of which
40' will run within the CAMA 75' Estuarine Shoreline AEC and about 120' will be bored under coastal
wetlands, proposed trenchless install of 449' of 3" sewer force main of which 32' will be bored under coastal
wetlands, and proposed trenchless installation of 555' of 4” telecommunications line of which 110' will be
bored under coastal wetlands. No coastal wetlands or estuarine waters will be impacted by the proposed
utility relocations, and all bore pits will be located within high ground outside the AEC. Frac-out
contingency will be required and appropriately permitted waste disposal sites will be identified for drilling
slurry if necessary. There will be no additional work within CAMA regulated areas from the proposed U-
5914 roadway project. Section 404 and 401 permits will be submitted at a later date.

NCDOT proposes improvements to approximately 0.35 miles of NC 133 in Belville from SR 1551 (Blackwell Rd.) to SR 1554
(Old River Rd.). Proposed improvements include road widening to provide turning lanes at the intersection of River Road and
Blackwell Road and the intersection of River Road and Old River Road as well as providing a 10-foot multi-use path on the
west side of the roadway.
c. Describe the proposed construction methodology, types of construction equipment to be used during construction, the number of each type
of equipment and where it is to be stored.
Excavators will be used for digging bore pits for utilities and directional drills will be used for trenchless installation of utilities.
Equipment will be stored on highground within the R/W or at the nearby NCDOT Bridge Maintenance yard. All bore pits will
be located entirely within highground outside the AEC. Frac-out contingency plans will be in place.
d. List all development activities you propose.

At the beginning of the project, on the northwest end, an existing curb and gutter drainage system on NC 133 ties into a new
system of drop inlets and ditches that drain into an existing wetland on the south side of the road. Further south, roadway
ditches carry runoff to a 36” reinforced concrete pipe that flows from a wetland on the south side of NC 133 to the same
wetland as the 54” reinforced concrete pipe on the north side of the road.

In addition to this completed application form, (MP-1) the following items below, if applicable, must be submitted in order for the application
package to be complete. Items (a) – (f) are always applicable to any major development application. Please consult the application
instruction booklet on how to properly prepare the required items below.

The construction of the culvert will consist of two phases. The first phase will consist of constructing a stilling basin, temporary diversion channel and impervious dikes. In the
second phase the existing 36" RCP will be removed, and the double 7'x6' RCBC will be constructed. The inlet and outlet channels will also be constructed, and bank stabilization
will be placed, then impervious dikes, diversion channel and stilling basin will all be removed. Once construction is finished all disturbed areas will be stabilized.

Roadway fill slopes were steepened to as much as 1.5:1 in areas to prevent weltand impacts.

NCDOT is proposing to enhance approximately 0.24 miles of NC 133 in Belville, NC (Brunswick County) from SR 1551 (Blackwell Road) to SR 1554 (Old River Road) along with
pedestrian and bicycle facilities through the construction of a single multi-use path on the west side of the roadway. All work will occur within the existing 120' right of way along
NC-133. This site consists of CAMA and 404 wetlands.
Ditch velocities are designed to be non-erosive for the Q10 storm
Riprap energy dissipators are used at pipe outlets to reduce discharge velocity to non-erosive levels.

(Description of Minimization of Water
Quality Impacts)
A 36" RCP at -Y1- Sta. 14+10 will also be replaced with a double 7'x6' RCBC. The existing 36" pipe is undersized. With wetlands present on both sides of the roadway, a
double 7'x6' RCBC is being proposed to help alleviate existing flooding issues on Old River Road. Rip rap is being used at the culvert outlet to provide bank stabilization.
The project impacts one jurisdictional stream, an unnamed tributary to the Brunswick River. At -L- Sta. 22+03, there is an existing 36" RCP. Existing wetlands have been
identified on the upstream and downstream ends of the crossing. The proposed design calls to replace this pipe with a 66" RCP on the upstream side and 2@54" RCP on the
downstream side. The existing pipe inverts do not promote positive drainage and the inlet end of the existing pipe is broken. The new pipe inverts will be placed at existing
ground elevations. Rip rap is used at the pipe outlet to prevent scour and erosion.
