This permit, issued on June 14, 2018 is subject to compliance with the application (where consistent
with the permit), all applicable regulations, special conditions and notes set forth below. Any violation of these terms may
1) Unless specifically altered herein, this Minor Modification authorizes the docking facility
reconfiguration and boat lifts, all as expressly and specifically set forth in the attached letter and
workplan drawing. Any additional development activities may require a modification of this permit.
2) This permit does not authorize any additional formalized boat slips.
3) Should the adjacent property to the south be sold prior to the initiation of construction of the docking
facility, the permittee shall be required to obtain a signed riparian corridor waiver from the new
property owner prior to the placement of any structures within the 15 foot riparian corridor.
(See attached sheet for additional conditions)
This permit action may be appealed by the permittee or other
qualified persons within twenty (20) days of the issuing date.
This permit must be accessible on -site to Department
personnel when the project is inspected for compliance.
Any maintenance work or project modification not covered
hereunder requires further Division approval.
All work must cease when the permit expires on
December 31, 2019
In issuing this permit, the State of North Carolina agrees that
your project is consistent with the North Carolina Coastal
Management Program.

ADDITIONAL CONDITIONS
USACE Conditions
No portion of any structure shall be located within the bottom edge of the federally maintained channel.
An as -built survey shall be performed on the docking facility, and copies of the survey provided to the
U.S. Army Corps of Engineers and Division of Coastal Management, within 60 days of completion of
construction of these portions of the project.
General
The permittee and/or his or her contractor shall meet with a representative of the Division prior to
project initiation.
This Minor Modification shall be attached to the original Permit No. 36-16, which was issued on
3/14/16, and copies of both documents must be readily available on site when Division personnel inspect
,.the project for compliance.
All conditions and stipulations of the active permit remain in force unless specifically altered herein.
OTE: The U.S. Army Corps of Engineers assigned the proposed project COE Action Id. No. SAW-
2014-01342.
The Division of Water Resources assigned the proposed project DWR Project No. 15-0968.
A minor modification application processing fee of $100 was received by DCM for this project.

The enclosed permit constitutes authorization under the Coastal Area Management Act,
and where applicable, the State Dredge and Fill Law, for you to proceed with your project
proposal. The original (buff -colored form) is retained by you and it must be available on site
when the project is inspected for compliance. Please sign both the original and the copy and
return the copy to this office in the enclosed envelope. Signing the permit and proceeding
means you have waived your right of appeal described below.
If you object to the permit or any of the conditions, you may request a hearing pursuant
to NCGS 113A-121.1 or 113-229. Your petition for a hearing must be filed in accordance with
NCGS Chapter 150E with the Office of Administrative Hearings, 6714 Mail Service Center,
Raleigh, NC 27611-6714, (919) 733-2698 within twenty (20) days of this decision on your
permit. You should also be aware that another qualified party may submit an objection to the
issuance of this permit within twenty (20) days.
The project plan is subject to those conditions appearing on the permit form. Otherwise,
all work must be carried out in accordance with your application. Modifications, time extensions,
and future maintenance requires additional approval. Please read your permit carefully prior to
starting work and review all project plans, as approved. If you are having the work done by a
contractor, it would. be to your benefit to be sure that he fully understands all permit
requirements.
From time to time, Department personnel will visit the project site. To facilitate this
review, we request that you complete and mail the enclosed Notice Card just prior to work
initiation. However, if questions arise concerning permit conditions, environmental safeguards,
or problem areas, you may contact Department personnel at any time for assistance. By
working in accordance with the permit, you will be helping to protect our vitally important coastal
resources.

On behalf of the subject project owners, we submit this Minor Modification Request for
proposed changes to three of the previously approved piers and boat slip areas. The project permit
references are: Coastal Resource Commission Permit No. 36-16, issued on March 14, 2016, and
Action ID No. SAW-2014-01342. Enclosed you will find documentation from the adjacent
property owner notifications, the proposed sketch plan, and the $100 application fee.
As depicted in the attached sketch plan, the following changes are proposed:
Modify the ends of two piers from fixed to floating platforms, shown as 12' x 5' and
8'-5" x 6' in size. Overall footprints will remain per previous plans. Access to the
floating docs will be via ladders affixed to the ends of each pier.
Install two boatlifts, depicted in the proposed 13' x 7' areas designated.
All previously approved project features have been constructed, except the seven 8" round
stop piles. This proposed modification does not alter the requirement for placement of these piles,
which shall be installed prior to use. The boat lifts would attach to the existing fixed pier structure.
The other two fixed pier ends would be replaced with floating dock structures as depicted.

Development permit to undertake development activities at 68 Champion Drive, in Vamamtown,
Brunswick County.
Although processing of the application is nearing completion, additional time is needed for this
office to complete the review and make a decision on your request. Therefore, it is necessary that
the standard review time be extended. An additional 75 days is provided by G.S. 113A-122(c)
which would make January 31, 2016, the new deadline for reaching a decision on your request.
However, we expect to take action prior to that time and will do so as soon as possible. In the

Supply Group, Inc. - Lewis Dock Project
Varnamtown, Brunswick County
Mr. Michael Norton with McGill Associates, P.A. is requesting a minor modification to
State Permit #36-16 on behalf of the Supply Group, Inc. State Permit #36-16 was issued on March
11, 2016, authorizing the replacement of an existing bulkhead, excavation of a new boat basin, and
reconfiguration of an existing docking facility to provide mooring for vessels adjacent to the
Lockwoods Folly River at Vamamtown, Brunswick County. All of the structures authorized under
the permit have been constructed except for seven (7) 8" round stop pilings. State Permit #36-16
is due to expire on December 31, 2019.
Mr. Norton is requesting a modification to State Permit #36-16 to allow for the conversion
of two fixed piers to two floating platforms and the installation of two boatlifts within two
previously approved boat slips. The applicant states that at the ends of the two middle 6' wide
existing fixed piers, a 12' long section and an 8.5' long section of the piers would be converted to
floating structures. The floats would require six (6) additional 8" diameter pilings and would be
accessed by ladders at the ends of the existing fixed piers. Two (2) 13' by 7' boatlifts would
occupy two authorized boat slips as shown on Sheet 1 of 1 of the project plan. The proposed floats
and lifts would not encroach within the US Army Corps channel setback.
The applicant has submitted a check (check No. 2012) in the amount of $100 for the
modification request along with the letters and proof of notification to the adjacent riparian
property owners. As proposed, this office has no objection to the proposed project modification as
long as there are no objections from the other agencies to this proposal. It appears that the project
continues to be CONSISTENT with the use standards set forth in 15A NCAC 07H .0206. This
continues to be CONSISTENT with the general use standard enumerated in 15A NCAC 07H
.0208(b)(6).

Dear Mr. Lewis:
Through coordination with the North Carolina Division of Coastal Management, we have
learned of your request to modify the work associated with your Department of the Army (DA)
permit issued on March 23, 2016, which authorized the replacement of an existing bulkhead and
expand a docking facility located at 68 Champion Drive SW in Vamamtown, Brunswick County,
North Carolina. The requested modifications include requests to modify the existing permit by
allowing the conversion of two fixed piers to two floating platforms and the installation of two
boatlifts within two previously approved boat slips.
Your proposal continues to be consistent with the provisions and objectives of general
permit No. 19800291 (copy enclosed). The permit is hereby modified. It is understood that all
other conditions of your permit remain applicable in addition to the following special condition.
The expiration date of your DA permit is December 31, 2021.
1. An as built drawing is required and a post construction inspection/survey is required to
confirm that the project is not located within the U.S. Army Corps of Engineers Federal
Channel Setback

Hey Maria, I've been alright with floaters in 2' of water in the past before, sometimes with stops depending on the
location. But in this case, the depths increase to 6' over the 8.5' length of the proposed floating dock. The minimal
amount of floating dock that might hit the bottom on extreme tides doesn't cause concern for significant impacts. The
other floating dock to the north seems to have plenty of water. I'm okay with what's proposed.

I didn't give a call yesterday afternoon since Shaun had sent an email. I'll look at it today, though I've already glanced
through the scanned notes you provided in 2015/2016. The biggest deal would be if the floating structure rests on the
bottom since it's PNA and around -2'.

Stephen Lane is working on a CAMA Major Permit for B-5413, Bridge 20 over Smith Creek in Beaufort
County. I have not received some of the supporting paperwork in Raleigh. I talked briefly with Stephen about
this the other day, and he thought it was mailed already. Could you and he talk and try to figure out what
happened? Just in case this has anything to do with it, FYI, my mail needs to be addressed c/o DWR Wetlands
Unit, otherwise the people that distribute the mail here in Raleigh don't always know where I am. There are a
lot of people that share the same mailing address. If you did send it already, there are a few places I can check
in the building before you send it again.
Still needed: Public Notice, Placard, Major Permit Application Computer Sheet, Acceptance Letter, and
Application and Transmittal Processing Record.

GENERAL NOTES:
A NC Coastal Resource Commission Permit (No. 36-16) was issued on March
14, 2016 for this project site. Action ID No. SAW-2014-01342 was also assigned
to the subject work.
The owner, Supply Group Inc., wishes to modify the three existing fixed -pier
structures. It is desired to replace two of the previously approved boat slips with
13' x 7' boat lifts, and place two floating docks (12' x 6' and 8'-5" x 6' dimensions)
on the end of two previously approved fixed piers. All previously approved
project features have been constructed, except the seven 8" round stop piles.
This proposed modification does not alter the requirement for placement of
these piles, which shall be installed prior to use. The proposed floating docks
would overlay the same footprint of fixed piers and not change the footprint. The
floating docks would be accessed by vertical ladders placed at the ends of the
fixed piers.
