1) This renewal shall be attached to the Permit No. 11-16, which was issued to the permittee on 1/28/16,
as well as all subsequent renewals, refinements and modifications, and copies of all documents shall
be readily available on site when a Division representative inspects the project for compliance.
2) All conditions and stipulations of the active permit remain in force under this renewal.
NOTE: The N.C. Division of Water Resources has assigned the proposed project DWR Project No.
15-1280.
NOTE: The U.S. Army Corps of Engineers has assigned the proposed project SAW Action Id. No.
2015-02613
NOTE: A permit renewal application processing fee of $100 was received by DCM for this project.
This permit action may be appealed by the permittee or
other qualified persons within twenty (20) days of the issuing
date.
This permit must be accessible on -site to Department
personnel when the project is inspected for compliance.
Any maintenance work or project modification not covered
hereunder requires further Division approval.
All work must cease when the permit expires on
December 31, 2021

The request for extension of this major permit has been reviewed pursuant to the provisions
of reference (b). Reference (a) provided the required signed and dated attestations.
This office concurs that substantial development has begun and is continuing.
Changes in Coastal Resources Commission's rules and development standards since the
date of issuance of this permit are not considered relevant to the permitted development,
and do not raise concern for denial or modification of this permit.
It is recommended that a two-year extension of this permit be granted.
The required fee of $100, paid by check number 5770, was received on 12/16/19
Also attached is a copy of the subject permit, and the Extension request letter.

This is a request for the first renewal of CAMA Major #11-16 (the "Permit"), issued to Stevens Towing Co.
for the construction of a barge terminal. The Permit was issued on January 28, 2016 with modifications
issued on October 28, 2016 and June 16, 2017.
Steven's Towing Co. has completed most of the permitted work, but it is important to maintain this permit
for the purpose of future maintenance dredging and for completion of some components of the water
dependent upland development, including the barge washdown collection and treatment system. In
addition, it is possible that future activities will warrant modification to this permit in the future.
I have enclosed the $100 renewal processing fee along with a copy of the permit and three copies of the

Upland Development
1) Unless specifically altered herein, this permit authorizes the grading and other land disturbing
activities associated with the development of the above referenced property, including the building,
barge ramp/bay, roads, spoil retention areas, and basin, all as expressly and specifically set forth in
the attached permit application and work drawings.

This permit action may be appealed by the permittee or other
qualified persons within twenty (20) days of the issuing date.
This permit must be accessible on -site to Department
personnel when the project is inspected for compliance.
Any maintenance work or project modification not covered
hereunder requires further Division approval.

Bulkhead & Riarap D Er,
12) The alignment of the authorized bulkhead and riprap shall be staked by a representative of the Division of
Coastal Management within a maximum of 30 days prior to the start of construction.
13) The bulkhead shall be constructed prior to any backfilling activities.
14) The bulkhead shall be structurally tight so as to prevent seepage of backfill materials through the
structure.
1 S) The backfill material shall be clean and free of any pollutants except in trace quantities.
16) All backfill material shall be obtained from a high ground source and confined behind the permitted
bulkhead.
17) No backfill material is to be placed within 30 feet of the normal water line, except that which will be
used to backfill the area behind the permitted bulkhead.
18) The riprap material shall consist of clean rock or masonry materials such as, but not limited to, granite,
marl, or broken concrete. It shall be of a size sufficient to prevent its movement from the site by wave or
current action.
Barge Ramp
19) The boat ramp shall either be poured at an upland location and pushed into place once hardened, or
poured entirely within a watertight containment structure. Live concrete shall not be allowed to contact
waters of the State or waters that will enter waters of the State.
20) Placement of fill materials below normal water level shall be limited to the ramp structure.
21) Excavation and ground disturbing activities, associated with the placement of the boat ramp, above and
below the normal water line shall be limited to that absolutely necessary to establish adequate ramp
slope and provide a ramp no greater in size that specified in the attached permit application and
workplan drawings.
Barge Landing Facility
22) This permit authorizes only the concrete working platform, dolphin piles and other structures and uses
located in or over the water that are expressly and specifically set forth in the permit application. No
other structure, whether floating or stationary, shall become a permanent part of this barge landing
facility without permit modification. No non -water dependent uses of structures shall be conducted on,
in or over Public Trust waters without permit modification.

ADDITIONAL CONDITIONS
NOTE: The U.S. Army Corps of Engineers authorized the proposed project by way of Tier II
Programmatic General Permit 198000291 (Action ID No. SAW-2015-02613), which was issued
on 1/21/16.
NOTE: The N.C. Division of Water Resources authorized the proposed project by way of General Water
Quality Certification 3900 and assigned the project DWR Project No. 15-1280.

NOTE: Future development of the permittee's property may require a modification of this permit. }
Contact a representative of the Division at (252) 264-3901 prior to the commencement of any
such activity for this determination. The permittee is further advised that many non -water
dependent activities are not authorized within 30 feet of the normal water level.
NOTE: An application processing fee of $475 was received by DCM for this project. This fee also
satisfied the Section 401 application processing fee requirements of the Division of Water
Resources.

2) This Minor Modification shall be attached to the original Permit No. 11-16, which was issued on
