dated 6/3/19 and 8/2/19.
This permit, issued on December 20, 2019 is subject to compliance with the application (where consistent
with the permit), all applicable regulations, special conditions and notes set forth below. Any violation of these terms may

1) This renewal shall be attached to Permit No. 17-16, which was issued to the permittees on 2/8/16, as
and copies of both documents shall be readily available on site when a Division representative
inspects the project for compliance.
2) All conditions and stipulations of the active permit remain in force under this renewal.
NOTE: The N.C. Division of Water Resources has assigned the proposed project DWR Project No.
15-0929 and 15-0929 V2.
NOTE: The U.S. Army Corps of Engineers has assigned the proposed project SAW Action Id. No.
2015-01937.
NOTE: A permit renewal application processing fee of $100 was received by DCM for this project.
This permit action may be appealed by the permittee or
other qualified persons within twenty (20) days of the issuing
date.
This permit must be accessible on -site to Department
personnel when the project is inspected for compliance.
Any maintenance work or project modification not covered
hereunder requires further Division approval.
All work must cease when the permit expires on
December 31, 2021
In issuing this permit, the State of North Carolina agrees
that your project is consistent with the North Carolina Coastal
Management Program.
Signed by the authority of the Secretary of DEQ and the Chair
of the Coastal Resources Commission.

Dear Mr. Spivey
Rita McCoy, Mark McCoy's wife and I would like to renew our permit no.17-16.
1- We plan to begin work on the project in the next 60 days and complete within a
reasonable time.
2- Do to Mark McCoy's sickness with cancer and ultimate death we could start the
project as planned. His son, Dr. Andrew McCoy is now in a position to take over
some responsibility so we can get the project started but due to the amount of
work to be done we can't complete in the current time allotted.
3- There is no change in the plans since the issuance of the original permit with no
modifications.
4- There is no change of ownership except Rita McCoy now has %100 of Mark
McCoy's interest due to his death.
5- The project is in compliance with all conditions of the current permit.

CAMA Major Permit No. 17-16 was issued to Mark McCoy & Wayne Woolard February 8,
2016. Due to sickness and death of owner work has not been completed and will not be
completed by the expiration date of December 31,2019. Permit work would stay within the
original permit guidelines.
I recommend that we renew CAMA Major Permit No.17-16 as requested
The $100 fee to cover the cost of renewal has been deposited in the Department's account if

CAMA Major Permit #17-16 was issued to Mark McCoy and Wayne Woolard on 8 February 2016 for
development located off SR 1308 at the terminal (east) end of the Mimosa Shores subdivision adjacent Pamlico
River and Cypress Knee Gut (aka Riverside Canal) near the City of Washington in Beaufort County, North Carolina.
The Permit expires on 31 December 2019. Mr. Mark McCoy passed away and his wife Rita B. McCoy has
requested that her name replace her late husbands on the CAMA Major Permit #17-16. Additionally, Mr. Wayne
Woolard has requested that the Permit be renewed. This renewal is currently being processed.

This permit, issued on February 8, 2016 , is subject to compliance with the application (wher
consistent with the permit), all applicable regulations, special conditions and notes set forth below. Any violatioi
of these terms maybe subject to fines, imprisonment or civil action; or may cause the permit to be null and voic
Boat Basin
1) Excavation shall not exceed -5 feet below the normal water level. In no case shall the depth of
excavation exceed the depth of connecting waters.
2) A turbidity curtain shall be left between the upland basin area to be excavated and the existing canal
and a 24 hour period after completion of excavation shall elapse prior to curtain removal to prevent
unnecessary siltation into the adjacent water body.
3) The temporary placement or double handling of excavated or till materials within waters or vegetated
wetlands is not authorized.
4) No excavation shall take place outside of the alignment of the area indicated on the workplan
