This permit, issued on April 21, 2011, is subject to compliance with the application and site drawing (where consistent with the permit), all applicable regulations and special conditions and notes set forth below. Any violation of these terms may subject permittee to a fine, imprisonment or civil action, or may cause the permit to be null and void. This permit authorizes: Building a new single family residence inside the 75 foot AEC and a 6' path that will interconnect with the existing dock: (1) All proposed development and associated construction must be done in accordance with the permitted work plat drawings(s) dated received on March 28, 2011. (2) All construction must conform to the N.C. Building Code requirements and all other ko 1, State and Federal regulations, applicable local ordinances and FEMA Flood Regulations. (3) Any change or changes in the plans for development, construction, or land use activities will require a re-evaluation and modification of this permit. (4) A copy of this permit shall be posted or available on site. Contact this office at 579-6747 ext 1 for a final inspection at completion of work. Graham Mirror Permit # 02-2011 Date: April 21, 2011 Page 2 (5) The amount of impervious surface shall not exceed 30% of the lot area within 75 feet of Normal High Water (Estuarine Shoreline Area of Environmental Concern), in this case, a maximum of 1716 square feet is authorized. (6) Unless specifically allowed in 15A NCAC 07H. 0209(d)(10), and shown on the permitted plan drawing, all development1construction shall be located a distance of 30 feet landward of Normal High Water Line. No portion of the roof overhang shall encroach into the 30 R. buffer. (7) All unconsolidated material resulting from associated grading and landscaping shall be repined on site by effective sedimentation and erosion control measures. Prior to any land -disturbing activities, a barrier line of filter cloth must be installed between the land disturbing activity and the adjacent marsh or water areas, until such time as the area has been property stabilized with a vegetative cover. (8) All otherdisturbed areas shall be vegeptivdy stabilized (planted and mulched) within 14 days of construction completion. (9) If any soil within the 75 foot AEC is disturbed, erect a silt fence outside the 30 foot CAI A line. Sedimentation and erosion control measures shall be maintained through the construction period or removed by approval of the LPO Official. (10) Prior to any development or after placement of erosion control measures, contact the LPO Official at 579-6747 ext 1 to schedule an inspection. (11) A silt fence shall be installed and maintained throughout the construction period and shall not be removed until the construction is complete or by approval of the LPO Official. On subsequent review, I have discovered that additional Information Is needed to complete the review process, Accordingly, I am requesting that you submit the following additional information to this office: 1. In application, under Land Owner, provide correct zip code. 2. In application, under Location of Project provide name of adjacent waterbody.. 3. In application, under Size of Lot I Parcel indicate correct square feet and acres. 4. In application, under (2) Coastal Shoreline AEC's confirm correct square feet of impervious or built upon surfaces. Note: Drawings only show house wl roof drip edge which Is noted as 1680 square feet, Provide calculations which will account for the balance, Also note that these impervious calculations shall take into account a 6' wile natural direct path from the base of the rear stairs to the 30' CAMA line for the purpose of access to the water (Calabash River) unless drawings indicate a pervious connecting walk, such as a raised open slated wood walkway. 5. The adjacent property letters signed by Terry Frink and Vicki Reaves are invalid since there is no receipt for certified mail. You may use either form attached but select the coned fused on how you wish to contact the adjacent landowners. A copy of this CAMA application including drawings shall the sent to both adjacent landowners and a copy of each with the appropriate form attached shag be submitted to the Town of Calabash CAMA LPO. 6. On Town of Calabash CAMA Site Calculation Worksheet, confirm square footage of Total of land disturbed within 75' AEC. If area of disturbance is outside the roof drip edge, then these areas must be delineated (hatched) on yourslte plan. 7. On site plan, label bulkhead as Normal High Water lire (NHL), Lo. Bulkhead = Normal High Water. Please note. - What you provide on you site plan Is what you are allowed to do on your property. If you decide to after something within the 75' AEC that Iremeses your impervious calculations or disturbs land outside the scope Indicated on the site plan, than you will most likely need to go thru this process again including contacting your adjacent neighbors, wailing thru another advertisament period, and the appibstion fee. Many of the above items can be changed by hand on the otolnel appltadpn and site plan, however the notification of the adjacent landowners must be resubmitted.
