in an area of environment concern pursuant to Section 113A-118 of the

Drive SW, in Shallotte, NC , as requested in the permittee's application, dated 25-Sep-09. This permit, issued on
22-Oct-09, is subject to compliance with the application and site drawing (where consistent with the permit), all
applicable regulations and special conditions and notes set forth below. Any violation of these terms may subject
permittee to a fine, imprisonment or civil action, or may cause the permit to be null and void.
This permit authorizes: Garage/Boat Shed/Fill & Grade/Repair of Septic System including Installation of Interceptor Drain
(1) All proposed development and associated construction must be done in accordancewith the permitted work plat
drawings(s) dated received on 05-Oct-09.
(2) All construction must conform to the N.C. Building Code requirements and all other local, State and Federal regulations,
applicable local ordinances and FEMA Flood Regulations.
(3) Any change or changes in the plans for development, construction, or land use activities will require a re-evaluation and
modification of this permit.
(4) A copy of this permit shall be posted or available on site. Contact this office at 910.253.2034 for a final inspection at
completion of work.

This permit action may be appealed by the permittee or other qualified persons
within twenty (20) days of the issuing date. From the date of an appeal, any
work conducted under this permit must cease unfit the appeal is resolved. This
permit must be on the project site and accessible to the permit officer when the
project is inspected for compliance. Any maintenance work or project
modification not covered under this permit, require further written permit
approval. All work must cease when this permit expires on:

In issuing this permit it is agreed that this project is consistent with the local Land
Use Plan and all applicable ordinances. This permit may not be transferred to
another party without the written approval of the Division of Coastal
Management.

(5) The amount of impervious surface shall not exceed 30% of the lot area within 75 feet of Normal Hiigh Water
(Estuarine Shoreline Area of Environmental Concern), in this case, 883square feet is authorized.
(6) Unless specifically allowed in 15A NCAC 07H. 0209(d)(10), and shown on the permitted plan drawing, all
developmenticonstruction shall be located a distance of 30 feet landward of Normal High Water. No portion of the
roof overhang shall encroach into the 30 ft. buffer.
(7) All unconsolidated material resulting from associated grading and landscaping shall be retained on site by effective
sedimentation and erosion control measures. Prior to any land -disturbing activities, a barrier line of filter cloth must
be installed between the land disturbing activity and the adjacent marsh or water areas, until such time as the area
has been properly stabilized with a vegetative cover.
(8) Any proposed for grading within the 30' buffer from the Normal High Water must be contoured to prevent additional
stormwater runoff to the adjacent marsh. This area shall be immediately vegetatively stabilized, and must remain in
a vegetated state.
(9) All other disturbed areas shall be vegetatively stabilized (planted and mulched) within 14 days of construction
completion.

RE: Issuance of a Repair Permit for 1352 Riverbend Drive, Shallotte, 28470

Mr. Musi:
This office received an application to visit your site to determine if a new on -site wastewater
system serving a three -bedroom residence could be permitted on the property referenced above.
On August 18, 2009, I went to the site to evaluate the soils. The soil/site evaluation was
completed according to the North Carolina Laws and Rules for Sewage Treatmen6 and
Disposal Systems using the hand auger method.
Findings
The property referenced above is utilizing an existing system for an existing residence. The
residence is a three bedroom single family dwelling. The wastewater system includes a septic
tank and one drain line. There was no evidence of sewage/effluent surfacing to the ground at the
time of my visit. However, the ground can be spongy/squishy occasionally where the drain line
is located and it is very obvious in the yard due to the heavy vegetation growth. Also, part of the
drain line is located within 50 feet of mean high water mark for the Shallotte River classified as
Shellfish Area (SA) waters. Because of these findings, this office is recommending you to repair
your septic system.
The soil borings were evaluated using a hand auger. During the evaluations notes were collected
in regard to soil texture, landscape position, soil morphology (color of the soil) and available
space. The most limiting soil texture was loamy sand (Group I Soils) in the area of the
replacement septic system. The landscape position for this lot varies from a shoulder slope
position (convex slope) to a linear side slope position. The replacement wastewater system is
located on the lower side of the linear side slope landscape position, which is nearest to the
Shallotte River. The seasonal high water table is determined by soil morphology (color). The
soil that contains chroma 2 colors or less (referencing the Munsell Color Chart) within 12 inches
of the natural occurring soil surface is classified unsuitable. The soil color with chroma 2 or less
was noted at depths less than 12 inches however, they were determined to be relic features.
These features were present to a depth of 42 inches.
Since the system is located on the lower side of the linear side slope an interceptor drain must be
installed. The interceptor drain is to disrupt the lateral water movement that flows through the
soil from the higher landscape position. '
Reauirement
Obtain a CAMA permit from the county and then submit to the health department a repair
application for an replacement on -site wastewater system.
Once the CAMA permit is issued and you have submitted the repair application to this office, a
pre -construction conference will be required to discuss the installation standards and the required
site visit this office will need to make to assure the interceptor drain is installed accordingly.
Once the pre -construction conference is completed the site modification (installation of the
interceptor drain) can begin. The following specifications and the schematics enclosed for
installing the interceptor drain must be utilized.
Interceptor Drain
• 6-8 inch sock tile located along the front/right side of the existing dwelling.
• Trench Dimensions 1' x 48'
• Trench Bottom Depth is 44 inches (3 ft 8 in) below top of septic tank or 83 (6 ft 11 in)
below top of the front porch near the column right of the door way.
• Place 2" of stone in the bottom of the trench then place the sock tile on top of that stone.
• Fill the rest of the trench with stone to 10 — 6 inches from the soil surface.
• Place a filter cloth on the rock stone then back fill with loamy sand top soil.
• Site shall be landscaped to shed surface water away from the wastewater system(s).
Grass vegetation shall be established (seed or sod) immediately after system installation.
• Minimum site disturbance shall occur at the site. The proposed drain -field area shall be
protected at all times (prior to and post -construction). Area shall not be exposed to
vehicular traffic.
• The stone rock used must be number 4 or 5; not 57 or smaller (crush and run material
like).
All site modifications must be completed prior to issuing the Authorization to Construct Permit.
Conventional Repair/Replacement Wastewater System
• Existing use = a 3-bedroom SFR dwelling unit (2-occupant)
• Daily sewage design flow -rate (maximum) = 360 gpd
• Soil limitation(s) encountered:
Soil Wetness Condition... approximately 42-54 inches below the natural soil surface.

• Long Term Acceptance Rate is 0.9 gallons per day per square feet
• Total nitrification/treatment and disposal field square footage = 900-square feet
• Total linear footage of laterals = 198-linear feet (minimum)
• Number of Lines is 6 each 31.5'(length)
• Bed Dimension 18' (wide) x 33' (length)
• End distribution: REFERENCE THE SITE PLAN
• Drain line spacing = 3-foot on -center
• Drain line installation = Trench Bottom Depth (TBD) must be — 26 inches (below) the
bench mark elevation (BME).
• BME = Top of the tank on the outlet side
• Required site modifications
o Excavate all of the existing drain lines and haul to an approved location for
disposal. Replace the existing drain field with clean group 1 soil.
• Site shall be landscaped to shed surface water away from the wastewater system(s).
Grass vegetation shall be established (seed or sod) immediately after system installation.
• Minimum site disturbance shall occur at the site. The proposed drain -field area shall be
protected at all times (prior to and post -construction). Area shall not be exposed to
vehicular traffic.
• If needed; all utilities lines and storm water diversion pipe will need to be re-routed
outside of the system area. If water lines are to be installed for irrigation purposes make
sure these lines are a minimum of 10 feet (horizontally) from any part of the wastewater
system.
Enclosed is a site plan and cut sheets for installation purposes. Follow the detail schematic
drawings for installing the interceptor drain and drain field. If you have any further question
