On behalf of The Bonanza Group, LLC, who owns 21 Causeway Drive, Ocean Isle Beach in
Brunswick County, I am requesting a 5-year maintenance excavation renewal of State Permit
480-18. State Permit 480-18 was issued to The Bonanza Group, LLC on 7/31/18 for the
construction of a commercial docking facility and excavation of an existing canal and the permit
is due to expire on 12/31/21.
There has not been any changes to the authorized project or ownership of the property and the
project/docking facility is in compliance with all conditions of the current permit.
A check (# 1110) in the amount of $100 is enclosed for the required processing fee, a copy of the
authorized agent form and a copy of the original permit. Please feel free to contact me if you

This permit, issued on July 31,.2018 _ T . , is subject to compliance with the application (where consistent
with the permit), all applicable regulations, special conditions and notes set forth below. Any violation of these terms may
be subject to flies, imprisonment or civil actiM or may cause the permit to be -null and void.
Docking Facility
1) This permit authorizes only the docks, piers, and other structures and uses located in or over the
water that are expressly and specifically set forth in the permit application. No other stricture,
whether floating or stationary, shall become a permanent part of this docking facility without permit
modification. No non -water dependent uses of structures shall be conducted on; in or over Public
Trust wafers without permit modification.
2) No sewage, whether tremed oruntreated, shall be discharged at any time from any boats using the
docking facility. Any sewage discharge at the docking facility shall be considered a violation of this
permit for which the permittee`is responsible. This prohibition shall be applied and enforced
throughout the entire existence of the permitted. structure.

This permit action may be appealed by the permittee or other
qualified persons within twenty (20) days. of the issuing date.
This permit must be accessible on -site to Depamnent
personnel when the project is inspected for compliance_
Any maintenance work or project modification not covered

.11 The permittee shall maintain the authorized warp in good condition and in conformance with the terms
and conditions of this permit. The permittee is not relieved of this requirement if he or she abandonsfihe
Permitted activity without having it Transferred to a third party.
4) The permittee shall install and maintain at his experEse any signal lights or signals prescribed by the D.S.
Coast Guard, through regulation or othemise, on the authorized facilities. At a minimum, permanent
reflectors shall be attached to the structure in order to make it more visible during hours of darkness. or
inclement weather.
5) This permit .authorizes a maximum of 3 formalized boat slips. Decking space on the landward side of the
floating dock indicating. Slip No. 3 is not authorized under this permit.
6) This perruit does not authorize any tie pilings associated with the docking facility.
7) The docks and associated. structures shall have a minimum setback distance of 15 feet between any pates
of the structures and the adjacent property owners.' riparian access corridors.
8) No portion of the authorized docking facility shall extend more than one quarter of the width of the water
body. Measurements to determine the width of the water body shall be made from the waterward edge of
any coastal, wetland vegetation which borders the water body.
9) hi order to ensure compliance with Conditions 5-8, an as -built survey shall be performed of the docking
facility and copies of the survey provided to the ;Division of Coastal. Management within b� days of
completion of construction: of this portion of the project.
1 Q) No attempt shall be made by the permittee to prevent the full and free use by the public of all navigable
waters at or adjacent to the authorized work.
Excavation
11) Excavation shall not exceed -6' below the normal: low water. fn no case shall the depth of excavation
exceed the depth of connecting waters.
12) No vegetated wetlands shall be excavated or filled. No excavation shall take place within 10' of any
Coastal Wetlands.
13) The temporary placement of double handling of fill materials within. waters or vegetated wetlands is not.

16) The spoil disposal area & offloading site shall be. inspected and approved by a Tepxesentative of the
Division of Coastal Management prior to the commencement of any dredging activities.
Maintenance Clause
l The Division of Coastal Management shah. be notified in writing at least two (2) weeks in. advance of any
maintenance work authorized by this permit, and such notification; shall include_
A. The number of the original permit.
& A statement. that no dimensional changes are proposed..
C. A copy of the original permit plans with cross -hatching indicating the area to be maintained; the
area to be used for spoil disposal, and the estimated amount of material to be removed. The
location, design and holding capacity of the spoil disposal site shall be approved by a representative
of the Division prior to the initiation of any maintenance dredging activities.
D. The date of map revision and the permittee's signature shown anew on the original plan.
General
1 S) The permittee and/or his or :her contractor shall meet with a representative of the Division prior to
Project initiation.
19) This permit shall not be assigned, transferred, sold, or otherwise disposed of to a third party without the
written approval of the Division of Coastal Management.
20) This permit does not authorize the interference with any existing or proposed Federal project, and the
permittee shall not be entitled to compensation for damage to the authorized structure or work, or injury
which may be caused from existing. or future operations undertaken by the United States in the. public
interest.
NOTE: Future: development of the permittee'sproperty may require a modification of this permit.
Contact a representative of the Division. at [phone] prior to the commencement of any
such activity for this determination. The permittee is Ru-ther advised that many non -water
dependent activities are not authorized within 30 feet of the normal high water level.
NOTE: This permit does not eliminate the need to obtain any additional state, federal or. local permits,
approvals or authorizations that may be required.
NOTE: The U.S. Army Corps of Engineers assigned the proposed project Action ID No. SAW-2018-
0041.1.
