as authorized by the State of North Carolina, Department of Environment,
and Natural Resources and the Coastal Resources Commission for development
in an area of environment concern pursuant to Section 113A-118 of the
General Statues, "Coastal Area Management"

Permit Number
Issued to Charles L. Madison, authorizing development in Estuarine Shoreline at 1432 Milligan Lane,
Shallotte NC 28470 as requested In the permittee's application, dated 30-1un-03. This permit, Issued on
06-Aug-03, is subject to compliance with the application and site drawing (where consistent with the
permit), all applicable regulations and special conditions and notes set forth below. Any violation of these
terms may subject permittee to a fine, imprisonment or civil action, or may cause the permit to be null and
void.
This permit authorizes
(1) A silt fence or natural buffer must be positioned between any land disturbance and adjacent waters or
wetlands. In no case, shall erosion of disturbed areas result in deposition of this material into adjacent
waters or wetlands.
(2) All proposed development and associated construction must be done in accordance with the permitted
work plat drawings(s) (Single Family Residence] submitted on 30-Jun-03.
(3) All construction must conform to the N.C. Building Code requirements and all other local, State and
Federal regulations, applicable local ordinances and FEMA Flood Regulations.
(4) Any change or changes in the plans for development, construction, or land use activities will require a
re-evaluation and modification of this permit.

This permit action may be appealed by the permittee or other
qualified persons within twenty (20) days of the issuing date. From
the dale of an appeal, any work conducted under this permit must
cease until the appeal is resolved.
This permit must be on the project site and accessible to the permit
officer when the project is inspected for compliance.
Any maintenance work or project modification not covered under
this permit, require further written permit approval.
All work must cease when this permit expires on December 31,
Zoos.
In issuing this permit it is agreed that this project is consistent with
the local Land Use Plan and all applicable ordinances.
This permit may not be transferred to another party without the
written approval of the Division of Coastal Management.

(5) All development projects, proposals, and designs shall limit the construction of impervious
surfaces and areas not allowing natural drainage to only so much as is necessary to adequately
service the major purpose or use for which the lot is to be developed.
(6) No impermeable surfaces shall be allowed over any functional part of the septic tank system.
(7) All unconsolidated material resulting from associated grading and landscaping shall be retained
on site by effective sedimentation and erosion control measures.
(8) All disturbed areas shall be vegetatively planted and mulched within fourteen days of construction
completion.
(9) The first floor elevation must be in compliance with Federal Flood Insurance Rate Map Elevations.
(10) Pursuant to 15 NCAC, Subchapter 7J.0406(b), this permit may not be assigned, transferred, sold or
otherwise disposed of to a third -party unless done so by the Director of the Division of Coastal
Management.
(11) A building permit and zoning compliance must be obtained prior to any construction.
(12) The applicant has effectively demonstrated, through innovative construction design, that the amount
of impervious surface areas may be exceeded in a way that runoff water will be managed and the AEC
protect so as to allow 41 percent of impervious areas within AEC.
(13) Upon completion of construction, prior to issuance of a Certificate of Occupancy, a certification must
be received from an appropriate designer for the system installed certifying that the permitted system
has been installed in accordance with this permit, the approved plans and specifications must be noted
supporting documentation. Any deviations from the approved plans and specifications must be noted
on the Certification. A permit modification may be required for those deviations.
(14) The landowner or permittee shall at all times provide the operation and maintenance necessary to assure
the permitted stormwater system functions at optimum efficiency. The landowner or permittee shall
maintain the permitted stormwater system for the life of the project.
(15) A copy of this permit shall be posted or available on site. Contact this office for a final inspection at
completion of work.
