Any violation of these terms may subject permittee to a fine, imprisonment or civil action, or may cause the permit to be null and void. This permit authorizes: SINGLE FAMILY RESIDENT, GRAVEL PERVIOUS DRIVEWAYS AND STORMWATER RETENTION DITCHES. (1) All proposed development and associated construction must be done in accordance with the permitted work plat drawings (survey) dated RECEIVED June 19, 2019. (2) All construction must conform to the N.C. Building Code requirements and ail other local, State and Federal regulations, applicable local ordinances and FEMA Flood Regulations. A building permit is required prior to construction. A elevation certificate is required prior to final inspection. (3) Any change or changes in the plans for development, construction, or land use activities will require a re-evaluation and modification of this permit (4) A copy of this permit shall be posted or available on site. (Additional Permit Conditions on Page 2) This permit action may be appealed by the perrnittee a other qualified persons within twenty (20) days of the issuing date. From the date of an appeal, any work conducted under this permit must cease until the appeal is resolved. This permit must be on the project site and accessible to the permit officer when the project is inspected for compliance. Any maintenance work or project modification not covered under this pearril, require further written pemrit approval. The total impervious on the lot will be 1,615 (6) Unless specifically allowed in 15 NCAC 07H. 0209(d)(10), and shown on the permitted plan drawing, all developmentfconstruction shall be located a distance of 30 feet landward of normal high water level. (7) All unconsolidated material resulting from associated grading and landscaping shall be retained on site by effective sedimentation and erosion control measures. Prior to any land -disturbing activities, a barrier line of filter cloth must be installed between the land disturbing activity and the adjacent marsh or water areas until such time as the area has been Properly stabilized with a vegetative cover. (8) The (4) 9'x20' parking spaces shall remain accessible for the life of the structure. (9) No grading is permitted within the 30' buffer from the normal high water. Any land disturbance must be contoured to prevent additional stormwater runoff to the adjacent marsh. This area shall be immediately vegetatively stabilized and must remain in a vegetated state. (10)AII other disturbed areas shall be vegetatively stabilized (planted and mulched) within 14 days of construction completion. (11)Areas below the structure which are below the lowest floor shall not be designed .or used for human habitation, but shall only be used for parking of vehicles, building access or limited storage of maintaince equipment. Flood Vents shall be installed in accordance with FEMA regulations and Holden Beach Town Ordinance. Access to the enclosed area shall be limited to a (standard exterior door), or entry to the living area (stairway or elevator). The inferior porfion of such enclosed area shall not be finished or parlitioned into separate rooms, except to enclose storage areas. Flood vents and break a way walls are required all areas below the Flood zone. (12)Concarets slab shall be an elevation of 5A9. (13)No fill material is being pemitted on this lot, area in right of way shall remain the same elevation as existing. (14)The stormwater ezflow trence drain shall not be driven -over with vehicles unless verified and submitted by the stormwater engineer that design the system. The stormwater shall be inspected by the planning & inspections department once installed prior to being covered up. There is additional information required by Towns Zoning Ordinance and CAMA regulations for a complete application. The information needed is the proposed concrete slab to be placed on the stormwater plan and that plan stamped by a licensed stormwater engineer and signed. Also on the stormwater plan CAMA requires the total impervious coverage calculation within the 75 foot AEC. Pursuant to NCGS 113A-121(b), the undersigned hereby gives notice to the applicant once the required information is submitted that for good cause, and in order to properly consider all information necessary to making a decision on this permit application, the time period within which a final decision shall be made has been extended an additional twenty-five (25) days.
