the Coastal Resources Commission for development in an area of environment concern
pursuant to Section 113A-118 of the General Statutes, "Coastal Area Management'
Issued to Sheila and Paul Schreyer authorizing development in the Estuarine Shore Area of Environmental Concern
(AEC) at 3824 Courtenay Drive SW in Supply NC 28462 as requested in the permittee's application package, dated
06-Mar-19 and received complete 22-Mar-19. This permit, issued on 02-Apr•19, is subject to compliance with the
application and site drawing (where consistent with the permit), all applicable regulations and special conditions and notes
set forth below. Any violation of these terms may subject permittee to a fine, imprisonment or civil action, or may cause the
permit to be null and void.
This permit authorizes: Single Family Residence, Covered Porch and Open Uncovered Deck.
(1) All proposed development and associated construction must be done in accordance with the permitted work plat drawings(s)
dated received on 22-Mar-19.
(2) All construction must conform to the N.C. Building Code requirements and all other local, State and Federal regulations,
applicable local ordinances and FEMA Flood Regulations.
(3) Any change or changes In the plans for development, Construction, or land use activities will require a re-eveluallon and
modification of this permit.
(4) A copy of this permit shall be posted or available on site. Contact this office at (910) 263.2034 for a final Inspection at
completion of work.

The Wilmington Regional Office received a complete Stormwater Management Permit Application for SeeScape
at Holden Plantation on January 16, 2008. Staff review of the plans and specifications has determined that the
project, as proposed, will comply with the Stormwater Regulations set forth in Title 15A NCAC 2H.1000. We are
forwarding Permit No. SW8 990913 Modification, dated April 2, 2008, for the construction of the subject project.
This permit shall be effective from the date of Issuance until April 2, 2018, and shall be subject to the conditions
and limitations as specified therein. Please pay special attention to the Operation and Maintenance requirements
In this permit. Failure to establish an adequate system for operation and maintenance of the stormwater
management system will result In future compliance problems.
Prior to the development of any future area or additional phase noted on the approved plans, a permit
modification must be submitted to and approved by the Division. The modification must demonstrate that the built -
upon area does not exceed the maximum amount permitted under this permit and that it meets the Intent of low
density development per the stormwater rules and available guidance.
If any parts, requirements, or limitations contained in this permit are unacceptable, you have the right to request
an adjudicatory hearing upon written request within sixty (60) days following receipt of this permit. This request
must be in the form of a written petition, conforming to Chapter 1508 of the North Carolina General Statutes, and
filed with the Office of Administrative Hearings, P.O. Drawer 27447, Raleigh, NC 27611-7447. Unless such
demands are made this permit shall be final and binding.

construction, operation and maintenance of a 25% low density subdivision with a curb
outlet Swale system, three infiltration trenches, an one infiltration basin In compliance
wkh the provisions of Title 15A NCAC 2H .1000 (hereafter referred to as the
"stormwater rules) and the approved stormwater management plans and specifications
and other supporting data as attached and on file with and approved by the Division of
Water Quality and considered a part of this permit.
This permit shall be effective from the date of issuance until April 2. 2018 and shall be
subject to the following specified conditions and limitations:
I. DESIGN STANDARDS
1. This permit is effective only with respect to the nature and volume of stormwater
described in the application and other supporting data.
2. The overall tract built -upon area percentage for the project must be maintained at
25% per the requirements of Section .1005 of the stormwater rules. This project
is permitted for a maximum of 5,343,849 square feet of BUA.
3. Prior to the development of any future area or additional phase noted on the
approved plans, a permit modification must be submitted to and approved by the
Division. The modification must demonstrate that the built -upon area does not
exceed the maximum amount permitted under this permit and that it meets the !
intent of low density development per the stormwater rules and available
guidance.

4. The stormwater systems proposed to treat the runoff generated by the
designated high density areas have been approved for the management of ;3TON, HC
stormwater runoff as described in Section 1.10 on page 3 and Section 1.11 on
page 4 of this permit.

5. The subdivision is Permitted for 550 lots, each limited to that maximum amount of
built -upon area as indicated In Section 11.24 (f) of this permit.
6. The low density portions of this project will maintain a minimum 30 foot wide
vegetated buffer between all impervious areas and surface waters.
7. Within the low density portions of the permitted project, a curb outlet system is
proposed. Each designated curb outlet swale or 100' vegetated area shown on
the approved plan must be maintained at a minimum of 100' long, maintain 5:1
(H:V) side slopes or flatter, have a longitudinal slope no steeper than 5%, carry
the flow from a 10 year storm In a non -erosive manner, maintain a dense
vegetated cover, and be located in either a dedicated common area or a
recorded drainage easement.
8. No piping shall be allowed except those minimum amounts necessary to direct
runoff beneath an impervious surface such as a road or under driveways to
provide access to lots.
9. Runoff conveyances other than the curb outlet system swales, such as perimeter
ditches, must be vegetated with side slopes no steeper than 3:1 (H:V).
10. The following design elements have been permitted for the three Infiltration
trenches associated with this stormwater facility, and must be provided in the
system at all times.

12. Level spreaders shall be provided at the ends of all swales that discharges into
regulated wetlands and surface waters such that a sheet flow condition is
created. Additional passive measures may be required for projects within % mile
of SA waters.
11. SCHEDULE OF COMPLIANCE
1. No homeowner/lot owner/developer shall fill in, alter, or pipe any drainage feature
(such as swales) shown on the approved plans as part of the stormwater
management system without submitting a revision to the permit and receiving
approval from the Division.
2. The permittee is responsible for verifying that the proposed bunt -upon area for
the entire lot does not exceed the maximum allowed by this permit. Once the lot
transfer is complete, the built -upon area may not be revised without approval
from the Division of Water Quality, and responsibility for meeting the built -upon
area limit is transferred to the individual property owner.
3. If an Architectural Review Board or Committee is required to review plans for
compliance with the BUA limit, the plans reviewed must include all proposed
built -upon area. Any approvals given by the Board do not relieve the homeowner
of the responsibility to maintain compliance with the permitted BUA limit.
4. The Director may notify the permittee when the permitted site does not meet one
or more of the minimum requirements of the permit. Within the time frame
specified in the notice, the permittee shall submit a written time schedule to the
Director for modifying the site to meet minimum requirements. The permittee
shall provide copies of revised plans and certification in writing to the Director
that the changes have been made.
5. Approved plans and specifications for projects covered by this permit are
incorporated by reference and are enforceable parts of the permit.
6. Deed restrictions are Incorporated into this Permit by reference andPn6t be -D
recorded with the Office of the Register of Deeds prior to the sale of any lot.

7. The stormwater management system shall be constructed in its entirety,
veeggetated and operational for its Intended use prior to the construction of any
bu eupon surface.
8. The runoff from all built -upon area within the drainage area of the pocket of high
density permitted stormwater system must be directed into the permitted
stormwater control system.
9. The infiltration trenches 1 and 2 associated with the permitted stormwater system
must be operated with a 50' vegetated filter strip.
10. Within the low density portions of the permitted project, all roof drains shall
terminate at least 30 feet from the mean high water mark of surface waters.
11. The permittee shall submit all information requested by the Director or his
representative within the time frame specified in the written information request.
12. The issuance of this permit does not prohibit the Director from reopening and
modifying the permit, revoking and reissuing the permit, or terminating the permit
as allowed by the laws, rules, and regulations contained in Title 15A of the North
Carolina Administrative Code, Subchapter 2H.1000; and North Carolina General
Statute 143-215.1 et. al.
13. The permittee shall submit to the Director and shall have received approval for
revised plans, specifications, and calculations prior to construction, for any
modification to the approved plans, including, but not limited to, those listed
below:
a. Any revision to the approved plans, regardless of size.
b. Project name change.
C. Transfer of ownership.
d Redesign or addition to the approved amount of built -upon area.
e. Further subdivision, acquisition, or sale of all or part of the project area.
The Project area is defined as all property owned by the permittee, for
which Sedimentation and Erosion Control Plan approval or a CAMA Major
permit was sought.
f. Filling in, altering, or piping of any vegetative conveyance shown on the
approved plan.
14. The Director may determine that other revisions to the project should require a
modification to the permit.
15. All stormwater collection and treatment systems must be located in either
dedicated common areas or recorded easements. The final plats for the project
must be recorded showing all such required easements, in accordance with the
approved plans.
16. During construction, erosion shall be kept to a minimum and any eroded areas of
the system will be repaired immediately.
17. Upon completion of construction, prior to issuance of a Certificate of Occupancy,
and prior to operation of this permitted facility, a certification must be received
from an appropriate designer for the system installed certifying that the permitted
facility has been installed in accordance with this permit, the approved plans and
specifications, and other supporting documentation. Any deviations from the LIVLIU
approved plans and specifications must be noted on the Certification.

18. if the stormwater system was used as an Erosion Control device, it must be
restored to design condition prior to operation as a stormwater treatment device,
and prior to occupancy of the facility.
19. Unless otherwise shown, permanent seeding requirements for the stormwater
control must follow the guidelines established in the North Carolina Erosion and
Sediment Control Planning and Design Manual.
20. This permit shall become void unless the facilities are constructed in accordance
with the conditions of this permit, the approved plans and specifications, and
other supporting data.
21. The permittee shall at all times provide the operation and maintenance
necessary to assure that all components of the permitted stormwater system
function at optimum efficiency. The approved Operation and Maintenance Plan
must be followed in its entirety and maintenance must occur at the scheduled
intervals including, but not limited to:
a. Semiannual scheduled inspections (every 6 months).
b. Sediment removal.
c. Mowing and re -vegetation of side slopes.
d. immediate repair of eroded areas.
e. Maintenance of side slopes in accordance with approved plans and
specifications.
f. Debris removal and unclogging of structures, orifice, catch basins and
piping.
g. Access to all components of the system must be available at all times.
22. Records of maintenance activities must be kept by the permittee. The records will
indicate the date, activity, name of person performing the work and what actions
were taken.
23. Prior to transfer of the permit, the stormwater facilities will be inspected by DWQ
personnel. The facility must be in compliance with all permit conditions. Any
items not in compliance must be repaired or replaced to design condition prior to
the transfer. Records of maintenance activities performed to date will be
required.
24. Prior to the sale of any lot, the following deed restrictions must be recorded:
a. The following covenants are intended to ensure ongoing compliance with
State Stormwater Management Permit Number SW8 990913, as issued
by the Division of Water Quality under NCAC 2H.1000.
b. The State of North Carolina is made a beneficiary of these covenants to
the extent necessary to maintain compliance with the Stormwater
Management Permit.
C. These covenants are to run with the land and be binding on all persons
and parties claiming under them.
d. The covenants pertaining to stormwater may not be altered or rescinded
without the express written consent of the State of North Carolina, Division
of Water Quality.
e. Alteration of the drainage as shown on the approved plans may not take
place without the concurrence of the Division of Water Quality.

The maximum built -upon area per lot is outlined in Atachment "A" and
must be maintained for all lots to keep the overall low density classification
for this development. This allotted amount includes any built -upon area
constructed within the lot property boundaries, and that portion of the
right-of-way between the front lot line and the edge of the pavement. Built
upon area includes, but is not limited to, structures, asphalt, concrete,
gravel, brick, stone, slate, coquina and parking areas, but does not include
raised, open wood decking, or the water surface of swimming pools.
g. Filling in, piping or altering any designated 5:1 curb outlet swale or
vegetated area associated with the development Is prohibited by any
persons.
h. Filling in, piping or altering any 3:1 vegetated conveyances (ditches,
swales, etc.) associated with the development except for average
driveway crossings, is prohibited by any persons.
i. A 30' vegetated buffer must be maintained between all built -upon area and
the Mean High Water line of surface waters.
j. All roof drains shall terminate at least 30' from the Mean High Water mark.
k. Each designated curb outlet swale or 100' vegetated area shown on the
approved plan must be maintained at a minimum of 100' long, maintain
5:1 (H:V) side slopes or flatter, have a longitudinal slope no steeper than
5%, carry the flow from a 10 year storm in a non -erosive manner, maintain
a dense vegetated cover, and be located in either a dedicated common
area or a recorded drainage easement.
I. For those lots within CAMA's Area of Environmental Concern, where DCM
calculates a different maximum lot built -upon area, the governing lot BUA
shall be the most restrictive of the two amounts.
M. Built -upon area in excess of the permitted amount will require a permit
modification.
25, A copy of the recorded deed restrictions must be submitted to the Division within
30 days of the date of recording the plat, and prior to selling lots. The recorded
copy must contain all of the statements above, the signature of the Permittee, the
deed book number and page, and the stamp/signature of the Register of Deeds.
Ill. GENERAL CONDITIONS
This permit is not transferable to any person or entity except after notice to and
approval by the Director. In the event there is either a desire for the facilities to
change ownership, or there is a name change of the Permittee, a completed
"Name/Ownership Change Form" must be submitted to the Division of Water
Quality, signed by the parties involved, along with the applicable documents as
listed on page 2 of the form. The project must be in good standing with DWQ.
The approval of this request will be considered on its merits and may or may not
be approved.
The permittee is responsible for compliance with all of the terms and conditions VED
of this permit until such time as the Director approves the transfer request.

3. Failure to abide by the conditions and limitations contained in this permit may
subject the Permittee to enforcement action by the Division of Water Quality, In
accordance with North Carolina General Statute 143-215.6A to 143-215.6C.
4, The issuance of this permit does not preclude the Permittee from complying with
any and all statutes, rules, regulations, or ordinances, which may be imposed by
other government agencies (local, state, and federal), which have jurisdiction.
5. In the event that the facilities fail to perform satisfactorily, including the creation of
nuisance conditions, the Permittee shall take immediate corrective action,
Including those as may be required by this Division, such as the construction of
additional or replacement stormwater management systems.
6. The permit may be modified, revoked and reissued or terminated for cause. The
filing of a request for a permit modification, revocation and re -issuance or
termination does not stay any permit condition.
T Permittee grants permission to staff of the DWQ to access the property for the
purposes of inspecting the stormwater facilities during normal business hours.
8. Approved plans and specifications for this project are incorporated by reference
and are enforceable parts of the permit.
9. The permittee shall notify the Division in writing of any name, ownership or
mailing address changes at least 30 days prior to making such changes.
10. A copy of the approved plans and specifications shall be maintained on file by
the Permittee at all times.

The Owner shall be subject to the following conditions, limitations, and construction requirements for the installation of this
system
1 This authorization is effective only with respect to the specific design flow and facilities, and the nature and volume of
waste described in the Permit Application, and other supporting data.
2. This permit is not transferable and must be reapplied for at such time as ownership, or management changes
3. The designated repair area is to have no parking, driveways or other impervious material located on it. This
area is to be protected, reserved and maintenance in a natural state.
4. All subsequent owners of this property and sewage system, shall execute a contract between the owner and an
approved management entity before the operations permit is issued Management of the sewer system to be
provided at all times
5. All contracts between owners, management entity (Public or Private), or Health Dept shall be reviewed and approved
before being accepted.
6 The issuance of this permit does not preclude the permittee from complying with any and all statutes, regulations, or
ordinances which may be imposed by other government agencies which have jurisdiction, or any other permits issued
by this department.
7. This authorization shall be valid for 60 months from the date of issue. The authorization shall become invalid if the
information submitted in the application was falsified or changed, if the permit was based on inaccurate or incomplete
information, or if the designated site is aRered, or expiration of this authorization, a new application shall be submitted
to the BCHD with updated information, as may be applicable or requested
8. ALL APPLICABLE SETBACKS SHALL BE MAINTAINED, NO IRRAGATION SYSTEMS, STORM WATER PONDS
BASINS, SURFACE/SUBSURFACE DRAINAGE SHALL BE LOCATED IN THE AREA OF THE WASTEWATER
SYSTEM OR ITS REQUIRED SETBACKS.
INSTALLATION REQUIREMENTS
1. The installer shall be currently registered with this department.
2 The contractor shall be responsible for notification of the engineer and the BCHD for system inspection in stages as
required and prior to backfilling any portion of the system No portion of the system shall be backfllled or placed into
use without prior approval of the BCHD
3 The system shall be installed in accordance with the approved set of plans and specifications. Any deviation in site
modifications, plans, specification, layout, materials or other system component shall be approved by the design
engineer and the Brunswick County Health Department prior to installation of the system. Failure to do so may result
in delay or refusal of final approval of the system, and may render the Permit null and void
4 The system shall be installed in a timely manner and staged so as to avoid unnecessary exposure to weather.
5 The location and identification of all property lines. easements, water lines, and other appropriate utilities shall be the
responsibility of the installer.

1. An Operation Permit shall be issued by the BCHD prior to placing the system into use, or making any connections to
the system.
2. Prior to issuance of the Operation Permit
- the system shall be completed, installed, and tested in accordance with the approved design, induding proper
abandonment of tanks existing wells, and other components,
- final landscaping, water diversion devices, and vegetative cover requirements and pressure adjustment shall be
completed,
- the engineer or applicable responsible parties shall submit as -built plans to the BCHD, the plans shall reflect any
changes or alterations from the as -approved plans.
3 For systems that have been engineered, the design engineer shall certify in writing to the Brunswick County Health
Department, that he has inspected the installation and that it has been installed according to the approved plans and
specifications.
- Any necessary easements, plats, or documents shall be recorded with the Register of Deeds and copies
submitted to the BCHD,
- The owner and a Public Management Entity/Subsurface System Certified Operator shall execute a contract whicn
addresses all of the requirements for maintenance, monitoring, and reporting in Section .1961 and the
requirements of the Schedule of Operation and Maintenance to be a part of the Operation Permit for the facility
Provisions of said contract shall be in effect for as long as the system is in use.

agencies- The owners, the Environmental Heath Section RS or SS. general or building contractor, designer,

the completed but uncovered drain field before the tanks and pumps are complete, H weather issues may effect

2. Electrical Power is available from temporary Power pole or generator, 115 voles for the Control and 23o volts
for the pump. Some generators may require an extension cord with twist lock plugs
3. All pressure pipe connections are primed and glued, riser vent with insect screen is present and secure.
4. Control Boats or pressure bell is set to system specifications, mandold tum-up is marked for specified (read
Pressure setting or monometer is attached to the manifold clean-cut or lateral clean -out for LPP systems
5. Risers and non-grommeted tank connections and penetrations are sealed with mortar mix.
6. Conduit from user to control panel is sealed at both ends with non -hardening putty or sealer.
7. It junction box is used, it is mounted above grade, Junction boxes are ramr-tylht, not water proof.
8. Control panel is mounted securely to the building or post, control panel must be within 50 feet of and within
sight of riser to meet electrical code requirements for the pump disconnect means. Factory specsheet and instructions
are present with parcel, mark the model of the pump used and the float or level settings on the spec. sheet
