1. EXCAVATION This section not applicable
a.Amount of material to be excavated from below NHW or NWL in
cubic yards.
Approx. 10,750 CY (access channel) and approx. 2,500 CY (boat
basin). Material excavated for Living Shoreline will be redistributed
as backfill for marsh restoration. Approx. 300 CY of dredge
material will be necessary for additonal backfill to complete marsh
restoration.
b.Type of material to be excavated.
Mixture of sands and silts. Not suitable for beach disposal.
c.(i) Does the area to be excavated include coastal wetlands/marsh
(CW), submerged aquatic vegetation (SAV), shell bottom (SB),
or other wetlands (WL)? If any boxes are checked, provide the
number of square feet affected.
CW SAV SB
WL None
(ii)Describe the purpose of the excavation in these areas:
Material to be used as backfill for marsh restoration.
d.High-ground excavation in cubic yards.
300 CY
2.DISPOSAL OF EXCAVATED MATERIAL This section not applicable
a.Location of disposal area.
Upland disposal area on nearby White Point (12,950 CY)
Marsh restoration area (300 CY)
b.Dimensions of disposal area.
2.74 acres - White Point Material Placement Area
20,706 sq ft (.5 Ac) - Marsh restoration area
c.(i) Do you claim title to disposal area?
Yes No NA
(ii)If no, attach a letter granting permission from the owner.
d.(i) Will a disposal area be available for future maintenance?
Yes No NA
(ii)If yes, where?
White Point Material Placement Area
e.(i) Does the disposal area include any coastal wetlands/marsh
(CW), submerged aquatic vegetation (SAV), shell bottom (SB),
or other wetlands (WL)? If any boxes are checked, provide the
number of square feet affected.
CW SAV SB
WL None
f.(i) Does the disposal include any area in the water?
Yes No NA
(ii)If yes, how much water area is affected?
20,706 sq ft (0.5 Ac) will receive a total of approx. 300 CY.
Additional material will be redistributed to achieve contours
shown on plans.

3. SHORELINE STABILIZATION This section not applicable
(If development is a wood groin, use MP-4 – Structures)
a. Type of shoreline stabilization:
Bulkhead Riprap Breakwater/Sill Other:
b. Length: 1,885 ft
Width: 20-ft (425 LF); 28-ft (1,310 LF); 4-ft (150 LF)
c. Average distance waterward of NHW or NWL: 20-ft (425 LF); 28-ft
(1,310 LF); 33-ft (150-LF) Wave attenuator
d. Maximum distance waterward of NHW or NWL: 33-ft
e. Type of stabilization material:
Granite Rock ; concrete sections attached to composite piles for
wave attenuator.
f. (i) Has there been shoreline erosion during preceding 12
months?
Yes No NA
(ii) If yes, state amount of erosion and source of erosion amount
information.
g. Number of square feet of fill to be placed below water level.
Bulkhead backfill 300 CY over 20,706 sq. ft. Riprap
Breakwater/Sill 45,780 Other
h. Type of fill material.
i. Source of fill material.
Dredge spoil
4. OTHER FILL ACTIVITIES This section not applicable
(Excluding Shoreline Stabilization)
a. (i) Will fill material be brought to the site? Yes No NA
If yes,
(ii) Amount of material to be placed in the water
(iii) Dimensions of fill area
(iv) Purpose of fill
b. (i) Will fill material be placed in coastal wetlands/marsh (CW),
submerged aquatic vegetation (SAV), shell bottom (SB), or
other wetlands (WL)? If any boxes are checked, provide the
number of square feet affected.
CW SAV SB
WL None
(ii) Describe the purpose of the fill in these areas:
5. GENERAL
a. How will excavated or fill material be kept on site and erosion
controlled?
Material will be placed on White Point and contained with silt
fencing and a control berm. No material will be placed within 30
feet of MHW except the approx. 300 CY required for backfill for
the marsh restoration.
Approx. 3,060 CY will be redistributed or re-graded for sill
installation and marsh habitat enhancement.
b. What type of construction equipment will be used (e.g., dragline,
backhoe, or hydraulic dredge)?
Mechanical dredge (excavator) using a bucket, with a barge; also
most likely a bobcat or small grading equipment in marsh
restoration area. Excavator will also be used to place the
granite rock. Crane mounted on a barge may be used to place
pilings and wave attenuator.
c. (i) Will navigational aids be required as a result of the project?
Yes No NA
(ii) If yes, explain what type and how they will be implemented.
Temporary aids will be used during contructions; fixed lighted
aids to navigation will be installed on reef or sill structure.
d. (i) Will wetlands be crossed in transporting equipment to project
site? Yes No NA
(ii) If yes, explain steps that will be taken to avoid or minimize
environmental impacts.
Equipment will be lifted over wetlands for dredge project;
otherwise wetlands will be enhanced for marsh restoration.
