as authorized by the State of North Carolina,Department of Environment,
and Natural Resources and the Coastal Resources Commission for development
in an area of environment concern pursuant to Section 113A-118 of the
General Statutes, "Coastal Area Management"
Issued to Paul Fornwald, authorizing development in the Estuarine Shoreline (AEC) at 930 Broadway Street, in Surf
City, Onslow County, as requested in the permittee's application, received by DCM on May 24, 2012. This permit,
issued on June 12, 2012, is subject to compliance with the application and site drawing (where consistent with the
permit), all applicable regulations and special conditions and notes set forth below. Any violation of these terms may
subject permittee to a fine, imprisonment or civil action, or may cause the permit to be null and void.
This permit authorizes: Construction of a single-family residence.
(1) All proposed development and associated construction must be done in accordance with the permitted work plat
drawings(s)dated received on May 24,2012.
(2) All construction must conform to the N.C. Building Code requirements and all other local, State and Federal regulations,
applicable local ordinances and FEMA Flood Regulations.
(3) Any change or changes in the plans for development, construction, or land use activities will require a re-evaluation and
modification of this permit.
(4) A copy of this permit shall be posted or available on site. Contact this office at 910-796-7221for a final inspection at
completion of work.

This permit action may be appealed by the permittee or other qualified persons
within twenty (20) days of the issuing date. From the date of an appeal, any
work conducted under this permit must cease until the appeal is resolved. This
permit must be on the project site and accessible to the permit officer when the
project is inspected for compliance. Any maintenance work or project Jason Dail

(5) The amount of impervious surface shall not exceed 30%of the lot area within 75 feet of Normal High Water
(Estuarine Shoreline Area of Environmental Concern). In this case,895 sq. ft. of impervious coverage is permitted
within 75'of the Normal High Water level.
(6) Unless specifically allowed in 15A NCAC 07H. 0209(d)(10), and shown on the permitted plan drawing, all
development/construction shall be located a distance of 30 feet landward of Normal High Water. No portion of the
roof overhang shall encroach into the 30 ft. buffer.
(7) All unconsolidated material resulting from associated grading and landscaping shall be retained on site by effective
sedimentation and erosion control measures. Prior to any land-disturbing activities, a barrier line of filter cloth must
be installed between the land disturbing activity and the adjacent marsh or water areas, until such time as the area
has been properly stabilized with a vegetative cover.
(8) Any proposed for grading within the 30' buffer from the Normal High Water must be contoured to prevent additional
stormwater runoff to the adjacent marsh. This area shall be immediately vegetatively stabilized, and must remain in
a vegetated state.
(9) All other disturbed areas shall be vegetatively stabilized (planted and mulched)within 14 days of construction
completion.

Dear Mr. Fornwald:
The Division of Coastal Management's Wilmington Regional office received a CAMA Minor Permit application from you on
May 21, 2012, requesting approval for development activities at 18°7'930 Broadway Street, in Surf City. In reviewing your application,
we have discovered that additional information is needed to complete the review process.Accordingly, I am requesting that you submit
the following additional information to this office:
1. On page 1 of the application, please correct number(2) Coastal Shoreline AEC.As indicated on the permit application
and site drawings,the proposed house will be 39'x 35',from drip edge to drip edge,giving a total footprint of 1,365 sq.
ft. of impervious coverage. In addition,the proposed driveways will account for some impervious coverage as the
Division of Water Quality only allows partial credit for pervious concrete(40%-60%depending on construction method).
The application and survey will need to be revised to account for all impervious coverage on the property,taking into
account the%credit given for the pervious driveway.
2. If the proposed development will create impervious greater than 30%within the AEC,you will need to provide a plan for
stormwater collection and/or treatment.
In accordance with the Department of Environment and Natural Resources regulations,we note that the application,as
submitted on May 21, 2012, is incomplete for processing. Upon resubmission of a complete application, a local decision will be made
in 25 days, provided this period is not extended as provided by law. Please contact me at 910-796-7221 if you have any questions.

agency authorized to issue CAMA permits in areas of environmental concern, hereby gives NOTICE
that on May 24, 2012 Paul R. Fornwald applied for a CAMA permit to remove the existing structure,
and construct a new single family dwelling at 930 Broadway St., adjacent to a man-made canal, in
