Issued by WiRO SC 11-15 Surf City Permit Number CAMA MINOR DEVELOPMENT PERMIT as authorized by the State of North Carolina,Department of Environment, and Natural Resources and the Coastal Resources Commission for development In an area of environment concern pursuant to Section 113A-118 of the General Statutes,"Coastal Area Management' Issued to Steven and Cecilia Gregory, authorizing development in the Estuarine Shoreline Area of Environmental Concern (AEC), at 7053 70 Street, in Surf City, Onslow County as requested in the permittee's application, dated September 20, 2011. Any violation of these terms may subject permittee to a fine, imprisonment or civil action, or may cause the permit to be null and void. This permit authorizes:The construction of a single-family residence. (1) All proposed development and associated construction shall be undertaken in accordance with the work plat drawings(s) (4),dated 9/8/2011 (Boundary Survey received on 10/19/2011)and 9/12/2011 (Page 1,2 and 3 received 10/19/2011). (2) All construction must conform to the N.C. Building Code requirements and all other local, State and Federal regulations, applicable local ordinances and FEMA Flood Regulations. (3) Any change or changes in the plans for development, construction, or land use activities will require a re-evaluation and modification of this permit. (7) All unconsolidated material resulting from associated grading and landscaping shall be retained on site by effective sedimentation and erosion control measures. Prior to any land-disturbing activities,a barrier line of fitter cloth must be installed between the land disturbing activity and the adjacent marsh or water areas, until such time as the area has been properly stabilized with a vegetative cover. (8) Any proposal for grading within the 30'Coastal Shoreline Buffer shall be contoured to prevent additional stormwater runoff into the adjacent marsh/waterbody. This area shall be immediately vegetatively stabilized,and must remain in a vegetated state. (9) All other disturbed areas shall be vegetatively stabilized(planted and mulched)within 14 days of construction completion. In reviewing your application, we have discovered that additional information is needed to complete the review process. Accordingly, I am requesting that you submit the following information to this office: 1. Please provide the original signed certified mail receipts from the two adjoining property owners. 2. Please complete and have the owner sign the Agent Authorization agreement form provided herein. 3. Please provide a scaled copy of the work plan drawing. The drawing submitted with the application package shows a 1"=30' scale; however, the 8.5"x 11"drawing does not measure at 1"= 30'. 4. Please indicate whether or not the proposed "10'x 18'deck" will be covered. Additionally, the proposed deck is only shown on the "Boundary Survey & Preliminary Plot Plan"for Steven and Cecilia Gregory, not the remaining drawings. Please make sure that all of the attached drawings are consistent with one another. 5. Please number all of the attached workplan drawings. 6. On the "Boundary Survey& Preliminary Plot Plan"for Steven and Cecilia Gregory drawing, you note that the proposed home will be constructed on pilings; however, the remaining drawings (the typical section plans) show this unit being constructed on a crawl space with a basement. Again, if you plan to submit multiple drawings please make sure they are consistent with one another. 7. Please provide a copy of the sewer approval letter, or Improvement Permit, from Pender County/Surf City authorizing the septic system upgrade, if applicable. If an expansion/upgrade permit is not applicable, please identify the reason why and provide supporting documentation. 8.
