Statement of Compliance and Consistency
This permit is subject to compliance with this application, site drawing and attached general and specific conditions. Any
violation of these terms may subject the permittee to a fine or criminal or civil action; and may cause the permit to become
null and void.
This permit must be on the project site and accessible to the permit officer when the project is inspected for compliance. The
applicant certifies by signing this permit that 1) prior to undertaking any activities authorized by this permit, the applicant will
confer with appropriate local authorities to confirm that this project is consistent with the local land use plan and all local
ordinances, and 2) a written statement or certified mail return receipt has been obtained from the adjacent riparian
landowner(s) .
The State of North Carolina and the Division of Coastal Management, in issuing this permit under the best available
information and belief, certifythatthis project is consistentwith the North Carolina Coastal Management Program.

property at 2216 Highway 24, Newport:

➢ A copy of a current survey, with an accompanying blow up of the waterfront and dock area,
showing the setbacks from adjacent properties.

Allen Johnson. Note that he listed his mailing address in Smithfield and not his address on
Highway 24
Please note that we twice provided the neighbor to the west of us, at 2220 Highway 24, Newport for
signature but could not get him to sign and return it. This owner lives in Burlington, so we sent the
letter, survey, blow up of the waterfront and dock area, and Adjacent Riparian Property Owner
Statement to that address, and received no response (about 6 weeks ago). And we subsequently sent
the same materials through this property owner's realtor (this lot is for sale) — who assured us she
would get a response — but no response was given. This was about 3 weeks ago.
We are asking for your help in moving our request for a CAMA permit forward. Please call if you have
any questions at all.

Description of Proposed Dock Rebuilt for 2216 Highway 24, Newport, NC
As owner of 2216 Highway 24, Newport, NC, I plan to rebuild the dock located there which is currently in
disrepair. I plan to keep the same configuration, use the same pilings, and simply replace the framing
materials and decking.
Attached is a general survey of this property, along with an enlarged view of the waterfront showing the
current and planned location of the dock (which are the same). The near end/entrance of the dock is
roughly 45' away from the property lines on either side (east and west), and the far end of the dock (the
platform) is closer to 35' of the property lines on either side. Setback requirements indicate that my
dock should be a minimum of 15' away from any adjacent property line, so we are well within that limit.
I indicated "NA" (not applicable) for the waiver section of this document.
