violation of these terms may subiect the permiftee to a fine or criminal or civil action; and may cause the permit to become
null and void.
This permit must be on the proiect site and accessibletothe permit oflicerwhen the proiect is inspected for compliance. The
applicant certifies by signing this permit that I ) prior to undertaking any activities authorized by this permit, the applicant will
confer with appropriate local authorities to confirm that this proiect is consistent with the local land use plan and all local
ordinances, and 2) a written statement or certified mail return receipt has been obtained from the adiacent riParian
landowner(s) .

DESCRIPTION
We propose to install a new wooden retaining wall. Wall will be 40' long, in an
approximate 12'x 16' U-shape, to form a patio area along the slope down to the
water. This will include an upper and a lower set of 4'wide steps.
We propose to install a new concrete pier consisting of a 5'wide x 250' long
walkway connecting to an 18' wide x 20' long platform.
We propose to install a new L2.5' wide x 16.5' long gazebo roof. This will be a hip
roof having a pitch of no greater than 6/12-
We propose to install four (4) new 10" x 30' long pilings for a future boat lift.
