River Basin rules.
Statement of Compliance and Consistency
This permit is subject to compliance with this application, site drawing and attached general and specific conditions. Any
violation of these terms may subject the permittee to a fine or criminal or civil action; and may cause the permit to become
null and void.
This permit must be on the project site and accessible to the permit officer when the project is inspected for compliance. The
applicant certifies by signing this permit that [)prior to undertaking any activities authorized by this permit, the applicant will
confer with appropriate local authorities to confirm that this project is consistent with the local land use plan and all local
ordinances, and 2) a written statement or certified mail return receipt has been obtained from the adjacent riparian
landowner(s) .
The State of North Carolina and the Division of Coastal Management, in issuing this permit under the best available
information and belief, certifythatthis project is consistentwith the North Carolina Coastal Management Program.

Thanks for your assistance this morning.
Attached is a pdf of our sketch/plot plan for our property and the proposed dock at the above address. I think
I have set out all the information called for in the online instructions.
Also attached are the signed adjacent riparian owners' consent statements. The property immediately east of
our lot is owned by Alton and Kathryn Hedgepeth. They very recently purchased the property (previously
owned by FOG, LLC), but I think the tax records are updated to reflect their ownership. The second statement is
signed by Harlow Halliday. The owner of record is Ruth T. Halliday, who passed away in February of this
year. Harlow Halliday indicated to me that the property was devised to him as the sole owner according to
terms of Mrs. Halliday's will. I wasn't familiar with the specifics of Mrs. Halliday's will when 1 prepared the
form for signatures, so the two other parties, siblings of Harlow Halliday, did not sign, and their names were
crossed out by Mr. Halliday. I also think he is the executor of the Ruth Halliday estate. If any of this requires
verification, I'm happy to provide that.
Let me know if anything is missing or unclear on our sketch plan. While 1 imagine you know this area well, we
have been told that the GPS instructions to our address may be misleading. After taking Salty Shores Road to
the "tee" intersection with Salty Shores Point Drive, go right, to the end of the drive/road. A shed is in the
front - #191 is the last house on the left.
