violation of these terms may subie<t the permittee to a fine or criminal or cMl action; and may cause the perrnit to become

This permit must be on the proiect site and accessible to the permit ofiicer when the proiect is inspected for compliance. The
applicant cenifies by s@ing this permit thd I ) prior to undertaking any activities authorized by $is permit, the applicarft will
confer with appropriate local authorities to confirm that this prolect is consistert with the local land use plan and all local
ordinances, and 2) a written statement or certified mail return receipt has been obtained from the adiacent riparian

The State of North Carolina and the DMsion of Coastal Management, in issuing this permit under the best available
information and beliel ceftiry that dris proiect is consistent with the Nordr Carolina Coastal Management Program.

DESCRIPTION
We propose to install a new concrete pier extension consisting of a 5'wide x 54'
long walkway extending waterward off of the existing pier platform'
we propose to install four (4) new 10" x 30' long pilings for future boat lift.
We propose to pull up and reset four (a) right side existing lift pilings along side of
the new walkway extension. Remove what is remaining of the existing lift.
Level out lifted pilings on existing pier platform as best as possible' This will
include pulling four (4) pilings on the left side of the platform and resetting in a
new configuration for a future kayak platform to be built by others'
