This Permit is subiect to compliance with this application, site drawing and attached general and specific conditions. Any
violation of these terms may subject the permittee to a fine or criminal or civil action; and may cause the perrnit to become
null and void.
This Permit must be on the proiectsite and accessible to the permit oflicerwhen the prolect is inspected forcompliance. The
applicant certifies by signing this permit that I) prior to undertaking any activities authorized by this permit, the applicant will
confer with aPpropriate local authorities to confirm that this proiect is consistent with the local land use plan and all local
ordinances, and 2) a written statement or certified mail return receipt has been obtained from the adjacent riparian
landowner(s) .
The State of Nonh Carolina and the Division of Coastal Management, in issuing this permit under the best available
information and belief, certirythatthis project is consistentwith the North Carolina Coastal Management Program.

Mr. Davenport, please see attached info for permit request for the construction of pier and bulkheads we wish to build
at 221 Jones street.
David Windberry has agreed to give me an easement to build pier back to original length along his property shoreline.
Will that need to be a recorded easement in the county office or just an written agreement between David and myself?
Please advise on that matter to what you will need.
Should any questions arise, please call me at cell number listed below.
lf there is any way to get this permit moving quickly, it would be greatly appreciated as I have been told I have only 6
months to live as I have asbestos cancer in my right lung and want to have allthis work done before I move on in my
next venture of life. My many thanks if this can happen quickly.
