This permit is subject to compliance with this application, site drawing and attached general and specific conditions. Any
violation of these terms may subject the permittee to a fine or criminal or civil action; and may cause the permit to become
null and void.
This permit must be on the project site and accessible to the permit officer when the project is inspected for compliance. The
applicant certifies by signing this permit that I) prior to undertaking any activities authorized by this permit,the applicant will
confer with appropriate local authorities to confirm that this project is consistent with the local land use plan and all local
ordinances, and 2) a written statement or certified mail return receipt has been obtained from the adjacent riparian
landowner(s).

On behalf of Linda & David Kanoy, I would like to request a CAMA General Permit for the property
located at 811 Salt Water Lane, Supply, Brunswick County, NC. Please find enclosed the $200 fee
check, permit drawings (two sheets) and supporting materials. The applicant is requesting to build a
fixed pier, platforms, and floating dock. Adjacent riparian landowners have been notified
and provided the attached signed the property owner statements. Please note: the applicants also
own the property at 819 Salt Water Lane. It is not correctly reflected on the Brunswick County tax
site so we included the deed for this property at the end of this package.
